Current state

BRUNELLO adds year-one vision noninferiority with more proliferative-retinopathy events and adverse-event discontinuations. MCO-010 reports four-year inherited-RP gains, with follow-up denominators and intervals missing; this does not establish acquired-AMD benefit.

Status: active. Selective baseline completed September 12, 2026; not an exhaustive systematic review. Hearing and vision remain distinct subtracks within this file.

Questions to follow

  • Hearing: can treatments prevent or reverse common age- or noise-related loss, and do hearing-threshold gains improve speech understanding, especially in noise?
  • Vision: can therapies preserve useful vision or restore reading and daily function in common degenerative eye disease?
  • Distinguish rare genetic disease from common aging, preservation from restoration, and implanted assistive devices from biological repair.
  • Track formal human drugs, gene/cell therapies, and clinically studied restorative devices. Require durability, meaningful functional outcomes, and procedure/device safety alongside headline responder rates.

Programs and next milestones

Focused update — 2026-10-16

A phase 3 program will test extracellular vesicles for sudden hearing loss

Development stage: Sun Yat-sen Memorial Hospital first posted NCT07878559 on October 16. This planned 146-person randomized phase 3 study uses masked outcome assessment and compares intratympanic autologous circulating extracellular vesicles plus glucocorticoids with glucocorticoids alone in adults with acute total-type sudden sensorineural hearing loss (pure-tone average above 70 dB).

Evidence and limitations: The primary endpoint is the proportion with more than 10 dB pure-tone-average improvement one month after treatment. Speech recognition and quality of life are separate secondary outcomes. The registry's narrative refers to prior exploratory work, but supplies no fully appraised randomized result here. This is acquired acute hearing loss, not inherited deafness, chronic tinnitus or common age-related loss. Registration does not demonstrate benefit, durability or long-term safety.

U.S. status: A China-based investigational program; no U.S. approval or routine access established.

Next milestones: The record remains not yet recruiting despite its elapsed September 15, 2026 estimated start. Actual initiation is unverified. Estimated primary completion is September 30, 2029; final completion March 31, 2030.

Sources: Source 1.

Remaining gaps: Focused human-program and access update, with current vision conference findings deduplicated. Named trial records were retrieved; broad discovery returned 541 records, only the first 100 retrieved. Registered intratympanic extracellular-vesicle phase 3 program was appraised. SPI1005 expanded-access record is temporarily not available, so registration was not described as open access. October 21 4DMT investor day remains scheduled. The complete BRUNELLO/MCO-010 underlying tables, every inherited/acquired sensory pipeline, regulator labels and original conference recordings remain incomplete. No full successful-check advancement.

Informal-source coverage: Revisited targeted AC102/hearing rumor searches and official/registry leads. No official phase 3 tinnitus start or authenticated confirmation of the saved purported-email 2027 claim was established. Registration of a different sudden-hearing-loss intervention does not corroborate AC102. The saved rumor remains unconfirmed; full original podcast/audio coverage incomplete.

BRUNELLO adds visual-acuity intervals and a retinal safety imbalance

Source/event date: October 10, 2026; Merck confirms the AAO presentation occurred that day. Study: BRUNELLO/NCT06571045, randomized masked phase 2b/3, 984 adults with diabetic macular edema, monthly injections for year one.

Mean BCVA gains were +9.1 ETDRS letters (95% CI 8.1–10.1) with remigromig 0.5 mg and +8.7 (7.7–9.8) with 0.8 mg, versus +11.8 (10.7–12.8) with ranibizumab 0.5 mg. Both doses met the prespecified noninferiority criterion; the multiplicity-adjusted p=.0129/.0222 values concern noninferiority. No secondary endpoint demonstrated superiority.

Safety: PDR-related events, including new-onset proliferative diabetic retinopathy, were 6.7%/6.1% versus 0.9%; discontinuation due to adverse events 4.9%/4.5% versus 0.9%. The sponsor's possible natural-progression/mechanistic explanation is not an established causal explanation. Further analyses are ongoing.

Limits/U.S.: Year-one vision noninferiority does not establish better everyday function, durability, or a safety advantage. Investigational, without new U.S. approval. The second year varies injection intervals; companion BAROLO and detailed safety analyses remain pending without a verified exact readout date.

