PP405 AWAKEN has dosed its first participant; women's trial planned next
Sponsor-reported initiation milestone plus original Phase 2/3 registry; no new efficacy result
A summary update checks selected findings. It does not mean every program, safety question or access detail was reviewed again. Source and event dates remain attached to their findings below.
Selected fields reviewed October 16, 2026. Original source dates and stronger prior findings remain separate; full-topic coverage remains incomplete in the scheduled research log.
Topic baseline checked coverage: Selective baseline with primary sources and one explicitly labeled Reddit timing inference. KX-826 release accessible through indexed primary text only; some registry pages did not render. No Phase 3 launch verified for PP405 or GT20029.
Read the dated research notesAdult BRAVE-AA follow-up adds four-year response maintenance among selected prior responders and safety follow-up up to five years. For pattern hair loss, PP405 now has sponsor-confirmed first dosing in Phase 2/3 AWAKEN and a separate women's Phase 2 study forecast by end-2026; RLS-1496 remains a planned program without verified patient efficacy. Alopecia areata and pattern hair loss remain separate.
The U.S. Olumiant label now includes severe alopecia areata in adults and adolescents aged 12 and older. This autoimmune indication is separate from pattern hair loss and retains the label’s boxed warnings. The October 2 adult report adds long-term maintenance and harms for baricitinib in severe alopecia areata; it does not extend approval to pattern hair loss.
Adult BRAVE-AA follow-up adds four-year response maintenance among selected prior responders and safety follow-up up to five years.
Among selected adult week-52 baricitinib responders, 115/133 on 4 mg and 61/72 on 2 mg maintained SALT ≤20 at week 200. These are prior-responder maintenance rates, not success among all participants or off-treatment cure.
Across 1,303 treated adults and 3,047.73 patient-years, serious infections, thromboses, malignancies and a cardiovascular event occurred. No long-term placebo comparison; JAK boxed warnings remain.
Separate women's Phase 2 initiation by end of 2026 (sponsor forecast); AWAKEN primary December 2027 / final June 2028 (registry estimates).

Pelage's October 14 announcement confirms that the first participant has been dosed in AWAKEN, the Phase 2/3 trial of topical PP405 for male androgenetic alopecia. The exact dosing date was not disclosed. This confirms trial initiation beyond the previously saved recruiting listing; it is not a new regrowth result. Pelage also forecasts a separate Phase 2 study in women with androgenetic alopecia by the end of 2026.
The original AWAKEN record lists 600 planned adult men, PP405 0.05% gel versus vehicle, with the primary change in non-vellus vertex hair count at week 24 and treatment/follow-up through week 52. Actual start is September 21, 2026; primary completion December 2027 and final completion June 2028 are registry estimates, not promised publication dates.
Limits and U.S. status: PP405 remains investigational. The sponsor repeats earlier Phase 2a findings but provides no new controlled benefit or long-term safety results here. This pattern-hair-loss program is separate from autoimmune alopecia areata. The women's start remains a sponsor forecast; no actual dosing date was verified.

November 2026 start and July 2027 completion remain estimates.; AWAKEN, HEADLINE, VDPHL01, GT20029, HU045 and named AA records refreshed without duplicate alerts. Complete Kintor international releases, every updated label, unregistered programs, full safety census and full audio/transcript coverage remain gaps.
Informal-source coverage: Original BQ-002 and clascoterone timing threads revisited with sponsor text. Both timing claims remain unconfirmed without a material status transition. October 14 PP405 repost is dependent on the original Pelage release; wording suggesting a final trial is not proof that all regulatory work is finished. Broad October comments repeating 2027 launch optimism are not independent confirmation. No repeat rumor alert.
Published October 2, 2026; newly appraised October 13. BRAVE-AA1/AA2 (NCT03570749/NCT03899259) adult severe-alopecia-areata follow-up concerns continued baricitinib, not pattern baldness. Among selected week 52 responders remaining on the same dose, SALT≤20 (at least 80% scalp coverage) persisted at week 200 in 115/133 (86.5%) on 4 mg and 61/72 (84.7%) on 2 mg.
The integrated safety population comprised 1303 people and 3047.73 patient-years (median 2.6, maximum 5 years). Seventy-seven experienced serious adverse events; serious infections occurred in 17, nonmelanoma skin cancer in 4, other malignancy in 7, DVT/pulmonary embolism in 2 and MACE in 1; no deaths. Serious-infection incidence was 0.5 per 100 patient-years; corresponding DVT/PE incidence 0.06.
Limits: Efficacy selects previous responders, censors after rescue/discontinuation and imputes missing values by modified last-observation-carried-forward. There is no long-term placebo comparator; these are neither all-enrollee success rates nor off-treatment cure. Uncommon events and selected trial eligibility limit generalization. U.S.: already approved severe-AA therapy; JAK boxed warnings remain. Next: real-world long-term safety and replication, undated.
