Current state
A new P11-4 protocol tests repeated versus single application for early enamel lesions. The primary endpoint and completion timing differ between the paper and registry; no clinical results are yet reported.
Status: active. Selective initial baseline completed September 12, 2026. Includes orthodontic alignment explicitly, alongside tooth, enamel and periodontal repair. Follow human research worldwide and identify U.S. availability separately.
Questions to follow
- Which advances in clear aligners, braces, materials, attachments and treatment planning improve the final bite and visible alignment, reduce refinements, or make treatment less burdensome?
- Do acceleration methods shorten the whole course, while preserving root length, gum health and stability? Track photobiomodulation, other devices and formal drug programs without assuming a claimed mechanism establishes benefit.
- Can clinician-supervised remote monitoring improve outcomes and access? Separate fewer appointments, algorithm accuracy, and faster manufacturing from faster or better tooth movement.
- Which retainers and wear schedules reduce relapse, breakage and unwanted movement over years?
- Can enamel remineralization, periodontal regeneration or new-tooth development preserve natural teeth? Distinguish congenital missing teeth from teeth lost in adulthood, and early enamel lesions from missing bulk tooth structure.

Baseline: long-term tooth preservation after periodontal regeneration and orthodontics
Baseline context, not new October news. This paper appeared online May 28, 2026 despite an October journal issue. It followed 48 people with Stage IV periodontitis and one severely compromised, displaced non-molar tooth each after periodontal regenerative surgery, later orthodontics and tailored supportive periodontal care.
Thirty-three participants completed 20-year follow-up. Estimated test-tooth survival was 89.1% (CI 79.4–99.9); complication-free probability was 86.3% (CI 75.7–98.3). Five test teeth had complications—one pulp necrosis, two root fractures and two periodontal recurrences—and four were extracted.
Limits and practical meaning: This uncontrolled selected cohort cannot separate the effects of surgery, orthodontics and continuing care, or establish superiority to extraction/implant strategies. Attrition was 31.2%; sensitivity scenarios ranged from 60.4% to 91.7% tooth survival depending on missing participants. The finding concerns preservation of existing teeth and periodontal support, not growing new teeth or proving a novel U.S. indication. Further comparative follow-up has no verified date.
Remaining gaps: Read current canonical README, HOW_THIS_WORKS, WATCHLIST, catalog, state, LATEST, MILESTONES, LANDSCAPE and this entire topic page. Do not advance full successful-check timestamp.; Appraised an older May 28 online 20-year periodontal-regeneration/orthodontics cohort despite its October issue. Baseline context only, not new October news; valuable because it measures tooth survival and complications rather than a regeneration marker.; October 15 EV registry update NCT07867067 remains not yet recruiting, 22 planned, no results; affiliation/formatting changes do not establish treatment. Other October 14–16 small registrations include orthodontic acceleration, bracket bone/root outcomes, extraction-site volume preservation and endodontics; no outcomes. Untested ridge preservation is not whole-tooth regeneration.; SLCTR/2026/026 ALIGN-PBM is pending, 34 planned in Oman, October 15 anticipated start with no first-enrollment date; it studies pain after aligner change, not total treatment duration, final bite, root/gum health or retention. Formal registry checked as a discovery example, not an efficacy claim.; Toregem primary Japanese Phase 1/2 record, first Phase IIa dosing, every directly printed/shape-memory aligner program, long-term retention and root/gum safety, and complete new-tooth efficacy remain unresolved.
Informal-source coverage: October 9 Very Dental Marco Maiolino episode description and October 13 ADA Smart AI episode listing inspected. These discuss preservation/workflows, not newly measured regeneration or orthodontic benefit; no timestamped result claim extracted and full audio/transcript not reviewed. Old tooth-regrowth launch forecasts remain unconfirmed.