MCO-010 four-year RP follow-up is now reported after AAO

Source/event date: October 11, 2026. Kenneth C. Fan's AAO investigator interview/report identifies delivery of REMAIN four-year results, session PA044. This closes the presentation-occurrence gap; the actual slides and full follow-up table remain unavailable.

RESTORE/NCT04945772 originally randomized 27 people with severe inherited retinitis pigmentosa to high-dose/low-dose MCO-010 or sham. At week 204, the accompanying investigator report gives mean BCVA improvement of 0.261 logMAR at high dose and 0.244 at low dose after one intravitreal injection. It contrasts historical week-52 gains of 0.337/0.382 logMAR; those are earlier observations, not new comparisons.

Limits/safety: The report supplies no four-year arm denominators, confidence intervals, attrition analysis, or concurrent sham result. Do not present these means as a randomized four-year treatment effect or convert them into a guaranteed number of reading-chart letters. The investigator reports no serious adverse events and treatable mild/moderate intraocular inflammation, without detailed event counts.

U.S./next: MOGENRY remains under FDA review, with no approval or verified decision date. Retrieve the full REMAIN dataset and separate dry-AMD PO624 results; RP evidence does not establish benefit in acquired AMD.

Sources: Source 1; Source 2; Source 3.

Remaining gaps: Read topic/state and September 5 overlap; all 14 named registries and first 100 of 491 new-study matches retrieved. BRUNELLO primary October 10 detailed results appraised; direct Oct 11 MCO010 investigator interview/report confirms delivery and gives selected week 204 values.; MCO010 original slides, four-year denominators/CI/attrition and dryAMD PO624 exact tables remain unappraised; no inherited-RP/acquired-AMD substitution. BEST1 original PDF access and comprehensive common acquired-hearing inventory remain gaps. Remaining new-study pages not retrieved; many broad vision/hearing matches incidental.; Historical PRIMA/4D150 data not repeated as fresh news. AC102 record remains January 16 snapshot with elapsed estimated completions and no posted result.

Informal-source coverage: Original BRUNELLO and MCO010 investigator interview reports appraised as conference/interview evidence, not peer-reviewed full tables. Existing anonymous AC102 sponsor-email/2027 plans remain unconfirmed after current registry and official tinnitus page check; no phase 3 start or chronic-tinnitus efficacy inferred. Full interview/podcast census incomplete.

4D-150 expands into phase 3 for diabetic macular edema

Source dates: September 28 sponsor report; 4SIGHT/NCT07867080 first posted October 9. This recovers a September announcement, rather than new October efficacy data.

SPECTRA: At the planned phase 3 dose, nine participants had mean +10.8 ETDRS letters and −176 μm retinal thickness at two years. They averaged 5.2 supplemental injections after loading, versus 13 projected for labeled aflibercept; this is not a randomized comparator. Safety across 22 reported no intraocular inflammation or ocular serious events. Small numbers do not exclude rare harm. A 75% burden reduction using future trial criteria is retrospective modeling, separate from the actual 61% projection-based comparison.

4SIGHT: Phase 3 plans 514 treatment-naive DME patients, randomized against aflibercept 2 mg every eight weeks after five loading doses. Vision noninferiority at week 52 is primary; rescue injections remain allowed. Registry lists recruiting and actual start September 18, with January 2029 primary / January 2030 final completion estimates.

U.S. status / next milestone: Investigational; RMAT designation is not approval. Investor Day October 21, 10:30 a.m. Eastern is scheduled, not guaranteed to contain new results.

Original sponsor report; 4SIGHT registry; scheduled webcast.

Remaining gaps: Read current topic/state and September5 overlap. All13 saved registry records plus10 newly registered matches retrieved. Recovered Sept28 4DMT primary release, corroborated new Oct9 4SIGHT record; no duplicate PRIMA36month or MOGENRY priority-review announcements.; Official EURETINA final-session/late-breaking reporting opened. New 4D150 DME findings have primary sponsor counts; PST611 qualitative summary is not an exact efficacy table. PRIMA conference-summary threshold differs from sponsor release and was not silently substituted.; BEST1 fullPDF retried through web and direct network, still403. Actual MCO010 acquired GA tables/trial match, PST611 tables and full DAYBREAK/BRUNELLO safety intervals remain gaps. Hearing acquired-loss inventory not fully refreshed.