Original sponsor release September 16, 2026; recovered October 13. Rubedo's index repost is dated September 29, which is not a second trial event. The company plans an RLS-1496 androgenetic-alopecia study led by David Pariser, with an October 2026 start and initial data in the first half of 2027. These are sponsor forecasts; actual recruitment/dosing, sample size, comparator, phase and a matched hair-study registry were not verified.
RLS-1496 modulates GPX4 and targets senescent/stressed-cell biology. The release's finasteride comparison concerns extracted human follicles challenged with DHT, supported by company data on file. Biopsy molecular changes and animal results do not establish visible, durable hair regrowth in patients, superiority to finasteride, or reversal of gray hair. This is pattern-hair-loss development, distinct from alopecia areata.
Safety/U.S.: Adjacent early skin trials cannot establish AGA or long-term safety; no U.S. hair-loss approval verified. The separate AK study NCT07340697 remains active/not recruiting with no posted results; it is not the promised AGA trial. Next: verify a dated protocol and actual initiation; H1 2027 is an early-data forecast, not an availability date.

Source checked: October 9, 2026. Conference: October 8–11; Lilly's portal schedules this poster for October 8 at 5 p.m. Pacific. Actual presentation delivery was not independently verified. The downloaded poster and supplemental slides now resolve the prior numerical-data gap.
BRAVE-AA-PEDS/NCT05723198 pools adolescent cohorts. Among 96 participants originally assigned baricitinib 4 mg who had SALT ≤20 at week 52 (at most 20% scalp hair loss), 92.7% retained that threshold at week 76. Among 72 selected 4-mg “deep, multisite” responders, 95.8% maintained that stricter response; among 42 selected 2-mg deep responders, 83.3% did. Different eligibility rules for continued 2-mg and 4-mg treatment prevent a simple dose comparison. The extension uses modified last-observation-carried-forward and has no continuing randomized placebo comparison. These are maintenance rates among prior responders, not 92.7% success in everyone, off-drug durability or cure.
Safety: The 415-person all-baricitinib population supplies 615.8 patient-years, with maximum exposure 131.4 weeks; not everyone was followed 2.5 years. Seventeen participants had serious adverse events, including three serious infections; five had herpes zoster and five permanently discontinued for adverse events. One myxoid liposarcoma was judged unrelated by the investigator. No deaths or thromboembolic events were reported. The absence of a new signal does not remove boxed JAK warnings or establish rare-event safety. No confidence intervals for maintenance percentages are supplied.
U.S. status and next: The already recorded severe-AA indication from age 12 is separate from these follow-up results; pattern baldness is a different disease. Longer follow-up continues; a public release date was not verified.
Original poster and supplement; Lilly congress portal.
Original source date: September 22, 2026 registry-results posting; newly appraised October 9. This is recovered evidence, not an October trial completion.
NCT06393452 randomized 78 adults: 51 to PP405 0.05% gel and 27 to vehicle for 28 days, with follow-up to day 84. Forty-nine and 25 completed. Treatment-emergent adverse events affected 22/51 (43.1%) versus 10/27 (37.0%); these percentages are calculated from posted counts, without a comparative confidence interval. Serious events were 0/51 versus 1/27, the latter an upper-extremity fracture; there were no deaths. Twenty previous vehicle recipients entered an 84-day active open-label extension, 17 completed, and 8/20 had adverse events, with no serious events.
Interpretation: All posted primary endpoints concern safety/tolerability; secondary endpoints concern pharmacokinetics. No numerical hair-density or visible-regrowth outcome appears in the posted results. Therefore the earlier sponsor's 31% versus 0% male-subgroup response must remain a separately attributed exploratory claim with an unresolved subgroup denominator; the registry does not validate it. Small, short treatment and follow-up cannot establish long-term safety. Reported zero plasma measurements do not prove zero systemic exposure or zero risk.
U.S. status and next: Investigational. AWAKEN's October 7 registry refresh remains recruiting with 600 planned men and the previously recorded actual September 21 start; no new efficacy event. December 2027 primary and June 2028 final completion are estimates.
Original results record; AWAKEN record.
Source date: October 7, 2026 first posting. NCT07863856 at Assiut University is registered as Phase 2/3 and plans 40 men aged 18–40 with androgenetic alopecia, randomized to a single botulinum-toxin-A procedure or autologous nanofat injection. Six-month total hair-count change is primary; trichoscopic terminal/velus counts and related measures are secondary. No results are posted.
The registry is not yet recruiting, with estimated December 2026 start, August 2028 primary and October 2028 final completion. Outcome assessors are masked. Two active procedural arms without sham cannot establish efficacy over no treatment, creation of new follicles or durable visible scalp coverage. Procedural and harvesting harms require an actual safety report; no U.S. pattern-hair-loss authorization for these uses is established. Next: actual start, coherent schedule, blinded assessment details, full safety and visible benefit; no dependable release date.
