Current state

Banner has announced the first randomization in API-2, testing donanemab, nivagacetor and their combination in the Colombian PSEN1 E280A kindred. The registered primary endpoints concern amyloid removal/maintenance, so this is no demonstration of dementia prevention. It joins a broader pipeline in which biomarker and diagnostic-management findings must remain distinct from cognitive/function outcomes. The prior PLASMAR disclosure study did not test treatment efficacy, and presymptomatic combinations remain investigational.

Status: active. Initial baseline checked 2026-09-12; selective watchlist, not a systematic review.

Questions to follow

  • New screening or diagnostic tests, including blood-based approaches, and formal human studies of prevention and treatment.
  • Distinguish population screening of asymptomatic people from diagnostic testing of people with cognitive symptoms, specialist confirmation, and trial-enrollment tests.
  • For tests, track independent prospective validation, sensitivity, specificity, predictive values in the intended population, reference standards, downstream testing, clinical utility, and U.S. authorization.
  • For therapies, prioritize cognition, daily function, quality of life, and clinically meaningful progression alongside biomarker effects.
  • Follow safety, including relevant imaging abnormalities or hemorrhage for therapies where applicable; identify trial population, eligibility, monitoring requirements, and treatment burden.
  • Include new mechanisms and prevention trials, not just updates to existing treatment classes.

Programs and next milestones

Focused update — 2026-10-16

Colombian Alzheimer prevention study announces its first randomization

Development stage: Banner Health announced October 15 that the first participant had been randomly assigned in API-2 (NCT06996730). The phase 2/3 study plans 240 participants from the Colombian PSEN1 E280A kindred: 200 mutation carriers and 40 noncarriers. In Part 1, carriers receive donanemab to remove amyloid. Part 2 compares donanemab, oral nivagacetor (RG6289), both, or placebo after sufficient plaque clearance. Noncarriers receive placebo separately rather than entering the carrier randomization.

Evidence and limitations: This is an initiation milestone, not a prevention result. The registry's primary outcomes are PET-measured amyloid reduction and maintenance of low amyloid levels; the public registered outcome list does not establish a powered dementia-onset endpoint. Rare autosomal-dominant disease is distinct from common late-onset Alzheimer disease. The announcement does not provide the exact randomization date or new safety data.

U.S. status: Donanemab has an early-symptomatic Alzheimer indication, but presymptomatic treatment and this combination remain investigational. Nivagacetor is investigational.

Next milestone: Live registry lists recruiting, actual study start July 8, 2026, and estimated primary/final completion June 1, 2030. Its September 2 record predates the first-randomization announcement; study start is not the same event as randomization.

Sources: Source 1; Source 2.

Remaining gaps: Focused prevention/development update, with saved registries and all 38 discovery records retrieved. Added the investigator-announced first randomization in API-2, whose live registry was fetched separately. Previously saved PLASMAR, AHEAD, ultrasound/vaccine and SPINOZA entries were deduplicated. The October 8 Neurology Clinical Practice correction/replacement of a donanemab meaningfulness analysis was located by its journal index, but the correction text could not be retrieved; do not infer its substance. Full anti-amyloid label/ARIA/recall inventories, assay comparative utility and paid interviews remain incomplete. No full successful-check advancement.

Informal-source coverage: Indexed original October 10 Reddit/NYT discussion and Attia–Davidson public episode material were checked as leads. Neither provides new randomized confirmation of obicetrapib dementia prevention. SPINOZA remains a biomarker-focused registered program; the saved clinical-prevention hypothesis stays unconfirmed. Full paid transcript and podcast census remain incomplete.

PLASMAR finds earlier blood-test disclosure changes memory-clinic decisions

Publication: October 5, 2026; recovered October 13. PLASMAR/NCT06246019 randomized 220 new patients with subjective cognitive decline, MCI or mild dementia at one Spanish memory clinic to p-tau217/NfL disclosure at three versus nine months, alongside usual assessment.