Informal-source coverage: Original Oct2 investigator interview by ArshadKhanani discusses SPECTRA/4SIGHT, corroborating the public sponsor development announcement, not proving larger-trial safety. AC102 purported August29 email claim remains unconfirmed; live NCT05776459 retains January16 snapshot with elapsed February/July completion estimates, no posted results. No phase3/tinnitus launch inferred. Wider podcast sweep incomplete.

PRIMA reports three-year prosthetic visual-acuity benefit in a smaller follow-up group

Source date: October 5, 2026. Evidence: Sponsor-reported PRIMAvera 36-month follow-up, NCT04676854, following EURETINA; full peer-reviewed follow-up paper/slides not appraised. The official conference scheduled a 36-month symposium October 3; the release does not specify the exact late-breaking presentation date.

Of 38 implanted participants with advanced geographic atrophy due to AMD, 18 were efficacy-evaluable and 27 safety-evaluable at 36 months. Sponsor reports mean +29 ETDRS letters, versus 24.5 at 12 months, and 14/18 (78%) achieving at least 15 letters/0.3 logMAR improvement. Natural vision was reported stable. This is prosthetic vision using an implant, glasses and magnification.

Limits: Single-arm follow-up; substantial missing efficacy assessments, no intervals or missing-data analysis in the release. Different follow-up denominators prevent assuming the mean increase proves improvement for all 38. Chart performance is distinct from everyday function.

Safety: Three serious adverse events occurred after month 12; none attributed to the device alone. Sponsor reports no new signal during months 12–36; this does not mean no late events or surgical risk.

U.S./next milestone: Sponsor reports commercial access in Europe/UK and U.S. FDA approval pending, without a decision date. Await full follow-up tables, attrition reasons, functional outcomes and real-world safety.

Science Corporation release via Business Wire/AOL; scheduled symposium.

Japan accepts MOGENRY for priority review; approvals remain pending

Source/event date: October 1, 2026. Evidence: Sponsor-reported regulatory milestone; regulator letter independently unappraised.

Nanoscope announced Japan PMDA acceptance and priority review of MOGENRY (sonpiretigene isteparvovec / MCO-010) seeking marketing authorization for inherited retinal dystrophies. The application draws on RP phase 1/2a (NCT04919473), RESTORE phase 2b/3 (NCT04945772) and STARLIGHT Stargardt phase 2 (NCT05417126), plus REMAIN follow-up.

Limits/safety: Review acceptance/priority is not approval or proof of efficacy in all IRDs. This announcement provides no new quantitative comparative efficacy/safety table. The sponsor states no treatment-related serious events were reported, which does not exclude all events or rare harms. Keep inherited diseases separate from acquired AMD/GA and genotype-specific BEST1 therapy.

U.S. status: The separate FDA application for RP with severe vision loss remains under review according to the October 1 source; no U.S. approval or action date established.

Next milestone: PMDA/MHLW and FDA decisions, exact dates undisclosed. Review REMAIN and GA data when actual conference files are recovered.

MCO-010 adds an October 11 four-year RP presentation and dry-AMD poster

Source/event date: October 6, 2026. Evidence: Announced conference milestone; no new numerical results in the release.

At AAO, Kenneth C. Fan is scheduled to present REMAIN four-year follow-up of RESTORE retinitis pigmentosa (PA044, October 11, 11:42–11:49 a.m. Central, rooms 255–257, New Orleans). David S. Boyer’s PO624 dry-AMD poster, available on demand, concerns the first direct-to-phase-2 randomized MCO-010 study. The October 8 Eyecelerator panel/corporate update is a separate development discussion.

Limits/safety/U.S. status: Schedules do not verify delivery, visual benefit, safety or approval. Inherited RP follow-up must not replace acquired AMD/GA data. The actual October 4 EURETINA macular-degeneration abstract/slides remain an access gap; no positive completion is inferred. BEST1 EURETINA slides now appear on the sponsor index, but the PDF returned 403 and was not appraised; retain the corrected September 11 cohort-1 data.

Next milestone: Retrieve actual PO624 and the October 11 RP presentation. Regulatory decisions remain pending without exact verified dates.

Rincell-1 first-human start estimate moves to September 2027

Source/event date: 2026-10-01. Evidence: Trial registry timing revision.