Remaining gaps: Read the current assigned topic and canonical scope/state. SCOP-GLP1 surveyed 5,804 people in France, the U.S., Mexico and Brazil: 575 GLP-1 users, 1,329 unexposed people with type 2 diabetes and/or overweight, and 3,900 reference participants without those conditions or exposure. Reported hair loss was 65.2% in GLP-1 users versus 35.3% in metabolic controls; 53.9% versus 26.5% sought hair-related care. These are self-reported proportions, not prospective adverse-event incidence. The release supplies no adjusted estimate or confidence interval and does not separate drug effects from weight loss, nutrition or group differences. Its “3.4-fold risk” wording is inconsistent with the raw prevalence ratio; no multiplier is adopted here. The sponsor markets hair-support products, and this survey does not establish their efficacy. No new regulatory action occurred. Next: original abstract/tables and prospective studies; timing unknown. Cross-link GLP-1 therapies, retaining reactive shedding separately from pattern baldness and alopecia areata.

The current AWAKEN / NCT07819435 record lists RECRUITING, superseding the September 18 not-yet-recruiting snapshot. It records September 21, 2026 as the actual study-start date. This is a registry-reported study start, not an independently verified first-injection or first-dose announcement.
Study: Pelage’s registered Phase 2/3 randomized, masked, vehicle-controlled study plans 600 men aged 18–55 with androgenetic alopecia. PP405 0.05% gel and vehicle are compared for 24 weeks; active treatment continues through week 52, while vehicle participants are rerandomized. The primary outcome is week-24 change in non-vellus target-area hair count; visible scalp coverage and longer-term safety require separate assessment.
Interpretation and limitations: A recruitment update makes the late-stage program concrete; it supplies no new regrowth estimate or comparative safety finding. Hair counts, visible benefit, durability and creation of new follicles remain distinct. The registry’s Phase 2/3 designation must not be shortened to a completed Phase 3 result. Earlier emails, screenshots and informal recruitment dates are not retrospectively verified by this later registry update.
U.S. status: Investigational; no U.S. pattern-hair-loss marketing approval. Formal research enrollment is distinct from routine availability.
Next milestone: December 2027 primary completion; June 2028 final completion, both registry estimates. No public readout date is announced in the record.
The unchanged September 3 HEADLINE / NCT07317544 registry lists Phase 1 and Phase 2 in its structured phase field, while the official study title describes a Phase 1 first-in-human study. It remains recruiting. The previously saved June 24 sponsor report concerns early Phase 1 safety/pharmacokinetic cohorts; neither this phase-label clarification nor registration establishes Phase 2 regrowth efficacy. Retain “Phase 1 human evidence; registry lists Phase 1/2” when showing development stage. This is clarification of an existing record, not a new result or a separate medical-news alert. Original HEADLINE record; structured original registry.
Coverage: Focused retrieval of the AWAKEN and HEADLINE original registry APIs while building the Medical frontier visualization. Other hair-loss, alopecia-areata and hair-graying programs were not comprehensively rechecked. No full-topic successful-check timestamp or global watch-run timestamp is advanced.
Informal-source follow-up: The saved September PP405 recruitment claim is now partly corroborated by the registry’s current recruiting status and its reported actual September study start. Source/event date: 2026-09-30. Evidence: Sponsor-only EADV phase 2a open-label follow-up.
Q32 reported 16 weeks off treatment after SIGNAL-AA Part B (NCT06018428). Of 33 originally treated adults with severe/very severe alopecia areata, 18 entered off-drug follow-up; 44% had SALT ≤20 at week 52. Among the selected 12 with hair growth at week 36, all met the maintenance definition: less than a 20-point SALT worsening from their last on-treatment visit. This does not mean all participants maintained unchanged hair. Forty-two percent of those 12 improved further. There was no randomized off-drug comparator. The sponsor reported no serious or grade ≥3 treatment-related adverse events; injection-site reactions affected 36.3%, mostly mild. Lymphocyte reductions were observed without an infection correlation in this small dataset. These findings concern autoimmune alopecia areata, not pattern baldness, and do not establish cure or long-term safety. Bempikibart remains investigational in the U.S. Sponsor release.

FDA approval now includes patients aged 12 and older with severe alopecia areata. The September 2026 U.S. label confirms the expansion; the separately retrieved HTML label still displayed June and should not be used for current eligibility. BRAVE-AA-PEDS randomized 257 adolescents to placebo, 2 mg or 4 mg daily. At week 36, SALT ≤20 (at least 80% scalp coverage) occurred in 5%, 27% and 42%, respectively. Label-reported differences from placebo were 23 percentage points (95% CI 12–34) for 2 mg and 38 (26–49) for 4 mg; differences use underlying unrounded data. This is an indication expansion, not a new result for androgenetic hair loss. Boxed warnings cover serious infections, malignancy, cardiovascular events, thrombosis and mortality; efficacy does not establish risk-free long-term use. A newly registered supplement trial will test biological changes in gray-hair percentage; it has not begun recruiting and has no results. NCT07839494, Mitosynergy: 88 planned adults, randomized four-arm comparison of BioCopper1, Restore Cu1, Grow + Restore Cu1 and placebo. The primary endpoint is change in gray-hair percentage measured by HairMetrix at day 168. Hair density and skin measurements are secondary outcomes. Record first posted September 24; estimated start November 2026, primary and final completion July 2027.