P11-4 protocol tests repeated application, with endpoint and timing discrepancies
Protocol published October 7, 2026; newly appraised October 13. NCT07664046 plans 75 adolescents aged>10 and<18 with noncavitated smooth-surface enamel white-spot lesions. Three randomized groups receive one P11-4 application plus fluoride varnish, applications at baseline/3 months plus varnish, or varnish alone. QLF assessments are blinded; multiple lesions within patients are handled by mixed models. Fixed-appliance patients and cavitated lesions are excluded, limiting direct application to post-orthodontic lesions or missing tooth structure.
No results or observed harms are reported. The paper designates the 6-month ΔQ lesion-volume comparison between repeated/single treatment as primary; the June 23 registry instead lists ΔF and ΔQ changes at 12 months. Its effect-size assumption comes from P11-4-versus-fluoride evidence, not the actual repeated-versus-single contrast. These differences need reconciliation before interpreting later results.
Timing: Registry remains recruiting, actual start February 2, 2026, estimated primary October 31, 2027 and final December 14, 2027. Paper abstract expects July 2027 completion; its later results wording is also ambiguous. Neither is a promised publication date. U.S.: no new formulation-specific authorization established. Next: protocol/registry alignment, then controlled clinical durability and safety; fluorescence remineralization is not replacement of lost enamel.
Full protocol; original registry.
Remaining gaps: Read canonical README, HOW_THIS_WORKS, WATCHLIST, catalog, state, LATEST, MILESTONES, LANDSCAPE and assigned current topic. Small periodontal extracellular-vesicle trial enters the human registry
Source date: October 9, 2026 first registry posting.
Ain Shams University's NCT07867067 plans 22 adults aged 20–40 with persistent intrabony periodontal defects after initial therapy. The randomized, masked Phase 2 protocol compares a beta-tricalcium-phosphate scaffold/collagen membrane with or without small extracellular vesicles cultured from the participant's own gingival mesenchymal cells. Six-month radiographic bone fill is primary; attachment gain, early healing and local adverse effects are secondary. No results are posted.
Status and limits: Not yet recruiting despite an elapsed October 1 estimated start. Primary/final completion May 1/November 1, 2027 are estimates. Small size, imaging endpoints and short follow-up cannot establish histological regeneration, durable tooth retention, whole-tooth regrowth or long-term cellular-product safety. Structured arm numbering differs from the narrative; clarify the final allocation and protocol before result appraisal. No U.S. authorization for this adjunct is established. Next: actual start, product-quality/harms data and a complete controlled report; publication date unknown.
Remaining gaps: Read the current assigned topic and canonical scope/state. Named original registry records refreshed; all pages of broad ClinicalTrials.gov discovery for September 5–October 9 were retrieved (288 records, with incidental matches excluded). This honors the seven-day overlap from the saved September 12 full successful check.; Europe PMC original-literature discovery again returned HTTP 503. Targeted primary publisher, sponsor and conference searches were performed, but the worldwide paper/regulator/procedure and original-audio census remains incomplete. Preserve last_successful_check_utc.; P11-4 official abstract https://sciforum.net/paper/34045 remains the same October2 material already appraised; conference October7–9 does not prove extra results or actual delivery. Direct printed-aligner September24 publication and registry status deduplicated. NCT07496892 still not yet recruiting with stale elapsed estimates; OrthoPulse status unchanged.; Toregem news now has October6 BioJapan business presence but no PhaseIIa first dosing/results verification. Japanese original jRCT matched PhaseIIa and complete PhaseI numerical safety remain unresolved. Numerous new October6–9 retrospective/historical bracket-bonding, mouthrinse, pulpotomy and movement records lack outcomes; their registration was not relabeled as new demonstrated clinical benefit. Full alignment/root/gum/final-bite/retention and new-tooth/repair census remains incomplete.

P11-4 abstract reports fluorescence gains without superior visible masking
Source date: October 2, 2026 abstract publication. Meeting: October 7–9 online dentistry conference.