The October 1 update to NCT07032038 moves Rincell-1's estimated study start from November 2026 to September 2027, and estimated primary completion from July 2028 to May 2029; final completion is now estimated for August 2029. The phase 1/2 study remains not yet recruiting, with 20 planned adults eligible for cochlear implantation. It compares an implant plus otic neural progenitor cells with an implant alone. Its primary endpoint is adverse-event frequency and severity through 52 weeks; neural-response testing is secondary. This is a material delay to the expected evidence window. The record gives no reason for the change and provides no efficacy results, demonstrated everyday hearing restoration or U.S. approval.

Next milestone: September 2027 start; May 2029 primary and August 2029 final completion — all registry estimates

MOGENRY application accepted for FDA review

Source/event date: 2026-09-09. Evidence: Sponsor-reported regulatory milestone; recovered watchlist addition.

Nanoscope announced September 9 that FDA accepted for review its BLA for MOGENRY (sonpiretigene isteparvovec; MCO-010) for retinitis pigmentosa with severe vision loss. This is a gene-agnostic optogenetic therapy in an inherited retinal disease. Acceptance is not approval or routine U.S. availability. The release did not state an FDA action date. Supporting RESTORE (NCT04945772) enrolled 27 participants in a randomized sham-controlled trial; the registry labels it phase 2, while the sponsor describes phase 2b/3. The acceptance claim is sponsor-reported; an independent FDA acceptance document was not reviewed. The separate macular-degeneration program should not be conflated with RP evidence. This September event was missing from the notebook; it is not an October approval.

Next milestone: FDA action date not stated; October 4 EURETINA macular-degeneration presentation at 10:51 CEST is a separate program

Source 1; Source 2.

PRIMA 36-month outcomes scheduled at EURETINA

Source/event date: 2026-10-03. Evidence: Official conference schedule; future event. Date basis: Event date is scheduled; schedule publication date unknown; verified October 2.

DAYBREAK meets vision noninferiority endpoints; 54% reach six-month Zenkuda intervals

Source date: 2026-09-28. Evidence: Sponsor phase 3 randomized topline.

Zenkuda/tarcocimab and tabirafusp-ted met vision noninferiority versus aflibercept in treatment-naive wet age-related macular degeneration. DAYBREAK NCT06556368 randomized about 690 participants across three arms. Sponsor defined the primary analysis as mean BCVA change averaged across weeks 40, 44 and 48. Zenkuda reached 24-week dosing in 54% after four monthly loading doses. Aflibercept used labeled dosing; tabirafusp used every-eight-week dosing with additional treatment as needed.

Limitations: Noninferiority is not superior vision. Release omits mean letter gains, confidence intervals and margin; different retreatment schedules prevent a simple matched comparison of maximum durability. Registry still lists a stale 675-person estimate.

Safety: Sponsor: Zenkuda intraocular inflammation 0%, cataracts 0.5%; tabirafusp inflammation 0.4%, cataracts 0%; aflibercept cataracts 0.9%. Full comparative safety tables and longer follow-up remain needed.

U.S. status: Both investigational; Zenkuda is a proposed trade name, not an approval.

Next milestone: Zenkuda three-indication BLA forecast Q4 2026. Separate KSI-101 PEAK topline forecast December 2026; tabirafusp ALTO DME study enrolling.

Kodiak DAYBREAK sponsor release; DAYBREAK original registry; Kodiak first-quarter update: actual approximately 690 enrollment.

Otarmeni reaches a U.S. treatment center outside clinical trials

Source date: 2026-09. Date basis: Month shown on treating-center report; exact dates unknown. Evidence: Treating-center access announcement; recovered baseline. Baseline addition; not new news.

Children’s Wisconsin reports providing FDA-approved Otarmeni outside a clinical trial. The hospital’s September 2026 page calls it the first U.S. center to do so and describes itself as one of five activated centers. Exact treatment and publication days are not supplied.

Limitations: This is an access milestone, not a new approval or new outcome trial. The first-center claim is attributed to the hospital.

Safety: No new post-treatment safety or functional-outcome results supplied.

U.S. status: Existing approval is narrowly for eligible OTOF-related genetic hearing loss; this does not extend to common acquired hearing loss.

Next milestone: Long-term outcomes and the existing FDA-required confirmatory protocol deadline December 30, 2026.