Limitations: Registration does not establish pigment restoration or efficacy. This is a dietary-supplement study, not a Phase 3 drug program. The title says triple-blind while structured masking fields say quadruple; no inference about completed masking or enrollment. Results must distinguish newly pigmented growth from changes in overall hair counts.
Safety: Adverse events, gastrointestinal symptoms, vital signs and laboratory changes are planned outcomes; no safety results.
U.S. status: No U.S. therapeutic approval or demonstrated hair-graying indication established by this registration.
Next milestone: Verify actual recruitment after the November 2026 estimated start; July 2027 completion is an estimate, not a release date.

Source date: 2026-09-21. Event date: 2026-09-21. Evidence: sponsor presentation announcement; older topline context separately labeled.
Q32 will present additional 36-week findings and initial 16-week off-drug observations from SIGNAL-AA Part B at EADV. The announcement supplies no new numerical result. Bempikibart (ADX-914) blocks IL-7 receptor-alpha signaling. NCT06018428 Part B is an open-label phase 2a cohort of 33 patients with severe or very severe autoimmune alopecia areata. Older July 13 topline context: SALT at most 20 was reached by 10/33 enrolled patients (30.3%) and 10/25 in the modified analysis (40.0%). These are not September efficacy results.
Limitations: No concurrent control in Part B; selected disease duration and small sample. Persistence off treatment does not establish cure or relevance to androgenetic hair loss. Registry estimates for June primary completion and September completion are stale; the dated conference announcement is the useful upcoming event.
Safety: July sponsor report described no serious or grade 3-or-higher treatment-related adverse events; injection-site reactions occurred in 36.3%. Longer-term and comparative safety remain uncertain.
U.S. status: Investigational, no U.S. approval.
Next milestone: Scheduled: September 30, 2026 at 15:15 CEST, EADV oral abstract LB-375, Arash Mostaghimi; poster AS-726. Sponsor plans a registration-directed program in H1 2027, a forecast.
September 21 presentation announcement; July 13 topline baseline; SIGNAL-AA registry.
Source date: 2026-09-11. Event date: 2026-09-11. Evidence: peer-reviewed randomized phase 2 study; new publication of an older completed trial.
Splitting the same daily total into two doses produced greater regrowth in a small alopecia-areata study. NCT03811912: 57 adults, 8 mg twice daily versus 16 mg once daily for 24 weeks. Mean relative SALT improvement was 46.4% (90% CI 34.3–58.4) versus 18.0% (90% CI 7.6–28.4). More twice-daily participants achieved SALT at most 20 and at most 10.
Limitations: Trial completed in November 2019 and registry results existed before this September 2026 publication; this is publication context, not a new trial or indication. No placebo arm, small sample and short follow-up. These are 90% confidence intervals.
Safety: Abstract describes mild or moderate treatment-emergent events; it cannot exclude uncommon harms or replace the approved product warnings.
U.S. status: Leqselvi is already U.S.-approved for adults with severe alopecia areata; this paper does not establish approval of a new once-daily regimen.
Next milestone: No new dated regulatory or confirmatory milestone in the paper.
Source date: 2026-09-09. Event date: 2026-09-09. Evidence: trial-registry entry; retrospectively registered study.
A formal comparison will test whether a topical scalp solution and LED cap work better together than either component alone. NCT07810504: recruiting, 40 planned women aged 35–55, four randomized arms, 24-week follow-up. KeraFactor solution versus placebo, crossed with Revian Red cap versus no cap. Hair density, diameter, spacing and terminal-to-vellus ratio are primary measurements; satisfaction and quality of life are secondary. The record reports an actual March 17, 2026 start and first posting September 9.
Limitations: Small single-center study. Device use is unblinded; solutions are blinded and assessors blinded where feasible. No results. Registration is not new evidence of synergy.
Safety: Adverse-event collection planned; none reported yet.
U.S. status: This study does not itself authorize a new combination indication or establish its efficacy.
Next milestone: Registry estimates: December 31, 2026 primary completion; January 1, 2027 final completion. No promised publication date.
Coverage: Read full current topic, including September 19 mechanism-primer changes; searched September 5–22 original papers, sponsor releases, registries, new programs, alopecia areata and biological graying. Refreshed AWAKEN/HEADLINE and new KeraFactor/Revian records. Added Q32 presentation timing and appraised a new publication of an older deuruxolitinib dosing trial. PP405 still not yet recruiting; existing forecasts were not re-alerted.