At three months, clinician confidence of at least 90% in the etiologic diagnosis occurred in 56/112 (50.0%) versus 5/108 (4.6%), p<.001. Symptomatic-treatment initiation was 16/112 versus 5/108, difference 9.7 percentage points (95% CI 1.8–17.8). Diagnostic confidence generally converged after later disclosure; follow-up/discharge measures included clinician-reported plans.

Limits/harms: Confidence is not independently confirmed diagnostic accuracy. The trial did not establish improved cognition, function, quality of life or disease course. No significant worsening in emotional outcomes was found. Available paired questionnaires varied by instrument; HADS included 211 at baseline, 135 for baseline-to-three-month change and 94 for three-to-nine-month change. Open disclosure, one center and underpowered subgroup analyses constrain interpretation.

U.S./next: No new U.S. authorization or healthy-population screening recommendation. Multicenter diagnostic pathways, downstream outcomes and access need study; no announced next date.

Sources: Source 1.

AHEAD screening analysis finds differences in p-tau217 distributions

Publication: October 12, 2026. A cross-sectional analysis of 15,612 AHEAD 3-45 screening participants, aged 55–80 at 106 sites in four world regions, uses samples collected July 2020–October 2024 before medical/cognitive exclusions.

Plasma %p-tau217 levels were lower in self-identified Asian, Black and Hispanic groups than White/non-Hispanic groups; APOE ε4 was associated with higher levels throughout. The APOE–biomarker association varied by race/ethnicity. Associations between %p-tau217 and amyloid PET had no significant pairwise racial differences, but ethnicity-related differences were found.

Limits: This is a selected trial-screening cohort and association analysis, not a randomized screening strategy, a causal biological explanation by race, or demonstration of unequal sensitivity/specificity. Self-reported social categories, APOE, pathology prevalence and other context require careful interpretation. The primary abstract was appraised; full tables and supplements remain a gap. Do not transpose %p-tau217 findings to every assay or claim an improved health outcome.

U.S./next: No new asymptomatic-population authorization or cutoff recommendation. External calibration and prospective screening-pathway outcomes are needed; timing unannounced.

NMS-01 registers an early focused-ultrasound Alzheimer safety study

Source: First posted October 9, 2026; actual study start January 5, 2026. Neumous NMS-01/NCT07869238 is a recruiting, single-center open-label focused-ultrasound pilot in ten planned patients with mild/moderate Alzheimer disease.

Primary measures are successful/accurate transient blood-brain barrier opening, its recovery by 24 hours or seven days, and adverse events through day seven. It is a device study with no assigned drug phase.

Limits/safety/U.S.: No comparator or results. Opening the barrier is a technical endpoint, not demonstrated cognitive benefit or long-term safety. Registry access is not U.S. marketing approval. Do not infer benefit from the technology's proposed delivery mechanism.

Next: October 31, 2026 primary and November 30, 2026 final completion estimates. An elapsed/future estimate does not establish completed follow-up or available results.

Sources: Source 1.

Remaining gaps: Read full active scope, prior gaps/state and September 5 overlap. Retrieved all 14 named registries and 35 new-registration matches. Appraised PLASMAR full randomized clinical-utility article and Oct 12 AHEAD primary abstract; NMS-01 device record separately appraised.; AHEAD full tables/supplement not available. All antibody labels/ARIA changes, blood-assay recalls, worldwide prevention cohorts and clinical-outcome inventory remain incomplete. Registry October 13 refreshes do not by themselves mean new efficacy or actual first dosing.

Informal-source coverage: Revisited Attia/Davidson public original source and SPINOZA live registry; cognitive-prevention hypothesis remains unconfirmed. October 10 Reddit semaglutide-prevention headline appears to repeat older observational associations and is not a new randomized benefit. Patient-care anecdotes and stock discussions not promoted. Full paid interview and broad podcast review remain incomplete.

Baseline recovered: TREMHANCE is recruiting for oral SAR448851

Baseline, not new October news. First posted July 7, 2026; latest posted update September 22. The registry now verifies TREMHANCE/NCT07688213, Sanofi's phase 2 oral SAR448851 study, as recruiting, with actual start July 28, 2026.

