Summary updated Topic baseline checked Review coverage

A summary update checks selected findings. It does not mean every program, safety question or access detail was reviewed again. Source and event dates remain attached to their findings below.

Selected current fields reviewed October 9, 2026. Stronger prior evidence and unchanged programs retain their original sources. This does not advance the full-topic successful-check date; the research log states remaining gaps.

Topic baseline checked coverage: Selective baseline; SP0268 live registry/API inaccessible; current sponsor recruitment readable but completion date unresolved. Selective baseline; no appraised acne/rosacea rumor entry.

Read the dated research notes

Acne and rosacea

The matched U.S. AURORA denifanstat protocol is now public and recruiting, moving beyond the earlier unmatched trial plan; actual first dosing remains unverified.

Where things stand

Established uses

FDA’s May Rx-to-OTC switch covers Differin Epiduo Acne Gel for ages 12 and older: adapalene 0.1% plus benzoyl peroxide 2.5%. This is an access update, not a new mechanism.

In development

The matched U.S. AURORA denifanstat protocol is now public and recruiting, moving beyond the earlier unmatched trial plan; actual first dosing remains unverified.

Key finding

Two 16-week trials reported rosacea treatment success in 65% and 60% with Emrosi, versus 46% and 31% with doxycycline.

Main uncertainty

The acne vaccine has no verified efficacy readout in this baseline; recruitment does not establish durable remission or an acceptable benefit-risk balance.

What comes next

Actual first enrollment/dosing, unverified; August 2027 primary and April 2028 final completion are registry estimates.

Selected findings from the dated research notes; not an exhaustive treatment guide.

No additional finding was selected from these checked sources. Coverage remains incomplete; this is not a full-topic “no news” conclusion.

Remaining gaps: Read canonical README, HOW_THIS_WORKS, WATCHLIST, catalog, state, LATEST, MILESTONES, LANDSCAPE and assigned current topic. September 5–October 13 window preserves seven-day overlap from September 12 full successful check. Retrieved every page of bounded public registry discovery (53 returned records; incidental matches excluded), and all named records. Full successful-check date must remain unchanged.; Europe PMC retrieval now succeeded, closing prior HTTP503 access failure. Initial broad query downloaded up to 100 per topic with many incidental/nonhuman matches; title/abstract MED retry retrieved hair 97/97, skin 27/27, scars 25/25, dental 100/147. Retrieved records are not an exhaustive worldwide full-paper/safety appraisal.; AURORA recruiting 800 and B244 not-yet-recruiting 176 remain unchanged; October 9 site-activation announcement and September 30 Chinese denifanstat extension deduplicated. SP0268 vaccine and remibrutinib named records refreshed without new outcomes. October 11 interview reprises extension rather than a new trial event. Rosacea vessel/flushing/ocular, antibiotic resistance, U.S. label/device and complete safety census remain gaps.

Informal-source coverage: October 7 original London/Markham rosacea-recruitment Reddit post gives no uniquely verified drug identity; saved remibrutinib study remains recruiting separately. Targeted denifanstat/B244/new trial rumor queries found no distinct selected claim. Full sponsor KOL/Q&A replay and podcasts remain gaps.

AURORA registry now lists recruitment; sponsor reports sites activated

Source dates: October 7, 2026 first registry posting; October 9 sponsor update. Evidence: primary registration plus sponsor development statement.

The previously unmatched AURORA / NCT07863193, SB2640-CLIN-017 now lists recruiting. Sagimet's October 9 release reports activated sites and IRB approvals and says patient screening is beginning this month; accessible primary indexed text supplies that claim, while the full release returned 403. First participant enrollment/dosing is not independently established. The registry still labels the October 2026 start estimated.

The randomized double-masked U.S. Phase 3 protocol plans 800 participants aged 12+ receiving once-daily oral denifanstat or placebo for twelve weeks. Co-primary endpoints are clear/almost-clear EGSS with at least two-grade improvement and absolute inflammatory/noninflammatory lesion-count changes. About 530 may enter a further 40-week active open-label safety extension. No U.S. efficacy or safety results are posted; the already saved Chinese 52-week extension is separate and not re-alerted as fresh evidence. No comparison with isotretinoin or off-treatment remission benefit follows from registration.