A 45-person randomized post-orthodontic white-spot-lesion study compares P11-4, fluoride varnish and no treatment in adults aged 18–30, followed six and twelve months. The abstract reports a fluorescence ΔF advantage over varnish (p<.001), but no P11-4-versus-varnish ICDAS difference, and no lesion-area advantage over untreated controls. Its own conclusion says visible masking was similar. Effect sizes, confidence intervals and usable safety counts are absent; QLF is a mineralization surrogate, not replacement of missing enamel. A matching investigator/title/intervention registry, NCT06749977, describes a 2020–2023 study registered in December 2024. This is newly available conference material, not a newly completed trial; registry assessment times also need reconciliation with the abstract. Next: full report, safety/disposition, and replication; the October 7–9 presentation is scheduled, not guaranteed new data. No new U.S. formulation-specific authorization established.
Official abstract; matching original registry.
Registered study compares local drugs and micro-osteoperforation for alignment
Source/event date: September 25, 2026 first posting; recovered October 6.
NCT07842120 is a recruiting randomized Phase 2 study of 60 planned participants comparing micro-osteoperforation, local prostaglandin E1, local calcitriol and control. The primary endpoint is mandibular anterior alignment measured by Little's Irregularity Index at six months. It reports an actual April 16 start, predating registration; primary completion December 30, 2026 and final February 2027 are estimates. No results or comparative harms are posted. Short-segment movement cannot establish a shorter whole orthodontic course, better final occlusion, preserved roots/gums or durable retention. The registry's masking claim needs protocol appraisal because an invasive procedure and local injections may be distinguishable. This is formal human investigation, not a validated routine acceleration protocol or new U.S. drug indication. Next: protocol, full-course/root/gum outcomes and actual results; no promised publication day.
Periodontal platelet-fibrin and oxygen-gel pilot registered after completion
Source date: October 5, 2026 first registry posting. Reported actual study completion: July 10, 2026.
NCT07859358, Assiut University, discloses a completed randomized participant-masked Phase 1/2 study in 24 adults with Stage II periodontitis. Scaling/root planing is paired with injectable platelet-rich fibrin, peroxoborate oxygen-releasing gel or saline control. Planned primary measures are clinical attachment, pocket depth and bleeding through 12 weeks. No results are posted. Registration followed completion; the reported March 10 primary-completion date also needs reconciliation with January 20 start and the 12-week primary endpoint. These measures do not by themselves establish regenerated periodontal structure, long-term tooth retention or whole-tooth regrowth. No U.S. indication/availability claim is established. Next: full controlled outcome/harms report and timing clarification; publication date unknown.

OrthoPulse records remain completed/noresults or unknown/stale. Toregem Oct2 global startup support and Oct6 BioJapan booth announcements add no first-dosing or efficacy verification; phaseIIa August17 trial-notification remains separate from approval.; New P114 official conference abstract and matching original record appraised. Additional new orthodontic registrations disclose historical completed bracket/laser/ultrasound studies without results; no efficacy selected. Full printed-aligner/root/gum/retention papers, Japanese jRCT PhaseIIa primary record and broad repair/regeneration census remain unappraised; EuropePMC503.; Focused source checks completed, but full-topic coverage incomplete; preserve original last_successful_check_utc. Awaited printed-aligner trial now has a randomized publication
Source/event date: 2026-09-24. Evidence: Peer-reviewed randomized trial abstract plus registry; full text not appraised.
A September 24 publication reports the previously watched NCT06685029 trial: 52 participants randomized among directly printed aligners, Invisalign, ClearCorrect and fixed appliances. Treatment durations were 6.59±1.42, 5.36±1.63, 5.72±1.88 and 10.16±2.28 months, respectively; the fixed-appliance group took longer than each aligner group (p<0.001). The abstract does not establish direct printing as faster than other aligners. Between-group differences in salivary BPA and percentage PAR bite-score improvement were not significant; that does not prove equivalent toxicity or final bite quality. The registry restricts interpretation to ages 15–24 with non-extraction Class I mild-to-moderate crowding (0–6 mm) and healthy periodontium. Root/gum outcomes and long-term retention remain unanswered. Registration occurred after the study began; registry results are still absent, but a publication is now available. The abstract reports no harmful effects without detailed event counts. Publication; registry.
Next milestone: Independent replication and retention follow-up: no announced date verified.