Remaining gaps: Kintor original HKEX index and March/September filings found; the August interim PDF failed in browser text, and a matched Chinese pivotal registry remains unresolved. No new GT20029 late-stage start verified. Deuruxolitinib full paper not appraised; registry already had results from the older trial, so publication must not be treated as fresh trial completion. A September 15 maresin-1 canities/vitiligo mechanism paper used human profiling but intervention experiments in mice and cells; excluded as human-treatment news. BioQ candidate-specific registry and dated human results remain unverified. No comprehensive updated alopecia-areata regulatory safety-label inventory or full investor-call transcript review.
Informal-source coverage: Targeted Reddit/podcast/talk searches included PP405 registration discussion and ABS-201 investor speculation. Funding and registry estimates do not establish efficacy or first dosing. Original clascoterone and BQ-002 timing posts plus sponsor text rechecked; both remain unconfirmed with no repeat alert.
Next update: September 30, 2026: bempikibart EADV presentation; H1 2027: registration-directed program forecast. PP405 first dosing remains unverified.
Prepared a public, selected-treatment primer for androgenetic (pattern) hair loss. The diagram separates reducing DHT production, blocking androgen receptors, degrading receptor protein, supporting the growth phase, metabolic stem-cell activation, and prolactin-receptor inhibition. Its scope excludes autoimmune/scarring alopecia, graying, transplantation and devices. The mechanism map does not imply a precise tissue distribution, creation of new follicles, or a cross-trial efficacy ranking.
Established therapies. Rechecked the current Propecia label, women's topical minoxidil label, men's 5% solution label, Olsen randomized trial, and human-follicle potassium-channel study. Minoxidil's clinical effect is established while its complete hair-growth mechanism remains unresolved. Oral minoxidil for hair loss remains off-label; do not transfer topical labeling or hypertension event rates to a different formulation or exposure.
The finasteride example is the label's separate 212-man, 48-week phototrichogram study: total hair-count change +7 versus −10 in a 1-cm² target area, net +17 hairs/cm². This is not a conversion of the much larger vertex study's one-inch target-area count and is not its non-vellus endpoint. Confidence interval is not given in the label summary. The UI must show the negative placebo change correctly.
Clascoterone. Rechecked the sponsor's December 3, 2025 six-month report, April 15, 2026 extension report, July 23 filing guidance, and SCALP1 original registry API. Hair-count endpoints were statistically significant in both trials; the patient-reported endpoint reached significance in one and showed a positive trend in the other. The extension enrolled initial responders. Relative improvement headlines alone do not show absolute cosmetic benefit. U.S. filing remains a sponsor Q1 2027 target, not approval or launch. Acne-cream approval does not establish hair-loss solution approval.
GT20029 — older guidance newly resolved in this pass. The peer-reviewed phase 2 paper and publisher numerical abstract report 180 Chinese men randomized equally to six arms for 12 weeks. The illustrated 0.5% once-daily arm had least-squares mean non-vellus hair-count changes 16.80 versus 10.10 hairs/cm², net 6.70, p=0.032. The alternative 1.0% twice-weekly comparison was 11.94 versus 4.57, net 7.37, p=0.023. Older sponsor text rounds differences to 6.69/7.36; use the peer-reviewed displayed values. Events were mostly mild and incidence similar across groups. A confidence interval was not extracted.
Kintor's 2025 annual report, issued April 29, 2026, p. 8, forecasts a China male AGA phase IIb/III start in H2 2026; pp. 16/61 also discuss plans for U.S. phase 2. This is recovered older sponsor guidance, not a new September launch. Actual start remains unverified; NCT06692465 is the completed phase 2 study, last updated November 2024. The forecast qualifies the prior notebook phrase “no firm next date verified,” but does not prove a launch.
PP405. Live original AWAKEN API retains NOT_YET_RECRUITING, 600 planned participants, last posted September 18, 2026. PP405 0.05% gel versus vehicle, primary non-vellus hair-count change at week 24. Dates remain month-level estimates: September 2026 start, December 2027 primary completion, June 2028 final completion. Phase 2a sponsor report describes the 31% versus 0% week-eight result only in a male subgroup; denominator not stated. Keep it textual and exploratory. Mechanism report supports describing MPC inhibition and proposed follicle stem-cell activation, without claiming creation of new follicles.
ABS-201. Original HEADLINE API remains RECRUITING, 227 planned, last posted September 3, 2026, estimated final completion July 2028. June 24 sponsor report is blinded, pooled, single-dose safety in 32 adults, not human efficacy or a treatment-specific safety comparison. Human scalp culture results reported December 11, 2025 remain preclinical even though the tissue is human. Primary prolactin study likewise used cultured follicles. Do not depict PRLR as proven to be the primary cause of ordinary pattern hair loss.
