## Current state

Newly posted toner-adjunct results show no clear descriptive advantage alongside hydroquinone and sunscreen. The formulation and comparative statistics are missing; vitiligo programs retain their separate development status.

Status: active. Selective baseline reviewed **September 12, 2026**. Worldwide human studies at all phases; identify U.S. regulatory status separately.

## Questions to follow

- Which therapies improve melasma, post-inflammatory hyperpigmentation (PIH) and solar lentigines, with durable benefit after stopping?
- Track patient-visible improvement, blinded assessment, relapse, irritation and procedure-induced pigment changes, including results across skin tones.
- Keep **vitiligo repigmentation** as a separate subtrack: replacing lost pigment and treating autoimmune disease are different objectives from fading unwanted hyperpigmentation.
- Include well-designed human studies of topical formulations and procedures; product launch, biomarker change and clinical efficacy are separate events.

## Programs and next milestones

#### Posted hyperpigmentation toner trial shows no clear added separation

**Results first posted October 13, 2026; actual completion February 16.** NCT07136766, Good Molecules, randomized 49 adults to a brightening toner or placebo;47 completed and were analyzed (23/24, all women, mean age 57.7). Eligibility includes facial hyperpigmentation/melasma and generic 4% hydroquinone for up to 10 weeks; SPF50 is part of the regimen. The active toner ingredients and exact product were not disclosed, preventing a formulation-specific interpretation.

At 18 weeks, skin lightness L* changed 50.9 → 53.1 with toner and 56.1 → 58.2 with control: +2.2 versus +2.1 units. Expert pigmentation scores (lower better) changed 5.7 → 4.8 versus 5.8 → 4.6: −0.9 versus −1.2. Automated brown-feature scores (higher worse) increased 87.7 → 100.7 versus 93.4 → 108.1; these are software scores, not spot counts. Baseline lightness/race distribution differed. Descriptive arithmetic is not a statistical superiority/noninferiority test: no between-arm confidence intervals or p-values were posted.

**Safety:** No deaths/serious events among the 47 analyzed;1/23 active recipients developed comedones/pustules, while control 1/24 had unrelated rotator-cuff repair. One participant per arm withdrew for personal reasons. This small, short cosmetic-adjunct study establishes no drug approval, durable pigment normalization or general rare-harm safety. **Next:** disclose formulation and full comparative analysis; no date.

#### Povorcitinib adds a separate registered Chinese Phase 3 vitiligo trial

**Source date:** October 8, 2026 first registry posting.

NCT07865312/Povor-03-001, sponsored by Dermavon, plans 174 Chinese adults with nonsegmental vitiligo suitable for systemic therapy. Participants will be randomized 1:1 to oral povorcitinib phosphate 30 mg daily or placebo for 52 weeks, followed by four-week safety observation. The primary endpoint is F-VASI75 at week 52; body VASI, visible noticeability, patient global change and quality of life are secondary. No results or safety findings are posted.

**Status and limits:** Not yet recruiting. Estimated start November 2026; primary/final completion May/June 2028. These are registry estimates, not readout promises. This is a separate China program, not a new STOP-V1/V2 result or confirmation of a U.S. filing/approval. Vitiligo repigmentation differs from fading melasma/PIH, and response during treatment does not establish cure or long-term safety. **Next:** actual initiation, complete protocol/safety and controlled repigmentation outcomes, with no public release date verified.

[Original registry](https://clinicaltrials.gov/study/NCT07865312).

**Remaining gaps:** Read the current assigned topic and canonical scope/state. Named original registry records refreshed; all pages of broad ClinicalTrials.gov discovery for September 5–October 9 were retrieved (53 records, with incidental matches excluded). This honors the seven-day overlap from the saved September 12 full successful check.; Europe PMC original-literature discovery again returned HTTP 503. Targeted primary publisher, sponsor and conference searches were performed, but the worldwide paper/regulator/procedure and original-audio census remains incomplete. Preserve last_successful_check_utc.; STOP-V1/V2, DelVito and FXS5626 saved status retained; no new registry results. October 1 Viti-Up extension/phototherapy sponsor data deduplicated. Incyte actual EADV P2467/P1780/P2463 posters remain a retrieval gap.; No full melasma/PIH/solar-lentigine trial or regulator census. KT939 matched protocol/current cosmetic submission status still unresolved. October 8 maturational-hyperpigmentation characterization is not a treatment readout.