U.S. status and next: Investigational. August 2027 primary and April 2028 final completion are registry estimates. Verify actual first dosing and assess the controlled twelve-week result when released; no exact publication day is announced.

Original AURORA registry; October 9 sponsor announcement.

B244 microbiome spray adds a registered Phase 2b acne study

Source date: October 8, 2026 first registry posting.

AOBiome's B244 / AVB244-006, NCT07866105 is a newly registered Phase 2b randomized, masked vehicle-controlled study in 176 planned participants aged 12+ with moderate-to-severe acne. The spray is applied twice daily for twelve weeks, with a four-week off-treatment observation. The primary endpoint is clear/almost-clear IGA with a two-grade improvement; lesion counts, quality of life and local/systemic safety are assessed separately. The sponsor's earlier July plan is historical context, not a new October clinical result. Its nitric-oxide-producing bacterial platform is a proposed mechanism, not proof of efficacy.

Status and limits: Not yet recruiting; October 2026 start and August/September 2027 primary/final completion are estimates. No controlled outcome or harms are posted. A short off-treatment period cannot establish durable acne remission, scar prevention or antibiotic-resistance benefit. The separate atopic-dermatitis Phase 3 plan is not this acne trial's phase. No U.S. acne marketing approval. Next: actual initiation, controlled outcome and full tolerability/safety; readout date unannounced.

Original registry; historical July 1 sponsor plan.

Remaining gaps: Read the current assigned topic and canonical scope/state. Named original registry records refreshed; all pages of broad ClinicalTrials.gov discovery for September 5–October 9 were retrieved (52 records, with incidental matches excluded). This honors the seven-day overlap from the saved September 12 full successful check.; Europe PMC original-literature discovery again returned HTTP 503. Targeted primary publisher, sponsor and conference searches were performed, but the worldwide paper/regulator/procedure and original-audio census remains incomplete. Preserve last_successful_check_utc.; SP0268 vaccine, remibrutinib PPR and redness-study records refreshed; no new posted outcomes. Denifanstat Chinese extension already reported September 30 and not treated as a new Oct9 efficacy result.; New PRX S observational cosmetic record NCT07866131 and retrospective exosome-acne-scar disclosure noted without efficacy inference; scar repair belongs to scars. Full rosacea ocular/flushing/vessel and antibiotic-resistance/label/device census incomplete. Sagimet full October9 release blocked403, indexed primary clinical-development wording checked against complete registry.

Informal-source coverage: Targeted October6–9 Reddit and original talk/podcast queries did not recover a noteworthy new AURORA/B244/SP0268 rumor. Sagimet Fall Clinical/replay descriptions are sponsor context, not full Q&A verification. Earlier UK Finacea shortage anecdote was not generalized to U.S. availability; no manufacturer confirmation. Full recorded discussions incomplete.

Recovered baseline: remibrutinib is being tested for papulopustular rosacea

Original source date: July 2, 2026 first posting; current record posted September 28. Baseline addition; not October breaking news.

NCT07681271/CLOU064Q12201 adds a formal oral-drug mechanism to the rosacea watch: a randomized, double-masked placebo-controlled Phase 2 study of remibrutinib in 100 planned adults with moderate-to-severe papulopustular rosacea. It is recruiting and reports July 30, 2026 as the actual start. The primary endpoint is change in facial inflammatory-lesion count at week 16; the secondary modified IGA excludes erythema. Thus this study will not establish treatment of persistent redness, flushing or ocular rosacea. No results are posted. Health Canada's matching record reports May 20 trial authorization, separately from marketing approval. No U.S. rosacea indication is established by these records. Next: primary/final completion December 7, 2027 are registry estimates, not a release promise.

Original registry; Health Canada matching protocol.

Remaining gaps: AURORA October screening remains sponsor forecast; current denifanstat+acne registry query yields only two Chinese completed studies, no matched U.S. AURORA study. Sept30 extension already captured; Oct1 financing is not clinical efficacy/initiation. Existing RF acne/rosacea redness records unchanged; SP0268 full current API not refreshed in this batch.; Remibrutinib PPR baseline recovered with original registry and HealthCanada protocol. Denifanstat acne extension reported; U.S. phase3 screening forecast for October

Source/event date: 2026-09-30. Evidence: Sponsor-only open-label extension; prospective phase3 plan.