The double-blind placebo-controlled portion plans up to 160 adults with amyloid-confirmed MCI due to Alzheimer disease or mild Alzheimer dementia. Treatment lasts 48 weeks, followed by an open-label extension. The primary efficacy endpoint is plasma p-tau217 change at week 48; extension safety, including MRI ARIA, is tracked separately.

Limits/safety: No results are posted. Biomarker change is not proven preservation of memory, everyday function, or delayed dementia. The record does not establish long-term safety. A third-party September 28 date is not evidence of an October launch.

U.S. status / next milestone: Investigational; no new U.S. approval. July 31, 2029 primary/final completion is a registry estimate, not a promised readout.

Original registry.

SAGITTARIUS registers an amyloid vaccine trial before cognitive impairment

Source/event date: First posted September 18, 2026; refreshed September 24. Recovered October 9, rather than a new October start. Evidence: Phase 2 SAGITTARIUS/NCT07827768 registration.

The Institute for Molecular Medicine plans 160 cognitively unimpaired adults with preclinical Alzheimer disease, randomized to AV-1959R amyloid-β vaccine or placebo and followed for 78 weeks. Primary measures include adverse events, MRI ARIA-E/H, other safety assessments, suicidality, and antibody response. Amyloid/other biomarker outcomes are distinct from cognitive prevention.

Limits/safety: Not yet recruiting and no results posted. Generating antibodies or lowering amyloid cannot establish prevention of dementia, healthy years gained, or long-term vaccine safety. This population differs from trials treating symptomatic Alzheimer disease.

U.S. status / next milestone: Investigational vaccine; no new approval. March 1, 2027 estimated start; June 15, 2029 estimated primary/final completion. No guaranteed publication date.

Original registry.

Remaining gaps: Read current topic/state and September5 overlap; all12 saved registry records,32 new-study search matches and recovered SAR448851 live record appraised for scope.; SAR448851 registry access gap closed, historical start July28 retained as baseline. Selected new prevention vaccine registration. No new efficacy inferred from registry refreshes for ClarityAD/PDE4PET/ALN-5288.; All antibody labels/ARIA changes, assay recalls, worldwide prevention cohorts and full blood-test outcomes inventory remain incomplete. Several new biomarker/device records require full appraisal before promotion.

Informal-source coverage: Opened original Oct7–8 r/Alzheimers treatment-or-not thread. It mixes anonymous personal experiences, unsupported universal-treatment claims, and long-delay figures apparently derived from modeling; none establishes new randomized efficacy/safety or individual benefit. Existing Attia/Davidson obicetrapib prevention hypothesis remains unconfirmed. No distinct new development rumor selected.

Japan registers a study combining digital cognition and blood biomarkers for Alzheimer triage

Source/event date: October 6, 2026 registration. Evidence: Formal observational human study, STRIDE-PAD / UMIN000063185, Japan; no results.

Oita University plans 800 community-dwelling Beppu residents aged 50–89, independent in daily activities and without diagnosed dementia. The study combines community XpressO screening, self-administered MoCA Solo-J in primary care and plasma pTau217/Aβ42. It compares usual primary care, digital assessment alone, blood biomarker alone and a hierarchical combination against specialist assessment including CDR/amyloid PET. Primary measures are accuracy, sensitivity, specificity and predictive values, with Japanese cutoff calibration.

Limits/safety: Observational diagnostic validation does not establish dementia prevention, improved patient outcomes or replacement of specialist diagnosis. False positives, intermediate classifications and downstream testing remain relevant. The record lists preinitiation, although its anticipated September 6, 2026 start has passed; actual start/enrollment are unverified. Industry funders include Theoria Technologies and Fujirebio.

U.S. status / next milestone: Japanese research registration creates no U.S. screening authorization. March 31, 2028 is the listed last follow-up date, not a promised publication date. Confirm actual initiation, then prospective accuracy/workup results.