**Informal-source coverage:** October7 https://www.reddit.com/r/Vitiligo/comments/1x01ejq/11_yr_old_daughter_with_vitiligo_question_about/ includes an anonymous long-term povorcitinib participant anecdote and generalized safety assertions, not blinded outcome data or approval confirmation. October6 r/Scholar request for a 2025 tranexamic-acid review is a repost/access request, not fresh data. Saved melasma podcast remains descriptive with no new attributable forecast; full audio not reviewed.

#### Viti-Up reports continued repigmentation; small phototherapy substudy is exploratory

**Source/event date:** 2026-10-01. **Evidence:** Sponsor-only phase3 extension and randomized substudy.

AbbVie reported week-76 Viti-Up1/2 results for non-segmental vitiligo. Among continuously treated observed cases, F-VASI75 facial response was 55.1% and 47.4%; T-VASI50 body response was 42.5% and 40.1%. Week-76 denominators were 127 and 137. These are selected extension observations, not the original randomized-population placebo comparison.

An 84-adult substudy re-randomized participants with incomplete body repigmentation after week 48 to upadacitinib with or without twice-weekly narrowband UVB. Among prior upadacitinib users (27 per arm), week 76 F-VASI75 was 59.3% versus 37.0%, and T-VASI50 63.0% versus 33.3%. No confidence intervals or significance tests were supplied for these comparisons, so numerical separation should not be called proven superiority. The release reports no new safety signals without a numerical adverse-event table; systemic JAK warnings still matter. U.S. vitiligo review is ongoing; current EU product information includes eligible patients aged 12+. #### Delgocitinib facial-vitiligo study newly registered after enrollment

**Source date:** 2026-09-15. **Event date:** 2026-09-15. **Evidence:** trial-registry registration.

DelVito adds a small investigator-led human study of topical delgocitinib for facial vitiligo; no results are posted. NCT07820098, Gentofte Hospital: Phase 2, open-label single group, 12 actually enrolled participants. Record lists active, not recruiting, an actual April 8, 2026 start and first posting September 15. The primary endpoint is facial VASI change after 24 weeks; quality of life and adverse events are also assessed. Primary completion estimated April 2027; final completion January 2028.

**Limitations:** Retrospective registration and no control arm; changes cannot establish a causal treatment effect. Vitiligo repigmentation differs from treating melasma or reducing unwanted pigment.

**Safety:** Adverse-event frequency, duration and severity are planned endpoints; no safety results posted.

**U.S. status:** This record does not establish a U.S. vitiligo indication or availability for this use.

**Next milestone:** April 2027 primary completion and January 2028 final completion are registry estimates, not promised public results.

[DelVito primary registry](https://clinicaltrials.gov/study/NCT07820098).

**Coverage:** Searched vitiligo, melasma, PIH and skin-tone therapies; newly matched original DelVito registration. Previously logged photothermal-PRP and multicomponent tomato-extract trials deduplicated. September 30 EADV remains upcoming.

**Remaining gaps:** No full reappraisal of STOP-V, KT-939 or all EADV poster data; meeting has not occurred. Delayed pigmentation recurrence and long-term safety remain gaps; DelVito has no controlled efficacy results.

**Informal-source coverage:** September 9 JCMS episode 106 with Kirk Barber, Vincent Richer and Allison Sutton discusses longer oral-tranexamic-acid use and thrombosis considerations; show notes do not announce a new trial or result. #### KT-939 completes a year-long cosmetic-ingredient safety study

**Source date:** 2026-09-07. **Event date:** 2026-09-04. **Evidence:** sponsor regulatory-exchange announcement; preliminary safety findings.

Kintor reports completing its topical tyrosinase-inhibitor safety study and plans to finish a Chinese cosmetic-ingredient submission. The September 7 announcement reports September 4 completion: 114 participants used 0.2% KT-939 for 52 weeks, in an open-label, single-arm, single-center study. Preliminary data reported no adverse skin reactions. Earlier enrollment was 130; the release does not explain all disposition.