Coverage: direct printed aligners randomized trial September24 2026 NCT06685029; dental alignment regeneration September October 2026 clinical trial TRG035; site:reddit.com dental tooth regrowth September2026.
Remaining gaps: Full orthodontic paper could not be retrieved. NCT07496892 still not yet recruiting in March record although September completion estimates have elapsed; no presumed completion. Toregem phaseIIa first dosing and full regeneration/retention/root-safety inventory remain unresolved.
Informal-source coverage: Repost of an older forecast; no new development established; https://www.reddit.com/r/Biohackers/comments/1wq1adh/910_dentists_probably_wont_recommend_this/; Sept25 repost repeats possible2030 tooth-regrowth availability; no new controlled human results. Orthodontic telemonitoring study finds fewer recorded complications, with important selection bias
Source date: 2026-09-28. Evidence: peer-reviewed prospective comparative cohort.
A clinician-supervised photo-monitoring pathway plus caregiver education was associated with fewer recorded problems during childhood palate expansion. One Italian center enrolled 100 children aged 7–11: 50 caregiver-selected telemonitoring, 50 conventional weekly office follow-up. Recorded complications: 12/50 (24%) versus 23/50 (46%); unadjusted RR 0.52 (95% CI 0.29–0.93). All completed follow-up. Unscheduled visits: 12% versus 20%, RR 0.60 (0.24–1.53), not significant.
Limitations: Nonrandomized self-selection, unequal opportunities to detect transient problems, unmeasured caregiver differences and no adjusted analysis. Education and monitoring cannot be separated. Total visits, whole-course duration, final bite, roots and long-term retention were not established.
Safety: No documented delayed detection in either small cohort does not prove equivalent safety. Photo review retained in-person assessments and escalation when needed.
U.S. status: No regulatory action; evidence concerns a clinical follow-up pathway.
Next milestone: Randomized multicenter replication and longer retention follow-up remain undated.
Coverage: Selective September 5–29 overlap, emphasizing September 27–29; prior September 27 findings deduplicated. Appraised full September 28 caregiver-selected telemonitoring cohort; refreshed direct-printed aligner registry and Toregem original news; searched enamel/repair, acceleration, retention and root/gum outcomes.
Remaining gaps: Toregem archive still ends in August; Phase IIa first dosing unverified. Direct-printed aligner trial remains not yet recruiting despite elapsed September estimates. Broader retention/acceleration studies and complete jRCT record not appraised.
Informal-source coverage: September25–26 tooth-regrowth posts repeat known TRG035 background and extrapolate to implants without new human results; no material rumor status change. Orthodontic podcast/transcript search remains selective.
Next update: TRG035 Phase IIa dosing unverified; direct-printed aligner completion windows stale. Randomized telemonitoring and retention follow-up remain undated.

Two-year pulp-preservation trial finds no superiority of a newer cement
Source date: 2026-09-18. Event date: 2026-09-18. Evidence: peer-reviewed randomized human study; full text and registry appraised.
Two cements had similar observed two-year outcomes after partial pulpotomy, a tooth-preserving procedure; the study did not demonstrate regrowth of missing teeth. A single-center, single-operator Spanish study reports 139 treated permanent molars: success in 58/64 (90.6%) with White MTA Angelus and 69/75 (92.0%) with NeoPutty at 24 months, p=.774. All 139 reportedly completed follow-up. The comparison was designed for superiority, so the null result does not prove equivalence.
Limitations: The 24-month assessment extended the initial follow-up. Important unresolved registry discrepancies: NCT06881615 lists 71 actually enrolled, versus 139 in the paper, and was first posted after study completion. These inconsistencies reduce confidence in prospective reporting; longer-term structural preservation remains uncertain.
Safety: Twelve failures required conventional root-canal treatment. The paper does not provide a separate comprehensive adverse-event table.
U.S. status: No new U.S. authorization or indication is established by this publication.
Next milestone: Resolve paper–registry discrepancies; longer multicenter comparative follow-up has no announced date.
Original open-access study; Primary trial registry.