Children’s Wisconsin treatment-center report; FDA Otarmeni product page.

Coverage: September 5–29 overlap, DAYBREAK release/registry, Otarmeni access, AC102 original registry and retinal gene/atrophy conference discovery.

Remaining gaps: DAYBREAK full slides/CI/margin and safety tables not appraised; current PRIMA/other ophthalmology labels and broader hearing registries incomplete. Gildeuretinol Retina Society narrative requires original slides before new result attribution.

Informal-source coverage: AC102 original secondhand-email claim remains unconfirmed; registry still January16 snapshot with elapsed estimates, not proof of completed phase2 or 2027 tinnitus/phase3 launch. New indexed searches did not resolve original email.

Remigromig meets vision noninferiority endpoint but shows more retinal adverse events

Source date: 2026-09-24. Event date: 2026-09-24. Evidence: Sponsor phase 2b/3 randomized topline results.

Both remigromig doses met week-52 visual-acuity noninferiority versus ranibizumab in diabetic macular edema; the safety imbalance is material. BRUNELLO randomized 984 adults 1:1:1 to intravitreal remigromig 0.5 mg, 0.8 mg, or ranibizumab 0.5 mg every four weeks. Remigromig activates Wnt signaling. Trial: NCT06571045.

Limitations: No letter-change estimates, confidence intervals, noninferiority margin or absolute adverse-event rates disclosed. Noninferiority does not establish better everyday vision, retinal regeneration or longer dosing intervals.

Safety: Proliferative diabetic retinopathy, vitreous hemorrhage and adverse-event discontinuations were more frequent with remigromig than ranibizumab. Sponsor says further analyses are underway.

U.S. status: Investigational; not U.S.-approved.

Next milestone: Full first-year results scheduled for AAO October 10, 2026, 5:10 p.m. Central, New Orleans. BRUNELLO continues into a personalized-interval second year; BAROLO is a separate ongoing trial.

Merck BRUNELLO topline release.

OCU400 receives provisional Bahamas designation; treatment access still conditional

Source date: 2026-09-25. Event date: 2026-09-25. Evidence: Sponsor report of foreign regulatory/access milestone.

Ocugen reported provisional approval and priority designation from the Bahamas LARTA Board for OCU400 in inherited retinitis pigmentosa. The sponsor aims to treat the first expanded-access patient within 90 days, contingent on full LARTA approval. This is not evidence that treatment is currently available. Its U.S. phase-3 program continues.

Limitations: No new efficacy or safety data. An independent Bahamas regulator document was not reviewed; regulatory characterization is sponsor-attributed. Keep inherited retinal disease separate from common acquired vision loss.

Safety: No new comparative safety results disclosed.

U.S. status: No U.S. approval; Bahamas provisional status does not establish U.S. or unrestricted commercial availability.

Next milestone: Sponsor forecasts phase-3 topline in Q1 2027 and BLA submission Q2 2027. Expanded-access first treatment remains conditional, with no verified date.

Ocugen Bahamas provisional-designation release.

Coverage: BRUNELLO and OCU400 announcements; AC102, Rincell and paliroden registry refreshes; selected inherited/acquired hearing and retinal-program discovery.

Remaining gaps: Complete EURETINA/Retina Society/AAO source packets, PRIMA and otarmeni label/status not refreshed. AudioCure record is still January 16; actual completion and 2027 program dates unconfirmed. Rincell remains not-yet-recruiting with estimated November start; paliroden estimated January 2027 start, not actual enrollment. New Paliroden trial targets speech-in-noise difficulty with chronic tinnitus

Source date: 2026-09-17. Event date: 2026-09-17. Evidence: New formal human trial registration; no results.

Cilcare registered REACT, a phase 1b/2a study of transtympanic Paliroden (CIL001). NCT07826507 plans 60 adults aged 18–75 with chronic bilateral tinnitus, normal hearing thresholds and speech-in-noise deficit. Randomized placebo-controlled single injection; participant, investigator and outcome assessor masked. Primary endpoint is adverse events through 168 days; secondary endpoints include speech-in-noise and time-compressed-word performance.

Limitations: Not yet recruiting in the September 17 record. This tests presumed cochlear synaptopathy; it does not demonstrate regenerated nerves, restored everyday hearing or a tinnitus cure.