**Limitations:** Not a controlled melasma efficacy trial. The announcement supplies no full safety tables or confidence intervals and cannot demonstrate that rare/systemic harms are absent. Prior 48-week plans must not replace the current reported 52-week duration.

**Safety:** No adverse skin reactions reported among 114 continuous users; no broad claim of proven long-term safety.

**U.S. status:** Chinese cosmetic raw-material registration is being pursued; no U.S. therapeutic approval or new availability verified.

**Next milestone:** Submission of all NMPA application materials planned as soon as practicable; no specific date or approval guaranteed.

#### Vitiligo presentations added to the September 30 calendar

**Source date:** 2026-09-22. **Event date:** 2026-09-22. **Evidence:** sponsor conference schedule; no new numerical efficacy result.

Incyte has scheduled vitiligo analyses at EADV, including quality of life in genital vitiligo and povorcitinib dose-selection modeling. September 30 posters include P2467, open-label phase 2 ruxolitinib-cream genital-vitiligo quality-of-life findings; P1780, exposure-response modeling for povorcitinib phase 3 dose selection; and P2463, a prospective real-world vitiligo registry.

**Limitations:** These titles and schedules are not the promised full STOP-V phase 3 efficacy/safety readout, and modeling is not a new randomized clinical outcome. Numerical abstracts were not appraised.

**Safety:** No new safety conclusion in the schedule; assess full results when released.

**U.S. status:** Existing ruxolitinib labeling remains separate from any new population or body-site analysis; povorcitinib remains investigational for vitiligo.

**Next milestone:** Scheduled posters: September 30, 2026, EADV. Exact future STOP-V full-results or filing day remains unannounced.

[Original September 22 release](https://incytecorp.gcs-web.com/news-releases/news-release-details/incyte-data-european-academy-dermatology-and-venereology-eadv).

**Coverage:** Searched human melasma, PIH, solar spots and vitiligo papers; appraised new-to-notebook September 7 KT-939 safety announcement and September 22 Incyte EADV schedule. Refreshed STOP-V1/V2 records; no newly posted registry results. Prior melasma pilot/PRP findings deduplicated.

**Remaining gaps:** September 19 ruxolitinib-versus-tacrolimus prospective paper and September 13 graft-versus-cell-suspension vitiligo report had no original abstracts; full results inaccessible and not inferred from titles. September 21 picosecond laser melasma abstract reviewed: retrospective 29 women, 12-week follow-up, no comparator. Full tables unavailable; minor series was not promoted. KT-939 disposition from 130 originally enrolled to 114 continuous users, complete adverse-event accounting and matched registry remain unresolved.

**Informal-source coverage:** Reddit and podcast searches found personal tranexamic-acid/hydroquinone experiences and procedure discussions rather than a traceable new formal-program timing or efficacy rumor. No personal anecdote was recast as controlled evidence.

[Full coverage record](../updates/2026-09-22_scheduled_research_update.md).

**Next update:** **September 30, 2026:** EADV vitiligo posters. KT-939 submission and full STOP-V timing are unannounced; cosmetic-ingredient filing is not therapeutic approval.

### Focused update — 2026-09-18

#### Photothermal PRP does not outperform conventional PRP on melasma severity

**Source date:** 2026-09-12. **Evidence:** peer-reviewed randomized human study; original abstract reviewed.

A small split-face trial found no superiority on clinician-rated melasma severity despite a difference in instrument-measured pigmentation. TCTR20260318004; 26 women received three monthly sessions, with photothermal-activated and conventional PRP on opposite sides. At 12 weeks both mMASI scores improved; no between-treatment difference at any assessment. Photothermal PRP had lower melanin index at week 12.

**Limitations:** No untreated/sham side, short follow-up, small female-only sample. No numerical between-group effect or confidence interval in abstract; nonsignificance is not proof of equivalence. Publisher contents page verifies first publication September 12.

**Safety:** Mild injection-site bruising was the only reported adverse event; sample cannot resolve uncommon harms.

**U.S. status:** No U.S. melasma approval established by this trial.

**Next milestone:** Larger trials and relapse follow-up; no announced date.

#### Previously inaccessible melasma pilot now appraised from indexed original text

**Source date:** 2026-09-07. **Evidence:** peer-reviewed pilot randomized human study; gap resolution.

A multicomponent cosmetic cream and triple-combination comparator both improved short-term melasma scores; equivalence was not established. Singapore single-blind split-face pilot, 30 enrolled, 56 days. Cream includes tomato extract, cysteamine, niacinamide and other ingredients. Mean mMASI changes were −1.471 with cosmetic cream and −1.394 with tretinoin/hydroquinone/fluocinolone comparator.