Coverage: Searched tooth regeneration, preservation/repair, aligner/braces materials, monitoring, acceleration and retention. Appraised full September 18 pulpotomy RCT and registry; refreshed direct-printed aligner record and Toregem news.
Remaining gaps: Pulpotomy paper/registry enrollment and follow-up discrepancies unresolved. Direct-printed-aligner NCT07496892 still not yet recruiting despite September 20 estimated completion. Toregem news still ends with August events; jRCT original page did not render, Phase IIa first dosing unverified. New retention/root/gum and enamel trials not comprehensively appraised.
Informal-source coverage: September 25–26 Reddit tooth-regrowth posts repeat TRG035 background and extrapolate to replacing implants/dentures, without new original efficacy evidence; not a new trial milestone. Wear-time sensors do not improve orthodontic treatment outcomes in a small trial
Source date: 2026-09-15. Event date: 2026-09-15. Evidence: peer-reviewed randomized controlled human trial; abstract and registry appraised.
Adding wear-time monitoring did not significantly improve satisfaction, shorten treatment or increase successful bite correction. NCT05213338: publication reports 58 adolescents aged 10–15 using twin-block or headgear activator appliances, randomized to feedback from thermosensitive wear sensors or no monitoring. Investigators used intention-to-treat and per-protocol analyses; dropout was 21%.
Limitations: One center, no patient/clinician blinding. The current registry lists 52 enrolled versus 58 in the paper; unresolved. The 13-month treatment figure applies only to 39 successful cases, not a randomized between-arm advantage. Findings concern these removable functional appliances, not all AI/remote monitoring.
Safety: No harms detected in the abstract; full adverse-event reporting not appraised.
U.S. status: No new U.S. authorization; a monitoring technology does not automatically improve treatment.
Next milestone: Replication, longer-term stability and full-data appraisal undated.

Customized brackets trade shorter observed treatment for more breakages
Source date: 2026-09-11. Event date: 2026-09-11. Evidence: peer-reviewed retrospective comparative cohort; original abstract appraised.
A small private-practice comparison found shorter treatment with customized ceramic brackets, alongside substantially more bracket failures. Seventy-five patients: LightForce 36, conventional appliances 39. Treatment duration was 16 versus 19 months; at least one bracket failure occurred in 80.6% versus 43.6%. Scheduled visits were fewer but bracket-related emergency visits more frequent with LightForce. Final total ABO-OGS scores were 29 versus 25, p=.061; marginal-ridge scores were worse with LightForce.
Limitations: Retrospective, nonrandomized, one practice. Similar baseline severity does not remove selection/confounding. Nonsignificant final-score difference is not proof of equivalence. No long-term retention or comprehensive root/gum analysis.
Safety: Bracket failures and extra emergency visits are meaningful treatment burdens; long-term safety not established.
U.S. status: No new approval or product-availability conclusion.
Next milestone: Prospective replication and retention follow-up undated.
Original abstract; Original DOI.
Stem-cell adjunct does not establish added orthodontic benefit in a small pilot
Source date: 2026-09-09. Event date: 2026-09-09. Evidence: peer-reviewed nonrandomized prospective human pilot; abstract appraised.
Surgically accelerated orthodontics had shorter observed treatment than conventional care, but adding dental-pulp stem cells did not show a significant extra bone benefit. Thirty adults selected conventional treatment, corticotomy plus bone allograft, or that surgery plus cultured dental-pulp stem cells in a scaffold. Twenty-two completed: 9, 7 and 6 respectively. Both surgical groups had approximately 13.5 months of treatment versus 24.8 months for controls; added-cell versus graft-only bone-thickness difference was nonsignificant.
Limitations: Patient-preference allocation, small groups and attrition; qualitative CBCT grading. The surgical procedure, graft and cells cannot be credited equally, and nonrandomized duration differences are not proof of causality. No durability/retention conclusion.
Safety: No serious adverse events observed during available follow-up; small study cannot establish cell-product or surgical long-term safety.
U.S. status: Investigational cellular adjunct; no U.S. routine-use authorization verified.