Safety: Safety-first study; no human results in this record.

U.S. status: Investigational; no U.S. approval verified.

Next milestone: Registry estimates: start January 2027; primary completion January 2028; completion June 2028. Preserve month precision and estimate label.

REACT original ClinicalTrials.gov record.

Coverage: Read topic and prior gaps; searched inherited/acquired hearing and vision programs, safety/negative studies and entrants through September22. Verified Paliroden REACT registration; rechecked original AC102, Rincell, PST611 and BIRD1 records. Reviewed older terminated Roche geographic-atrophy and Regeneron uveitis entries as baseline leads, not new September terminations. BGE102/BEST1/PST611 prior events deduplicated.

Remaining gaps: PRIMA/Otarmeni latest labels, regulator safety and commercial availability not comprehensively refreshed. SENS601 August31 authorization/Annexon ARCHERII original older leads and ACHIEVE secondary biomarker full paper still need appraisal. AC102 original record remains January16 snapshot with elapsed July31 completion estimate, not confirmation of phase3/tinnitus plans. PST611 registry enrolled8 vs sponsor treated6 remains a denominator clarification. Recovered RO7669330 GA termination (last posted Aug10: dose-limiting AEs reached stopping criteria) and REGN7041 TITAN uveitis termination (Aug17: unfavorable benefit-risk) are older baseline leads without AE details; consider future baseline additions, not this week’s alerts.

Informal-source coverage: Original Aug29 AC102 secondhand-email thread revisited; no official confirmation of phase3/tinnitus 2027 claim located. September18 Paliroden Reddit lead traced to original registry and promoted as formal registration, not rumor. Additional informal searches were selective.

Early BEST1 gene-therapy findings include a corrected visual-acuity chart

Source date: September 9 announcement; corrected September 11. Evidence: sponsor phase 1/2 topline and SEC correction.

Opus reported early visual-test signals in five BIRD-1 participants on September 9 and corrected the visual-acuity conversion on September 11. NCT07185256; three BVMD participants had three-month follow-up, two ARB participants six months. The company reported gains on at least one visual test per participant; denominators vary by measure. September 11 correction gives treated-eye three-month ETDRS changes +12, +1, −10, +2 and +6 letters, versus fellow-eye +4, −7, −8, −5 and +2. Use revised presentation, not the original mean chart.

Limitations: Open-label n=5, multiple outcomes and mixed follow-up; fellow eyes are not a randomized control. Restricted to inherited BEST1 retinal disease, not common AMD. Registry enrollment estimate 10 predates sponsor cohort expansion.

Safety: No serious adverse events, dose-limiting toxicities or intraocular inflammation reported; related AEs mild/moderate. Long-term and rare risks unresolved.

U.S. status: Investigational; reported FDA endpoint discussions are not approval.

Next milestone: Scheduled presentation / sponsor forecast: EURETINA October 4, 2026, 13:15–13:21 CEST; cohort 2 dosing completion Q4 2026, further data Q2 2027 and potential pivotal dosing in 2027.

PST-611 geographic-atrophy presentation scheduled for October 2

Source date: 2026-09-16. Evidence: announced conference presentation; recovered phase 1 baseline.

PulseSight announced an October 2 EURETINA presentation. The six-person phase 1 findings substantially repeat its May ARVO report, rather than constituting a fresh efficacy trial. PST-611-CT1 / NCT07024732 studies nonviral transferrin-gene delivery. Sponsor treated analysis: six patients, two dose levels, 16 weeks. May report says BCVA remained stable and cites exploratory anatomical/patient-reported signals. Registry lists eight enrolled and completion April 7; enrollment is not necessarily the treated analysis denominator.

Limitations: Uncontrolled, safety-focused study with no quantified comparative efficacy. September release also says twelve-month study in its boilerplate despite four-month follow-up in the body; do not infer validated twelve-month benefit.

Safety: May report: predominantly mild ocular AEs, two moderate events, no treatment-emergent SAE or intraocular inflammation reported.

U.S. status: Investigational, not U.S.-approved.

Next milestone: Scheduled conference presentation: EURETINA Session 6 #138, 13:05 in Vienna. May sponsor forecast: phase 2a start H2 2026 and results 2028, contingent on authorization; actual start not verified.

September 16 announcement; May 11 original topline; PST-611-CT1 registry.