**Limitations:** Small feasibility-based sample; not powered for noninferiority, different application schedules, no post-treatment relapse follow-up. Cannot attribute the effect to tomato extract alone. Direct page is under maintenance; indexed primary methods/abstract/discussion were readable.

**Safety:** Mild self-limited adverse events reported, 6.7% per arm; does not establish long-term safety.

**U.S. status:** Cosmetic formulation study, not a new FDA-approved melasma indication.

**Next milestone:** Confirmatory controlled study and post-treatment follow-up undated.

[Original paper](https://www.mdpi.com/2079-9284/13/5/228).

**Coverage:** Appraised Sep 12 randomized photothermal-PRP melasma abstract and recovered indexed original content of Sep 7 multiingredient-cream pilot, resolving part of the previous access gap. Refreshed original STOP-V1/STOP-V2 povorcitinib records; no posted results/new readout milestone beyond prior sponsor guidance. This is a focused review, not a complete re-review of every program.

**Remaining gaps:** PRP full numeric tables and confidence intervals were not accessible; original abstract distinguishes clinical score from instrumental pigment result. Cosmetic-cream direct publisher page was under maintenance; primary indexed methods, abstract and discussion were recovered, but complete funding and registration details were not. No fresh comprehensive Incyte original-source pipeline review was completed; registry refresh alone does not verify unchanged sponsor plans.

**Informal-source coverage:** Targeted Reddit and podcast/talk searches found personal hydroquinone/tranexamic-acid experiences and generic procedure discussion, with no attributable new program-specific milestone rumor. Informal anecdotal efficacy was not added as formal evidence.

[Full coverage record](../updates/2026-09-18_scheduled_research_update.md).

**Next expected update:** Povorcitinib additional vitiligo data H2 2026 and filings H1 2027 (April sponsor guidance); no firm next malassezin/thiamidol milestone.

### Tracked programs and current evidence

#### Malassezin: an early melasma comparator study

A **January 1, 2026 peer-reviewed study** randomized facial sides in 20 women with mild-to-moderate melasma to topical malassezin 0.75% or hydroquinone 4%, with double-blinding and 12 weeks of treatment. Both sides improved; differences between treatments were not statistically significant. Mild side effects were reported. The small study does not establish equivalence, long-term safety or durable remission. No registry identifier was supplied in the abstract reviewed. [Original study, PMID 41493251](https://pubmed.ncbi.nlm.nih.gov/41493251/)

**Current position:** a human efficacy signal for further study; this paper is not FDA approval of a melasma drug. **Next:** larger controlled comparisons and relapse follow-up; no announced readout date verified. Record formulation and concentration, since differently formulated cosmetic products need not reproduce trial results.

#### Thiamidol: stronger controlled evidence, with important null endpoints

A **January 21, 2026 publication**, [NCT03926845](https://clinicaltrials.gov/study/NCT03926845), reports 200 adults randomized to 0.2% thiamidol cream or vehicle for 12 weeks; 196 completed. Modified melasma-severity scores decreased 36.1% versus 16.1%. However, patient global assessments, digital color analysis and freckles/solar-lentigine outcomes did not significantly separate. No significant adverse events were reported. [Original paper](https://link.springer.com/article/10.1007/s13555-025-01649-y)

**Interpretation:** improvement on one clinician score should not become a claim that all dark spots or all patients benefit. This is a cosmetic-formulation study, not proof of an FDA-approved indication. **Next:** replication, PIH-specific results, longer durability and sunspot-specific trials; no firm milestone date verified. This category retains solar spots and PIH even when an existing melasma program does not establish efficacy for them.