Next milestone: Larger randomized study and long-term follow-up undated.
Original abstract; Original DOI.
Coverage: Searched human aligner/braces materials, clinical outcomes, monitoring, acceleration, retention, root/gum health, regeneration and repair. Appraised negative wear-sensor RCT, LightForce comparative cohort and cell-adjunct orthodontic pilot. Rechecked printed-aligner and new self-ligating comparison registries, and Toregem primary news.
Remaining gaps: The September 20 final-completion estimate for NCT07496892 has now elapsed; record still not yet recruiting, last updated March 27, and no results. Remove any implication that the study completed. Toregem jRCT Phase 1 original page again returned 403; Phase IIa dosing and a matched registry remain unverified. Sponsor news still ends with August leadership/trial-notification events. Wear-sensor paper versus registry enrollment discrepancy (58 versus 52) unresolved; full comparative papers and complete safety/root/retention outcomes not appraised. New Bangladesh university aligner-versus-braces study is listed ongoing September 2026–August 2027, but design details and matched registry remain unverified; not promoted as an efficacy event. NCT07638397 was posted earlier and remains not yet recruiting despite September 15 estimated start.
Informal-source coverage: Targeted Reddit and podcast searches. September 14 Kliq This episode description repeats speculative tooth-regrowth-by-2030 discussion; no named new clinical source or verified transcript. Promotional aligner-speed posts and old generic tooth-regrowth forecasts were not repeated as new signals.
Next update: Direct-printed-aligner registry retains not-yet-recruiting status despite elapsed September 20 estimated completion. Actual progress, TRG035 enrollment and durable comparative outcomes remain unverified.

Silica-enriched toothpaste improves sensitivity but not measured mineral gain
Source date: 2026-09-12. Evidence: peer-reviewed pilot randomized human study; abstract reviewed.
A pediatric enamel-defect trial found a pain-sensitivity signal but no superiority over fluoride toothpaste on its mineral-gain endpoint. RBR-6d3g5hp / U1111-1327-7119; triple-blind pilot, 30 children aged 6–12, 95 teeth, 45 days. REFIX Booster versus 1450 ppm fluoride. Between-group change in fluorescence ΔF: −0.43 percentage points, 95% CI −2.73 to 1.86, p=.711; ΔFmax also nonsignificant. Sensitivity improved more, p=.011. Models accounted for teeth clustered within children.
Limitations: Short pilot; fluorescence is a surrogate, not replacement of missing enamel or tooth regeneration. No relevant aesthetic difference; secondary pain signal needs replication.
Safety: No adverse effects reported in this small short study.
U.S. status: No new U.S. authorization or availability established.
Next milestone: Definitive trial and durability follow-up undated.
Original abstract; Original DOI.
Platelet concentrates improve early extraction healing and selected bone dimensions
Source date: 2026-09-11. Evidence: peer-reviewed randomized human study; accepted early article.
A 60-person extraction trial found early healing and imaging gains with blood-derived concentrates; it did not study new teeth or long-term tooth retention. TCTR20251028003, retrospectively registered. Impacted lower-third-molar extraction; 20 each advanced platelet-rich fibrin, concentrated growth factors, natural healing. Both interventions improved early pain/swelling/healing. At 3 months A-PRF buccal-wall height change +1.32 ± 0.72 mm versus +0.01 ± 1.6 mm with CGF and −0.07 ± 1.12 mm controls; both preserved width.
Limitations: One small extraction setting; accepted version may be edited. Three-month radiographic dimensions do not establish durable functional regeneration or superior implant survival.
Safety: Accessible abstract does not provide a complete adverse-event table.
U.S. status: No new FDA authorization or generalized dental-regeneration claim.
Next milestone: Longer comparative follow-up undated.
Coverage: Searched alignment, retention, root/gum health, extraction healing, enamel repair and tooth regeneration. Appraised new REFIX enamel-defect pilot and A-PRF/CGF extraction RCT. Reviewed Sep 9 retrospective clear-aligner movement-predictability paper as a lower-priority lead. Refreshed direct-printed-aligner registry; checked Toregem news for first dosing/results. This is a focused review, not a complete re-review of every program.