SPINOZA now listed as recruiting

Source/event date: 2026-09-28. Evidence: Trial registry update.

The September 28 ClinicalTrials.gov update lists SPINOZA (NCT07794670) as recruiting, replacing the notebook's not-yet-recruiting snapshot. The planned 400-person, placebo-controlled phase 2 study tests obicetrapib 10 mg daily in preclinical Alzheimer's disease. Its primary outcome is change in plasma p-tau217 at 52 weeks; cognition (PACC5) is secondary. Recruitment is a development milestone, not evidence that obicetrapib prevents dementia. The September 2026 start remains an estimate, so this record does not establish an actual first dose. Primary completion remains September 2028 and final completion May 2029, both registry estimates. No results or Alzheimer's approval were established.

Next milestone: September 2028 primary completion and May 2029 study completion — registry estimates; actual first dose unverified

Remlifanserin misses Alzheimer psychosis primary endpoint; phase 3 continues

Source date: 2026-09-24. Event date: 2026-09-24. Evidence: Sponsor phase 2 randomized topline results.

RADIANT’s phase-2 portion missed its primary endpoint despite a nominal secondary signal; Acadia plans continued phase-3 enrollment. At six weeks, SAPS-H+D change with 60 mg was −12.6 versus −10.4 placebo (effect size .26; p=.0603). CGI-S-ADP changed −1.3 versus −.9 (effect size .37; nominal p=.0077). The 30-mg arm showed minimal benefit and is planned for removal from phase 3.

Limitations: Analyses used a prespecified modified population. Sample counts, confidence intervals and complete tables were not disclosed. These outcomes concern hallucinations/delusions, not Alzheimer progression or prevention.

Safety: Sponsor reported similar adverse-event, serious-event and discontinuation rates, no QT signal and no deaths in active arms; quantitative tables and longer follow-up remain needed.

SPINOZA is formally registered to test obicetrapib in preclinical Alzheimer disease

Source date: 2026-08-31. Event date: 2026-08-31. Evidence: Recovered baseline: phase 2b registry, no efficacy result.

The actual SPINOZA registry closes the prior uncertainty about whether the proposed Alzheimer program was registered. NCT07794670, NewAmsterdam Pharma: planned 400-person randomized placebo-controlled phase 2b study of obicetrapib 10 mg in preclinical Alzheimer disease. Primary endpoint: p-tau217 change at 52 weeks. PACC5 cognition is secondary; APOE eligibility is protocol-defined without full genotype criteria disclosed in the public record.

Limitations: First posted August 31, not a September 22 launch. Still not yet recruiting in that snapshot. A biomarker-primary study cannot by itself establish reduced dementia incidence.

Safety: No trial results or safety outcomes posted.

U.S. status: Not FDA-approved for Alzheimer prevention.

Next milestone: Registry estimates: start September 2026, primary completion September 2028, study completion May 2029. Actual initiation not verified.

p-tau217 plus APOE refines cognitive-risk estimates in pooled cohorts

Source date: 2026-09-09. Event date: 2026-09-09. Evidence: Peer-reviewed pooled prospective human cohorts; prognostic model/association.

A multi-cohort study connects plasma p-tau217 and APOE with later cognitive impairment, without testing screening-guided prevention. Seven cohorts across Canada, Dominican Republic and the U.S.: 8,582 baseline participants, 4,569 cognitively unimpaired in longitudinal analyses. Per 1-SD higher p-tau217, incident MCI/dementia HR 1.41 (95% CI 1.22–1.64); APOE ε4 carriers 1.76 (1.36–2.26), noncarriers 1.26 (1.12–1.42).

Limitations: Risk association/modeling, not a treatment or test-guided outcome trial. Group-level timing is not an individual countdown. Cohorts include substantial Black and Hispanic representation, but calibration and transfer across ancestry/populations remain relevant.

Safety: No intervention safety assessment; misclassification and interpretation limits matter.