#### Vitiligo: ruxolitinib baseline and povorcitinib development

**Ruxolitinib cream / Opzelura**, from Incyte, has U.S. approval for nonsegmental vitiligo in patients 12 and older. The pivotal studies [NCT04052425](https://clinicaltrials.gov/study/NCT04052425) and [NCT04057573](https://clinicaltrials.gov/study/NCT04057573) found approximately 30% achieved at least 75% facial repigmentation improvement at week 24 versus 10% with vehicle. It is limited to treating up to 10% body surface area; longer treatment may be needed. This is neither universal response nor a cure. JAK-related warnings and local reactions remain relevant. [FDA approval summary](https://www.fda.gov/drugs/news-events-human-drugs/fda-approves-topical-treatment-addressing-repigmentation-vitiligo-patients-aged-12-and-older)

**Povorcitinib**, an investigational oral JAK1 inhibitor, is being evaluated in Incyte's Phase 3 **STOP-V1/V2**, [NCT06113445](https://clinicaltrials.gov/study/NCT06113445) and [NCT06113471](https://clinicaltrials.gov/study/NCT06113471). The April 28, 2026 sponsor announcement reports week-52 facial-response rates of 18.9% versus 6.8% and 18.9% versus 3.1%, meeting both primary endpoints. Do not compare these rates directly with the cream trials: population, route and timing differ. Detailed subgroup, safety and maintenance data matter. [Sponsor topline](https://investor.incyte.com/news-releases/news-release-details/incyte-reports-first-quarter-2026-financial-results-and-provides)

**Next:** additional Phase 3 data in **H2 2026** and vitiligo regulatory applications in **H1 2027**, per April guidance. The July 28 update was also checked; it did not supply a replacement vitiligo date. These remain forecasts, not approval dates. Povorcitinib's separate hidradenitis-suppurativa submission does not mean approval for vitiligo. ### September 14, 2026 — Focused registry review

The primary [STOP-V1 record](https://clinicaltrials.gov/study/NCT06113445), last posted August 25, lists **467 enrolled**, active/not recruiting and actual primary completion on **February 24, 2026**. [STOP-V2](https://clinicaltrials.gov/study/NCT06113471), last posted March 31, lists **450 enrolled**, active/not recruiting and actual primary completion on **March 17, 2026**. Neither has posted registry results. These verified fields resolve part of the earlier registry-access gap; they do not constitute new efficacy data or a filing. The sponsor's **H2 2026 additional-data / H1 2027 filing** forecasts above remain separate from study completion.

A **September 7, 2026** [small melasma pilot comparing a tomato-extract formulation with Kligman's formula](https://www.mdpi.com/2079-9284/13/5/228) was located, but its original full report could not be retrieved. Results and safety were not promoted from search snippets. This is an explicit appraisal gap, not evidence of no new melasma research.

- **2026-01-21 — peer-reviewed study:** thiamidol improved severity scores, with several nonsignificant assessments retained.
- **2026-04-28 — sponsor topline:** both povorcitinib vitiligo Phase 3 primary endpoints met; further data and filing windows announced.
- **2026-09-12 — baseline check:** separate cosmetic hyperpigmentation and autoimmune repigmentation tracks established.

### Coverage and next check

Primary study abstracts, full thiamidol paper, FDA information and Incyte updates reviewed. Full malassezin methodology and current direct registry recruitment fields were not comprehensively retrieved. No exhaustive Reddit/podcast search completed; attributed leads remain in scope. Search for new entrants and PIH/solar-lentigine studies rather than following only these named products.

### September 14, 2026 — Focused research refresh

Refreshed the two povorcitinib primary registry records and checked [Incyte's latest quarterly update](https://investor.incyte.com/news-releases/news-release-details/incyte-reports-second-quarter-2026-financial-results-and). No newer verified efficacy or regulatory event was established in the checked sources. Hyperpigmentation, PIH and lentigines discovery searches were selective; the September 7 melasma report remains unappraised. Informal searches did not establish a new specific rumor worth recording.

### 2026-09-18 — Scheduled focused review

Appraised Sep 12 randomized photothermal-PRP melasma abstract and recovered indexed original content of Sep 7 multiingredient-cream pilot, resolving part of the previous access gap. Refreshed original STOP-V1/STOP-V2 povorcitinib records; no posted results/new readout milestone beyond prior sponsor guidance. Original event dates, safety and limitations are retained in the September 18 update above. The complete-topic successful-check date was not advanced because the listed gaps remain. [Run record](../updates/2026-09-18_scheduled_research_update.md).