Remaining gaps: Toregem matching Japanese jRCT Phase IIa registry and actual first dosing were not independently retrieved; August 17 regulatory-notification review is not efficacy or U.S. availability. Direct-printed-aligner study remains not yet recruiting despite elapsed estimated start/primary dates and an estimated September 20 final completion; do not forecast a result from those stale dates. New alignment paper was retrospective, 61 patients and 447 teeth, addressing initial-series movement prediction rather than whole-course speed, root injury or retention; no alert promoted. REFIXX/REFIX and extraction-study full safety tables were not obtained; abstract-level limitations retained.
Informal-source coverage: Targeted Reddit and podcast/talk searches found repeated tooth-regrowth timelines and missing-original social claims, mostly older posts. No new attributable in-window trial milestone rumor verified; generic 2030/2033/2035 availability predictions were not promoted.
Next expected update: TRG035 Phase IIa enrollment/dosing after August 2026 trial review; no firm dental readout date verified. Four orthodontic registry records were retrieved September 14; missing results and stale status estimates remain follow-ups.
Tracked programs and current evidence
Alignment: newer materials and treatment schedules
Directly printed and shape-memory aligners: registered human comparisons include NCT06685029, comparing direct-printed and conventional aligners with fixed appliances and assessing effectiveness, efficiency and material release, and NCT07496892, comparing movement accuracy of printed and thermoformed aligners. A posted study protocol supports the latter's research scope. The live registry/API was inaccessible at the September 12 baseline; the September 14 review below resolves those record fields. Registration or completion does not establish superiority or universal U.S. availability.
Faster tray changes: an August 22, 2026 randomized study analyzed 38 adults after 45 were randomized and seven excluded for noncompliance. It compared three-day trays with smaller programmed movement against seven-day trays with larger movement. The three-day arm needed more refinements and visits, without a significant improvement in total treatment duration or final occlusion. Thus, this does not support simply changing an existing prescription more frequently. Limitations include the small single-center population, exclusions after randomization and a missing usable registration ID in the abstract. Primary paper.
Monitoring, acceleration and keeping the result
DentalMonitoring: a randomized trial of 56 patients found about 1.5 fewer appointments with remote monitoring, but treatment lasted about 1.9 months longer; refinement and total-aligner counts did not significantly differ. It involved one orthodontist and could not blind participants or clinicians. This is a useful baseline for evaluating later AI-monitoring claims. Trial paper; ACTRN12620000475943.
Photobiomodulation: keep OrthoPulse tooth-movement/treatment-time study NCT02954133 and pain study NCT05361616 on the named watch. Pain relief is a separate endpoint from acceleration. Registry status was not recovered at the September 12 baseline and is updated below. No efficacy or speed conclusion is inferred from registration or completion alone.
Retention: a five-year randomized study initially enrolled 104 adolescents comparing lower removable vacuum-formed retainers with bonded retainers, while both groups used upper removable retainers. Lower-tooth alignment favored bonded retention, with attrition and the adolescent population limiting generalization. Continue following CAD/CAM retainers, long-term relapse, breakage and periodontal outcomes rather than assuming a newer retainer design is better. Five-year primary study.
Enamel and periodontal repair
P11-4 self-assembling peptide: a January 2026 triple-blind trial randomized 25 adolescents receiving fixed orthodontics to peptide-plus-fluoride gel or fluoride gel alone. At 18 months, initial lesions affected 7.2% versus 14.5% of evaluated surfaces. This is encouraging prevention/remineralization evidence, but surfaces are not independent patients and patient-level power was limited. It does not demonstrate regrowth of missing chunks of enamel. Exact formulation and U.S. regulatory claims require product-specific verification. Primary study; DRKS00028048.