VDPHL01 — recovered fuller topline context. April 27, 2026 sponsor report states 519 randomized participants, superseding the stale planned n=480 in the November 2025 registry record for NCT06724614. At six months, 8.5 mg once-daily produced a reported mean non-vellus target-area count change of 30.3 versus 7.3 hairs/cm²; net 23.0 is calculated, p<0.0001 is sponsor-reported. Improved/much-improved AAIRS coverage was 48.4% versus 13.4%. These are topline sponsor findings, not head-to-head superiority over immediate-release oral minoxidil. August 11 development guidance still forecasts confirmatory male results in H2 2026 and female pivotal results in H1 2027. A lack of cardiac events in this study is not proof of zero cardiac risk.
Coverage limits. Focused primer check as of September 19, 2026. No exhaustive all-pipeline, every-regulator or all-adverse-event re-review. Missing confidence intervals, unresolved GT20029 later-stage start, and source-limited safety detail remain explicit. ClinicalTrials.gov API reads were performed directly because the browser text tool did not expose those records. No patient-specific notes enter the public dataset.
Source date: 2026-09-15. Evidence: trial-registry update.
AWAKEN is now registered, but the latest record still says not yet recruiting. This makes Pelage’s late-stage plan more concrete without confirming first dosing. NCT07819435; Pelage; 600 planned men aged 18–55, Norwood-Hamilton IV/V. Randomized 1:1 PP405 0.05% topical gel versus vehicle for 24 weeks; active continues through week 52 while vehicle participants rerandomize. Primary endpoint: week 24 change in non-vellus target-area hair count; secondary participant-rated scalp coverage. First posted September 15, last updated September 18.
Limitations: No results. Men only and selected severity; registration does not establish efficacy or launch. Mirror dates September 30/December 31 are false precision: original gives month only.
Safety: Longer safety follow-up is planned, not observed evidence.
U.S. status: Investigational; no U.S. marketing approval.
Next milestone: September 2026 estimated start; December 2027 estimated primary completion; June 2028 estimated final completion. Public readout date unannounced.
Coverage: Focused Sep 5–18 human-drug, regulatory and trial-registry review, with overlap against Sep 14 notes. PP405 AWAKEN is a new material registration. Rechecked Pelage, Veradermics, Cosmo and original ABS-201 registry; searched alopecia areata and biological graying. Existing clascoterone filing, VDPHL01 and ABS-201 forecasts were not re-alerted. This is a focused review, not a complete re-review of every program.
Remaining gaps: Kintor KX-826/GT20029 current Chinese original reports and matching registries were not independently resolved in this pass. Alopecia areata and graying searches yielded no verified new in-window material development, but the full worldwide pipeline was not exhaustively checked. AWAKEN first dosing is unverified; not-yet-recruiting status and month-only estimated dates are retained.
Informal-source coverage: Targeted Reddit and podcast/talk searches. Sep 16 PP405 Reddit registration discussions led to primary registry verification; assertions that dosing already started are not confirmed. Existing clascoterone launch inference and BQ002 Phase 1/2027 rumor were revisited without confirmation. No new attributable podcast readout claim recovered.
Next expected update: VDPHL01 male data and ABS-201 interim proof-of-concept data H2 2026; ABS-201 full proof-of-concept data early 2027; PP405 late-stage start targeted 2026; clascoterone U.S. filing Q1 2027 (sponsor forecasts). Graying: no confirmed readout date.
Current assessment: several programs now have randomized human results, including late-stage topical androgen blockade and extended-release oral minoxidil. Regenerative approaches remain earlier. Compare placebo-adjusted absolute hair counts and visible improvement within each trial; relative percentages across unlike studies are not a reliable ranking.
PP405 — Pelage; follicle-reactivation approach. The Phase 2a randomized vehicle-controlled study, NCT06393452, enrolled 78 men and women. The June 17, 2025 sponsor report described four weeks of dosing: in a subgroup of men with greater hair loss, 31% exceeded a 20% hair-density increase at week eight, versus none on vehicle. This is a small, short, subgroup efficacy signal, not proof of broadly durable cosmetic restoration. Pelage's March 18, 2026 update still described late-stage studies as planned for 2026. A Phase 3 launch was not verified in this check. U.S.: investigational. Initial sponsor results, 2026 sponsor update.
Clascoterone 5% solution / Breezula — Cosmo; topical androgen-receptor inhibition. SCALP1 and SCALP2 randomized 1,465 men; NCT05910450, NCT05914805. Cosmo reported positive 12-month results on April 15, 2026. The extension selected initial responders, so continued improvement there does not estimate the response rate in all users. The sponsor reported gains declining after active treatment stopped and similar tolerability to vehicle; full absolute effects and peer-reviewed analyses remain important. A focused September 13, 2026 timeline check verified Cosmo’s July 23 targets: Q1 2027 for a U.S. application and Q2 2027 for Europe. Quarter-specific sponsor guidance. Approval of the separate acne formulation does not authorize this hair-loss product. 12-month sponsor report, H1 2026 filing guidance.
