Weight loss

SURMOUNT-1

20.9% reported weight loss · 3.1% loss with placebo · 72 weeks

15 mg once weekly

Adults with obesity, or overweight plus a weight-related condition, without diabetes

Treatment-regimen estimand: intention-to-treat effect regardless of discontinuation FDA-approved for adult weight management (Zepbound)

Safety context

GI events predominated, mostly mild to moderate during escalation.

Limitations: Different populations, durations and titration schedules prevent causal cross-trial ranking.

Glucose control

SURPASS-2

2.3 percentage-point HbA1c decrease · 1.86 pp decrease with Semaglutide 1 mg weekly · 40 weeks

15 mg weekly

Adults with type 2 diabetes

Treatment-regimen estimate, regardless of treatment discontinuation or additional glucose-lowering therapy.

Safety context

GI events were common. Glucose below 54 mg/dL occurred in 1.7% at 15 mg versus 0.4% with semaglutide.

Limitations: Open-label; trial-wide baseline. Comparator is 1 mg semaglutide, not current higher doses.

Body composition

EMBRAZE

1.9 kg less DXA lean mass lost with apitegromab added to tirzepatide (80% CI 1.2–2.7) · 24 weeks

Both groups received tirzepatide, titrated up to 15 mg weekly. The add-on was apitegromab 10 mg/kg every four weeks or placebo.

Adults 18–65 with overweight plus a complication or obesity; diabetes excluded; predominantly women.

Adjusted least-squares means in treatment completers with evaluable DXA; baseline weight/lean mass, age and sex covariates. CIs are 80%, not 95%.

Safety context

Nausea, fatigue and headache were numerically more frequent with apitegromab. One participant in each group had a serious adverse event.

Through week 40 · All dosed participants: 51 per arm. The week-24 efficacy completer populations were 43 and 44.

Safety results & reporting details

Limitations: Small phase 2 completer analysis; no multiplicity adjustment. DXA lean mass is not muscle function; exploratory function showed no notable difference. Eight weeks after stopping both drugs, the lean-mass difference narrowed to 0.9 kg.

  13. Announcement

Tirzepatide versus higher-dose semaglutide comparison depends on estimand

A new comparison uses separate SURMOUNT-1 and STEP UP trials, not randomization between tirzepatide and semaglutide 7.2 mg. Adjusted for sex, age, initial weight and A1c; residual cross-trial differences remain. Credible intervals are not confidence intervals. This indirect comparison does not establish head-to-head superiority.