Periodontal regeneration: recombinant FGF-2/trafermin and enamel matrix derivatives provide established human trial comparators for future programs. In one phase III trial, FGF-2 improved radiographic bone fill, while the other co-primary endpoint—clinical attachment gain—did not significantly differ from placebo. Keep bone fill, gum attachment and long-term tooth retention distinct. New indications, delivery systems and cell-based approaches are in scope when human studies appear. Phase III primary publication. This older evidence is baseline context; current U.S. product availability was not established here.
Tooth regeneration
TRG035 / anti-USAG-1, Toregem: the sponsor announced on August 17, 2026 that Japan's PMDA completed review of the clinical-trial notification for an exploratory phase IIa study in congenital tooth agenesis. This is permission-related trial progress, not marketing approval or demonstrated human tooth regrowth. The next important events are verified enrollment/dosing and controlled efficacy data, followed by any study specifically addressing acquired adult tooth loss. A U.S. approval or availability date was not identified. Dated sponsor announcement.
September 14, 2026 — Orthodontic registry gaps resolved
| Study | Primary-record status and date | Interpretation and next check |
|---|---|---|
| NCT06685029 — printed/conventional aligners and fixed appliances | Completed, 52 enrolled; primary completion December 12, 2024 and final completion February 18, 2025. Last posted September 25, 2025. | No posted results. Await material-release, full-treatment-time and final PAR bite-quality findings; completion does not establish safety or faster treatment. |
| NCT07496892 — printed versus thermoformed aligners | Not yet recruiting, 28 planned; last posted March 27, 2026. April 1 start and September 1 primary estimates have elapsed; final completion estimated September 20, 2026. | Stale estimates do not prove recruitment or completion. Verify a status refresh before treating the imminent date as a milestone. No posted results. |
| NCT02954133 — OrthoPulse movement/treatment time | Unknown, 167 enrolled; last posted January 15, 2019, with elapsed April 2019 primary-completion estimate. | Status remains stale; no posted results or dependable readout date. |
| NCT05361616 — OrthoPulse pain | Completed, 32 enrolled; actual primary completion January 1, 2025. Last posted June 4, 2025. | No posted results. Pain is distinct from overall treatment speed, root safety and long-term retention. |
Toregem's current news index did not establish a newer TRG035 clinical milestone beyond the August 17 trial-notification review. Japan's primary Phase 1 registry remained inaccessible, so secondary claims about recruitment completion were not promoted. No human tooth-regrowth efficacy or U.S. availability was verified.
Expected updates
| Program or question | Next meaningful milestone | Timing confidence |
|---|---|---|

Dated findings
2026-09-12 — Initial baseline and scope expansion. Added orthodontic technology, treatment efficiency, retention, enamel/periodontal repair and tooth regeneration. Recent anchors are the August 2026 tray-schedule trial, January 2026 enamel-prevention trial and August 2026 TRG035 trial milestone; older controlled studies provide context. No comprehensive market survey or personal treatment comparison was performed. Several registry/API pages were inaccessible; their status and timing gaps are explicitly retained. Informal-source searches found mainly promotional alignment-speed claims without an adequately specific new program signal, so none was promoted into an evidence finding. Future relevant rumors remain eligible under the notebook's rumor policy.
September 14, 2026 — Focused research refresh
Retrieved four primary orthodontic registry records and replaced the earlier access gaps with verified completed/unknown/not-yet-recruiting statuses, preserving absent results and stale estimates. Rechecked Toregem's news; no later clinical event was verified. Discovery searches covered aligners, retention, enamel/periodontal repair and tooth regeneration. In-vitro enamel reports were excluded; several newly found restorative and older retainer papers were not fully appraised. This was a focused check with remaining gaps, not evidence of no news across all dental research.
2026-09-18 — Scheduled focused review
Searched alignment, retention, root/gum health, extraction healing, enamel repair and tooth regeneration. Appraised new REFIX enamel-defect pilot and A-PRF/CGF extraction RCT. Reviewed Sep 9 retrospective clear-aligner movement-predictability paper as a lower-priority lead. Refreshed direct-printed-aligner registry; checked Toregem news for first dosing/results. Original event dates, safety and limitations are retained in the September 18 update above. The complete-topic successful-check date was not advanced because the listed gaps remain. Run record.