VDPHL01 — Veradermics; extended-release oral minoxidil. The August 11, 2026 company filing confirms positive topline results from the randomized male Phase 2/3 Study 302 and ongoing confirmatory Phase 3 Study 304: NCT06724614, NCT06972264. The company targets male confirmatory and 12-month data in H2 2026 and female Study 306 topline in H1 2027. These remain sponsor forecasts. U.S.: investigational. Its release profile is intended to improve benefit and tolerability; a safety advantage over immediate-release minoxidil requires direct evidence. Current sponsor/SEC update, sponsor trial index.
Pyrilutamide / KX-826 — Kintor; topical androgen-receptor antagonist. A March 18, 2026 China Phase 3-stage topline announcement reports target-area non-vellus counts rising 15.33 hairs/cm² with the 1% regimen versus 4.68 with vehicle: a calculated difference of 10.65 hairs/cm². Full methods, confidence intervals and a matched registry identifier were not appraised here; the release was accessible as indexed primary-source text but failed to open directly. Keep this formulation/trial distinct from older programs and cosmetic sales. No U.S. drug approval or dependable next regulatory date was verified. Dated sponsor announcement.
GT20029 — Kintor; topical androgen-receptor protein degradation. A peer-reviewed Phase 2 study, published December 2, 2025, randomized 180 Chinese men across six regimens/control groups for 12 weeks. Two active regimens significantly outperformed their matched vehicles on non-vellus hair count; adverse events were generally mild and similar across groups. This supports further study, with limited follow-up and population breadth. Paper, NCT06692465. No current Phase 3 start or U.S. approval was substantiated in this selective search.
Established treatments and other causes. Follow material new comparative, formulation and long-term evidence for minoxidil and finasteride. FDA's 2025 alert about compounded topical finasteride highlights the need to distinguish local delivery from absent systemic risk. For severe alopecia areata, FDA-approved JAK-pathway treatments include baricitinib, ritlecitinib and deuruxolitinib; the relevant watch is durability, new populations, long-term safety and new therapies, with separate endpoints from pattern hair loss. FDA topical-finasteride alert, FDA baricitinib context, ritlecitinib evidence, deuruxolitinib evidence. Cross-link relevant immune mechanisms to autoimmune therapies.
ABS-201 — anti-prolactin-receptor antibody. Absci's June 24, 2026 interim report covers 32 adults in the single-dose part of HEADLINE, NCT07317544. Blinded, pooled safety data included no serious adverse events; five participants had mild treatment-related events. These data do not establish a safety difference versus placebo or a hair-regrowth benefit. The sponsor reported an estimated half-life of at least 65 days and initiation of multiple dosing in participants with pattern hair loss. Original sponsor report.
The current registry, last posted September 3, 2026, lists recruiting, 227 planned participants and no posted results. Its July 2028 estimated final completion differs from the sponsor's earlier interim milestones: H2 2026 proof-of-concept data and early 2027 full 26-week data. Those are forecasts, not approval dates. Updated sponsor guidance. U.S.: investigational. Track visible regrowth, durability and longer-term safety separately from hair counts and drug exposure.
| Program | Timing and confidence | What to watch |
|---|---|---|
| ABS-201 | Sponsor forecasts: H2 2026 interim proof-of-concept; early 2027 full 26-week data. Registry final completion estimated July 2028. | Human efficacy, visible benefit, durability and longer-term safety; no regrowth efficacy established by the June safety report. |
| VDPHL01 | Sponsor targets: H2 2026 male Study 304 topline and Study 302 12-month data; H1 2027 female Study 306 topline, per August 11 filing. | Replication, visible benefit, absolute counts, cardiac/tolerability outcomes and filing plans. |
| Clascoterone 5% | Sponsor targets: Q1 2027 U.S. filing; Q2 2027 European filing, per July 23 guidance. Publication/presentation planned, without a firm date. | Actual submission/acceptance, full SCALP results and labeling. Filing is not approval or launch. |
| PP405 | Sponsor target: late-stage studies during 2026, per March release; actual start not verified. | Trial registration, first dosing, design, longer follow-up and broader efficacy population. |
| KX-826 / GT20029 | No firm next date verified. | Current registry identifiers/status, pivotal launches or filings, complete results and formulation-specific safety. |
| Alopecia areata / established treatments / restoration procedures | No single dependable category-wide date. | Material trial readouts, comparative outcomes, safety and regulatory changes. |
Scope expansion. Track prevention and biological reversal of age-related graying, pigmentation in new hair growth, visible coverage and durability after stopping. Distinguish these from dyes, shaft staining, hair-count gains and repigmentation associated with autoimmune disease. This is a bounded new subtopic, not a fresh review of every hair-loss program.
Starting evidence. Human observations show individual hairs can regain pigment: a 2021 study mapped pigmentation changes in relation to life stress. It did not test an anti-graying treatment or establish broad reversal of long-standing gray hair. Primary human study. A July 2026 pediatric alopecia-totalis case reported repigmentation during JAK-inhibitor therapy, but that disease context and single case do not establish an aesthetic treatment for ordinary aging. Case report.