Coverage: Read topic and prior gaps; checked acquired/inherited hearing and retinal trial news/safety, original OPGxBEST1 releases plus correction, PST611 repeat data and milestone, direct Rincell/AC102/PST611/BIRD1 records. Distinct populations retained. This is a focused review, not a complete re-review of every program.

Remaining gaps: PRIMA/Otarmeni current labels/regulator safety and availability not comprehensively refreshed. SENS601 August 31 authorization/Annexon ARCHERII older leads remain source-appraisal gaps; avoid calling September reposts new. ACHIEVE secondary biomarker study identified, likely first online September 3 (outside overlap), full source403 and date not confirmed from primary; no effect claim promoted. AC102 registry January 16 still active-not-recruiting with elapsed July 31 estimated completion; does not confirm rumored phase3/tinnitus plans. PST611 registry8 enrolled versus sponsor6 treated requires clarification; no unsupported pooling.

Informal-source coverage: Original AC102 Reddit reported-email source reviewed, exact post date recovered from indexed original as August 29. 2027 trial forecast remains unconfirmed. Other hearing/vision Reddit findings were stock reposts or general timelines; no independent corroboration.

Full coverage record.

Next expected update: Rincell-1 start estimated November 2026; Otarmeni protocol due December 30, 2026. QUELL-DME topline forecast H2 2027; PRIMA milestones undated.

September 14, 2026 — focused update

BGE-102 / QUELL-DME — first dosing announced September 8, 2026. BioAge’s randomized Phase 2 trial, NCT07812090, plans 180 adults with diabetic macular edema. The three groups compare anti-VEGF plus oral placebo, anti-VEGF plus BGE-102, and BGE-102 with sham injection. The primary outcome is best-corrected visual acuity at 12 weeks; retinal and inflammatory biomarkers are exploratory. No DME efficacy or comparative safety results are available. This is an investigational oral NLRP3 inhibitor, not proof of geographic-atrophy benefit or a new approval. The September 10 registry posting lists recruiting, an actual August 21 study start and estimated November 2027 primary completion. September 8 is the first-dose announcement date, not a separately verified dosing day. The sponsor forecasts H2 2027 topline; neither forecast nor registry status guarantees a result date or enrollment at every site. Sponsor announcement; current registry.

Rincell-1 recheck: The current registry, checked September 14, still returns the August 11 not-yet-recruiting record, November 2026 estimated start and July 2028 estimated primary completion. No actual start is inferred.

Tracked programs and current evidence

Hearing: Otarmeni — real approval, narrowly genetic indication

Otarmeni / lunsotogene parvec-cwha / DB-OTO received FDA accelerated approval on April 23, 2026 for pediatric and adult patients with specified severe-to-profound/profound hearing loss caused by molecularly confirmed biallelic OTOF variants, preserved outer hair-cell function, and no prior cochlear implant in the treated ear. A dual-AAV treatment delivered surgically into the cochlea supplies functional otoferlin. This indication does not extend to ordinary age- or noise-related hearing loss. FDA approval letter; FDA product page.

The evidence included an ongoing single-arm study of 24 children aged 10 months to 16 years; 16 of 20 efficacy-evaluable patients (80%) improved hearing. That percentage does not mean 80% achieved normal hearing. Comparison with untreated natural history is compelling for this otherwise persistent genetic defect, but the cohort is small and adult, speech-development, and durability evidence require further confirmation. Common adverse effects included middle-ear infection, nausea, dizziness, and procedural pain. FDA evidence summary.

The associated CHORD study is NCT05788536. FDA requires additional outcomes including sustained hearing, speech/language, and quality of life, with a confirmatory final report due August 30, 2030. Earlier interim publications may occur, but no firm next public readout date was verified. This regulatory deadline is not a prediction that no new data will appear before 2030. CHORD; required confirmatory study and deadlines.

Hearing: Rincell-1 — first human study relevant to auditory nerve degeneration

Rincell-1, Rinri's otic neural progenitor cell therapy, is intended to replace damaged auditory neurons. The UK phase 1/2a study NCT07032038 plans 20 participants in presbycusis and postsynaptic auditory-neuropathy cohorts, randomized between cochlear implantation plus cells and implantation alone. These are patients already eligible for cochlear implants, not people with mild loss or an established treatment for tinnitus. The first study primarily evaluates safety; physiological measures of neural function are not yet proof of better conversational hearing. Trial registry; sponsor clinical-research description.