Sagimet reported ASC40-304 (NCT06248008), a 240-participant Chinese extension following a 480-person randomized acne trial. At total study week 52, 56.7% met the composite of clear/almost-clear skin and at least a two-grade IGA improvement; total lesions decreased 71.8% from original baseline. All received active 50 mg daily during the 40-week extension: prior active recipients had 52 weeks of exposure, prior placebo recipients only 40. Rollover selection and the absence of a concurrent placebo limit causal interpretation and comparison with isotretinoin. The sponsor reported no drug-related serious adverse events or adverse-event-related permanent discontinuations; dry skin occurred in 7.1% and dry eye 5.9%. Full comparative harms and confidence intervals were not supplied.

The planned U.S. AURORA phase 3 program targets about 800 participants, including 450 adolescents, with October 2026 screening followed by enrollment. That is a forecast, not verified recruitment. Denifanstat remains investigational in the U.S. September 30 sponsor release.

Next milestone: AURORA screening October 2026 and first enrollment shortly afterward, sponsor forecast; matched U.S. registry not verified.

Clascoterone rosacea pilot adds early indication-specific evidence

Source date: 2026-08-01. Evidence: peer-reviewed single-arm phase 2 pilot; recovered baseline. Baseline addition; not new news.

An older 20-person study explored clascoterone 1% cream for papulopustular rosacea; this is separate from acne approval and hair-loss trials. Eighteen completed 12 weeks. Mean biopsy gland count decreased from 4.94 to 3.61 (p=.012), while gland-diameter change was not significant (p=.182). Quality-of-life scores improved from 7.5 to 3.3 (p=.029).

Limitations: No comparator, small sample and mainly histological/patient-reported outcomes. This does not establish a causal clearance effect. Related registry NCT06952517 remains stale and lists gland size as its primary endpoint; reconcile the complete paper and registration before presenting this as a successful primary-endpoint trial.

Safety: No serious events reported; one mild transient dryness and one eye irritation. Short follow-up cannot establish long-term safety.

U.S. status: No rosacea indication established; approved acne use and investigational hair-loss solution are distinct.

Next milestone: Controlled clinical trials and registry reconciliation remain undated.

Primary published abstract; Related rosacea study registry.

Review coverage and remaining gaps

Coverage: Selective September 5–29 overlap, emphasizing September 27–29; prior September 27 findings deduplicated. Refreshed SP0268 and redness-study records; appraised older clascoterone rosacea abstract and related registry; read September 21 single-case RF report.

Remaining gaps: Clascoterone related registry remains stale and primary-endpoint interpretation needs full-paper reconciliation. SP0268 still recruiting without results. RF report is one off-label case, not meaningful comparative evidence. EuropePMC API 503 limited discovery.

Informal-source coverage: Recent informal rosacea/hair episode and targeted searches did not establish a new trial-specific rumor; case-report author discloses device consulting, and protocol is not validated.

New-to-watch radiofrequency-versus-topical acne trial

Source date: 2026-09-04. Event date: 2026-09-04. Evidence: trial-registry record; recovered baseline. Baseline addition; not newly initiated research.

A registered randomized study will compare a sequential radiofrequency procedure with adapalene/benzoyl peroxide for inflammatory facial acne. No results are available. NCT07805655, Jeisys Medical: 60 planned adults with mild-to-moderate acne, 1:1 open-label comparison, one Mexican site listed recruiting. The September 4 record lists recruiting and an estimated September 15 start. The primary outcome is inflammatory-lesion count change; secondary outcomes include redness, quality of life, satisfaction and harms.

Limitations: The record has timing inconsistencies: December 25/31, 2026 completion estimates coexist with outcomes through week 24. An elapsed estimated start does not establish actual first treatment. Registration is older missing context, not a September 27 trial launch or efficacy finding.

Safety: Procedural pain, local skin reactions and treatment-emergent adverse events are planned assessments, not safety results.

U.S. status: No new U.S. authorization for this specific sequential acne protocol verified.

Next milestone: Seek an updated timetable and actual-start confirmation before using the December estimates as actionable result timing.

Primary randomized acne study record.

Review coverage and remaining gaps

Coverage: Searched new acne/rosacea therapies, vaccines, devices and comparative evidence; refreshed SP0268 and redness-study registries and retrieved new-to-watch RF comparison. Noninterventional biomarker/diet association papers not treated as treatment results.