BQ-002 — development lead. BioQ's website describes an injectable sustained-release candidate for gray-hair repigmentation. The readable page provides neither a matched trial identifier nor human results supporting its efficacy or safety claims. Company-level “clinical-stage” wording does not establish that this particular candidate has entered human testing. No U.S. approval or dependable start/readout date was verified.
Partly corroborated; earlier details remain unconfirmed. The saved community thread by roystreetcoffee described a September 2026 start and linked a secondary report and trial record. Earlier monitoring also recorded secondhand comments about an August email; that original email and those precise recruitment dates have not been verified. The currently retrieved Reddit page shows relative timestamps, so this check does not establish an exact original posting date beyond the saved September context. Original community thread.
Key: pp405_september2026_recruitment_rumor. Status history: prior saved checks through 2026-10-02 — unconfirmed; 2026-10-03 — corroborated but unconfirmed in part. The October 2 official registry now lists recruiting and records an actual September 21 study start, corroborating broad September commencement. It does not prove the earlier email’s authenticity, its claimed August timing, individual participant enrollment/dosing or future approval. Official corroborating record.
Next check: Keep future status/efficacy announcements separate; only mark a specific earlier claim confirmed if evidence supports that claim. No approval or results date follows from recruitment.
Clascoterone U.S. launch speculation — unconfirmed. Cosmo guidance still forecasts Q1 2027 U.S. filing; no verified launch date. Community late 2027/2028 availability inference remains unconfirmed.
BQ002 human-trial timing — unconfirmed. Original post and sponsor site rechecked; candidate existence is corroborated, candidate-specific human trial and 2027 start still unverified.
September 27 status check: Existing claims remain unconfirmed; no material status change or repeat alert.
Speculation/inference — unconfirmed U.S. clascoterone launch timing. A July 26, 2026 r/tressless post by jabex1 interprets a reportedly planned Q1 2027 filing as implying availability no earlier than late 2027, and perhaps 2028. The post is an original community interpretation; its underlying newsletter was not separately retrieved. Cosmo's current H1 report corroborates an early-2027 filing plan, but establishes no approval or launch date. Track this as a timing forecast, not a sponsor commitment. Original post. Key: clascoterone_2026-07-26_reddit_us-launch-inference. Status history: 2026-09-12 — unconfirmed. Next check: filing acceptance, official FDA deadline, or explicit sponsor launch guidance; preserve the distinction between an accurate filing forecast and an unverified availability inference.
Rumor — unconfirmed BQ-002 timing. On May 1, 2026, Reddit user ToastedCaffeine posted that Phase 1 was expected in 2027, linking BioQ. Original post. The sponsor corroborates the candidate's existence, but the accessible text does not corroborate that date; no matched registry was verified. The poster's expertise and affiliations are unverified. Key: bq002_2026-05-01_reddit_phase1-2027. Status history: 2026-09-12 — unconfirmed. Next check: dated sponsor guidance, trial registration and first dosing; do not treat a rumored start as a results forecast.
Coverage and timing. No established pharmaceutical reversal or firm near-term readout was verified in this selective search. New formal human programs and attributed leads remain in scope; routine animal stories and gray-market self-experimentation remain outside it.
Added the October 2 AWAKEN recruiting record and its separately dated registry-reported September 21 study start, clarified the existing HEADLINE combined-phase metadata, and updated only the relevant PP405 rumor status. The full hair-topic review and all-topic watch remain incomplete. Historical entries are preserved. Focused record.
2 newly recorded findings or milestones; primary evidence, uncertainty and incomplete coverage are detailed above and in the run record. Full-topic successful-check dates remain unchanged.
2026-09-29: 0 sourced entries, 0 cross-topic links and 2 informal-source checks. Coverage and gaps.
2026-09-27: 1 sourced entries and 1 cross-topic links from a focused scheduled check. Coverage and remaining gaps.
2026-09-22: Focused scheduled research retry; 3 sourced entries and 0 cross-topic links. Coverage and gaps.
Added the June 24 ABS-201 safety/PK report and current HEADLINE registry as previously missing baseline context, rather than September news. Pelage's release index, Veradermics' release index and Cosmo's July guidance did not establish a newer readout or PP405 late-stage launch in this focused check. Kintor's full release remained inaccessible; current Chinese registry matching remains incomplete.
Revisited the existing clascoterone and BQ-002 Reddit posts and the BioQ website. Both timing claims remain unconfirmed: filing guidance does not establish launch, and BioQ's readable text does not substantiate the 2027 BQ-002 trial-start rumor. Recent informal searches produced repeats and self-experimentation rather than a new formal-development signal. This was a focused check with gaps, not exhaustive surveillance or a new all-topic baseline.
Focused Sep 5–18 human-drug, regulatory and trial-registry review, with overlap against Sep 14 notes. Original event dates, safety and limitations are retained in the September 18 update above. The complete-topic successful-check date was not advanced because the listed gaps remain. Run record.