The registry record updated October 1, 2026 lists not yet recruiting, an estimated September 2027 start, May 2029 primary completion, and August 2029 final completion. These supersede the earlier November 2026 start and July 2028 primary-completion estimates; the reason for the delay is not stated. UK authorization to conduct this trial is not marketing approval; no U.S. approval or demonstrated human benefit was identified. Current registry; July 2025 trial-authorization announcement.

Vision: PRIMA — functional central vision with a retinal prosthesis

PRIMA, Science Corporation's subretinal photovoltaic implant used with external glasses, targets central visual loss from geographic atrophy due to age-related macular degeneration. It is a restorative device, not a drug that regrows a normal retina. In the peer-reviewed PRIMAvera study, NCT04676854, 38 people received implants and 32 were assessed at 12 months. 26/32 (81%) improved visual acuity by at least 0.2 logMAR; the estimate incorporating missing participants was about 80%. The study was single-arm, so these results require careful interpretation alongside device-assisted testing, selection, and attrition. Primary publication, online October 20, 2025.

There were 26 serious adverse events in 19 participants; most occurred soon after surgery, and most early events resolved within two months. Natural peripheral acuity was preserved on average. Improved chart performance is encouraging, but durability, everyday reading, training burden, device reliability, and surgical risk remain central outcomes. Primary study and safety results.

On July 22, 2026, Science reported CE marking and a European commercial rollout. That is a concrete regulatory advance, although reimbursement and individual site availability are separate. The same company announcement says PRIMA remains investigational in the U.S.; a humanitarian-use designation is not FDA marketing approval. No firm U.S. decision date was verified. Company release, syndicated from Business Wire.

Rumors and informal signals

September 29 informal-source check

AC102 phase 3 and tinnitus plans — unconfirmed. Fresh original registry API remains January 16, active-not-recruiting, with February 28/July 31 estimated completion. No verified confirmation of reported 2027 phase 3/tinnitus plans; no repeat alert.

Source; Source.

September 27 status check: Existing claims remain unconfirmed; no material status change or repeat alert.

  • ac102_reddit_email_2026_08_29_phase3_tinnitus_2027 — unconfirmed: Original purported-email claim remains unconfirmed. NCT05776459 still reflects January 16, active-not-recruiting, with estimated February 28 primary completion and July 31 study completion; neither is actual. No official confirmation of conditional 2027 phase-3/tinnitus plans found. Acute sudden hearing loss is distinct from chronic tinnitus. Original AC102 Reddit thread; ClinicalTrials.gov NCT05776459; AudioCure tinnitus research statement.

2026-08-29 — Rumor — unconfirmed

Reddit user ferttt2 says an AudioCure email reported completion of the active acute-sudden-hearing-loss study phase and conditional phase3 and possibly tinnitus trials in 2027.

ferttt2, r/tinnitusresearch; purported sponsor email, no original email headers or official release. Direct page shows text and author; indexed original gives August 29 date. Completion and plans remain secondhand. Current NCT05776459 record is stale and does not confirm 2027 dates; it concerns acute ISSNHL, not demonstrated chronic-tinnitus efficacy.

September 14, 2026 — focused ad-hoc review

Added the September 8 QUELL-DME first-dosing announcement and September 10 registry record, with study initiation clearly separated from results. Rechecked Rincell-1’s still-estimated November start. Full details and primary links are in the focused update.

Coverage gaps: Common acquired-hearing and vision preservation were searched selectively. OTOF/PRIMA full regulator and safety follow-up were not comprehensively refreshed. A September 3 SENS601 French-authorization report and Annexon ARCHER II timing surfaced, but the direct source/design chain was not fully appraised; they are follow-up leads, not new confirmed entries here. An informal hearing-restoration discussion offered speculative timelines without a precise attributable program milestone. No new rumor was promoted and no comprehensive podcast/transcript sweep was performed.

Dated findings

  • 2026-09-12 baseline: Otarmeni approval and PRIMA European status are already-established events; Rincell-1 is an early safety study with registry-estimated dates. Future additions can broaden the vision-preservation and common-hearing-loss subtracks as new human evidence emerges.