U.S. status: No new screening authorization results from this paper; do not extend symptomatic diagnostic-test indications to healthy-population screening.

Next milestone: Prospective testing of clinical utility and external calibration; no dated follow-up verified.

PrecivityAD2 FDA clearance recovered in baseline audit

Source date: 2026-08-19. Event date: 2026-08-19. Evidence: Recovered baseline: original FDA 510(k) clearance.

FDA cleared PrecivityAD2 on August 19; the September news item is not a new regulatory decision. K253240 combines plasma Aβ42/40 and percentage p-tau217 into APS2. Intended for adults 40+ with cognitive symptoms undergoing Alzheimer/cognitive-decline evaluation, together with clinical assessment.

Limitations: FDA labeling explicitly excludes screening and stand-alone diagnosis. Diagnostic amyloid-pathology assessment does not prove better patient outcomes from screening.

Safety: False positive/negative interpretation risks; avoid extrapolating beyond intended population.

U.S. status: FDA-cleared diagnostic aid; not clearance for asymptomatic population screening.

Next milestone: No new dated milestone verified.

FDA clearance letter and indications, August 19; FDA database decision.

Coverage: Read full topic, baseline/state and September18 gaps; September5–22 treatment, blood-test, human prevention, safety/negative searches. Verified SPINOZA original registry and FDA PrecivityAD2 clearance, appraised p-tau217/APOE original abstract and investigator discussion. Revisited Attia/Davidson original notes. Earlier ALZ101, PrevenTRON and tau-platform entries deduplicated.

Remaining gaps: Exact ALZ101 phase2 and PrevenTRON registry matching remains unresolved. All current anti-amyloid labels/recall-database changes and older lithium/XPro/external-counterpulsation full papers were not comprehensively appraised. SPINOZA registration is verified but actual enrollment/start is not; September estimate is not confirmation. Full p-tau217/APOE paper not accessed; original abstract used. Grifols forthcoming November RWE presentation identified, without new numerical results to appraise.

Informal-source coverage: Rechecked the original September14 Attia/Davidson episode chapter notes; registered-study existence now verified independently, preventive benefit remains a hypothesis. Targeted recent Reddit Alzheimer/biomarker queries mostly personal anecdotes or reposts; no separate new verified efficacy rumor extracted. Full podcast/transcript sweep remains incomplete.

Baseline evidence

2026-09-12 — Initial evidence baseline

Blood tests are advancing diagnosis in people with cognitive concerns, not validated population screening. FDA cleared Fujirebio's Lumipulse pTau217/amyloid-beta ratio in May 2025 for symptomatic adults aged 55 or older; it is not a standalone diagnosis or general screening test. FDA's evaluation used 499 plasma samples from cognitively impaired adults. A Class II recall, posted February 5, 2026, covers specified Lumipulse reagent lots because falsely elevated ratios could produce incorrect positive or indeterminate classifications; this is a lot-specific issue, not withdrawal of every assay. FDA clearance; FDA recall.

Roche's Elecsys pTau217 received FDA clearance August 19, 2026 (announced August 24), for adults 55+ with signs, symptoms or complaints of cognitive decline, used with other clinical information. It provides positive/intermediate/negative assessment of amyloid pathology. FDA-reviewed performance used stored samples from clinical cohorts with amyloid PET as reference; it does not demonstrate that asymptomatic testing prevents dementia. Watch independent prospective validation, effects of renal disease and other confounders, intermediate-result burden, diagnostic workflows and clinical utility. Next update: no reliable dated results milestone identified for these diagnostic programs. FDA decision; FDA-reviewed intended use and study methods.

Approved disease-modifying treatment: Leqembi (lecanemab; pivotal NCT03887455) and Kisunla (donanemab; NCT04437511) are used in early symptomatic, amyloid-confirmed disease. Randomized trials demonstrated slower decline, not cure or established prevention in healthy adults. FDA approved Leqembi's subcutaneous initiation regimen on July 13, 2026, allowing home dosing from the start. That formulation relied on pharmacokinetic/amyloid bridging to IV efficacy, not a separate large clinical-outcome trial. Kisunla is given by monthly IV infusion. Both require attention to ARIA/hemorrhage, APOE-related risk and monitoring burden. Next update: no dependable date identified for the next major safety or long-term clinical-outcome release. FDA Leqembi update; FDA Kisunla evidence.

Prevention is still being tested. Phase 3 TRAILBLAZER-ALZ 3 (donanemab, NCT05026866; approximately 2,996 participants) and AHEAD 3-45 (lecanemab, NCT04468659; approximately 1,400) study biomarker-selected people before dementia. Both are active, not recruiting in checked records. Registry-estimated primary completion: November 2027 and December 21, 2028, respectively; these are not promised publication dates. Clinical progression and cognition matter more than plaque reduction. TRAILBLAZER registry; AHEAD registry.

Trontinemab uses a brain-transport mechanism to deliver an anti-amyloid antibody. Phase Ib/IIa Brainshuttle AD (NCT04639050) supplied amyloid/biomarker and early safety findings; Roche presented extension data July 2026. These do not yet establish superior clinical benefit. Phase 3 TRONTIER 1/2 (NCT07169578/NCT07170150) are underway; TRONTIER 2's registry-estimated completion is June 7, 2028, with no firm readout date verified. Watch cognition/function, ARIA, discontinuations and durability. Roche also announced the PrevenTRON prevention design; enrollment/start timing was not established here. Roche July update; TRONTIER 2.

Diranersen (BIIB080), a tau-lowering antisense therapy, is the leading non-amyloid watch item here. Phase 2 CELIA (NCT05399888) missed its primary dose-response endpoint. Biogen's May/July 2026 conference reports nevertheless describe tau reduction and favorable prespecified cognitive analyses, with the lowest dose showing 26% less CDR-SB decline. These are sponsor-reported Phase 2 findings, not confirmatory efficacy. Biogen plans Phase 3; no reliable start/readout date was verified. Watch full publication, multiplicity, absolute effects, adverse events and actual registration. May primary-endpoint disclosure; July data.

Coverage gap: selective programs only; a full assay comparison and systematic review of all non-amyloid mechanisms remain outside this initial brief. Registry completion estimates may move and must be rechecked.

Rumors and informal signals

September 27 status check: Existing claims remain unconfirmed; no material status change or repeat alert.

  • obicetrapib_attia_davidson_2026_09_14_alz_prevention_discussion — unconfirmed: Formal SPINOZA program existence remains independently established, but registry still says not-yet-recruiting with estimated September 2026 start. No new cognitive-prevention benefit established. Public episode material is not a fully appraised paid transcript. Michael Davidson on The Drive; ClinicalTrials.gov NCT07794670.

September 22 follow-up: SPINOZA registration is now independently verified; the earlier prevention-benefit hypothesis remains unconfirmed. Registry entry is not cognitive-outcome evidence. NCT07794670.

October 2 follow-up: The September 28 registry update now lists SPINOZA as recruiting. This confirms recruitment status, not an actual first-dose date or dementia-prevention benefit. The earlier prevention-benefit hypothesis remains unconfirmed; the primary endpoint is a blood biomarker. Current registry.

September 14, 2026 — focused ad-hoc review

The September 5–14 overlap search checked Biogen releases, Roche program information, and Lilly pipeline material, plus searches for new human trials, negative results and informal discussion. No material new therapeutic finding was verified in this bounded pass. Recent-looking stories often concerned older studies or preclinical work.

Coverage gaps: AHEAD, TRAILBLAZER, TRONTIER and CELIA registries and blood-assay regulator/recall records were not fully rechecked. Lithium/MCI and external-counterpulsation publication leads still need original-paper/date appraisal; older ALZ-101 and XPro1595 papers are baseline leads, not new September results. Indexed Reddit searches did not establish a new specific program rumor; podcasts and interviews were not comprehensively searched. This is a selective review, not an all-clear or a new complete baseline.