Remaining gaps: SP0268 remains recruiting, estimated November 2028 completion, no results. Redness trial NCT07521462 unchanged. RF study has internal timing conflicts. Neurogenic-rosacea systematic review lacks accessible numerical abstract; full evidence appraisal pending. LUT014 EADV acneiform-rash lead concerns EGFR-cancer-treatment toxicity, not ordinary acne, and was not conflated with this topic.

Informal-source coverage: September 23 Well episode with Ginni Mansberg and Cath Mahoney discusses rosacea triggers and hair supplements; accessible show notes contain no specific trial milestone. Focused update — 2026-09-22

No additional finding was verified in the reviewed sources. Coverage remains partial; this is not a full-topic “no news” conclusion.

Coverage: Searched original September 5–22 acne/rosacea papers, investigational vaccine, topical drugs, devices, safety and sponsor developments. Refreshed SP0268 and rosacea redness-study records; their recruiting statuses/dates remain unchanged. No new material efficacy or regulatory result was verified within this focused check.

Remaining gaps: September 17 topical-ruxolitinib rosacea case series and September 11 spruce-extract exploratory report were located; full methods/safety not appraised and no indication-level efficacy conclusion made. An isotretinoin-study correction was indexed September 5; its material effect on results not resolved in this pass. No complete new worldwide device-clearance review or SP0268 investigator update beyond the registry.

Informal-source coverage: Targeted Reddit, podcast and talk queries. September 17 Dr Mitch Shulman radio-podcast segment is general acne-versus-rosacea discussion; episode notes lack a trial claim and no audio transcript was reviewed. No new attributable SP0268 rumor selected.

Focused update — 2026-09-18

No additional finding was verified in this focused search. The incomplete coverage below does not establish that nothing changed across the full topic.

Coverage: Searched human acne/rosacea drug, laser/device and regulatory developments; refreshed SP0268 original registry. No material new in-window result verified. Existing Sep 3 rosacea-device actual start and older OTC-switch news were not repeated. SGT-610 Sept 17 oncology milestone was handed to root for cancer treatment, not relabeled as acne. This is a focused review, not a complete re-review of every program.

Remaining gaps: SP0268 registry remains recruiting with estimated November 30, 2028 completion and no results. Japanese ACCLAIM/AviClear registration was disclosed August 1, outside overlap; not a newly verified September start. No full transcript of the Sep 1 rosacea webinar was retrieved; its linked moisturizer study was published May 20, not a new September trial report.

Informal-source coverage: Targeted Reddit, podcast and webinar searches found general routines and previously circulated research digests, without a traceable new SP0268 or other formal-program rumor. No podcast quotation or implied readout date added.

Full coverage record.

Next expected update: SP0268 first efficacy results remain undated; its registry estimates final completion November 30, 2028. A newly tracked rosacea redness study estimates completion June 30, 2027. These are registry estimates, not promised publication dates.

Initial brief — evidence available through 2026-09-12

Current assessment. The acne vaccine is a distinctive early human program; rosacea already has a newer approved oral option with randomized evidence. Pipeline lists can remain stale after sponsors change direction, so later announcements and filings matter. A cosmetic launch following a drug trial is a different regulatory event from approval of that investigated drug.

Sanofi SP0268 — therapeutic acne mRNA vaccine. The sponsor's September 2026 page lists NCT06316297 as recruiting Phase 1/2, with a planned 800 adults aged 18–45. The dose-finding study evaluates safety, efficacy and immune responses with a core study and optional extended follow-up; some cohorts are invited to 30 additional months. This is treatment research in existing moderate-to-severe acne, not demonstrated prevention in healthy adolescents. No efficacy readout was verified. The July 30, 2026 pipeline still lists SP0268. Enrollment does not establish response, durable remission or an acceptable benefit–risk balance. U.S.: investigational. The sponsor page supplies no dependable public-results date. Recheck the registry and any interim presentation before repeating older completion forecasts.

Emrosi — extended-release minocycline for inflammatory rosacea. Its November 2024 FDA label covers inflammatory lesions in adults. Two 16-week randomized trials, MVOR-1/2 (NCT05296629, NCT05343455), enrolled 653 participants. Investigator treatment success was 65%/60% with Emrosi, 46%/31% with doxycycline and 31%/27% with placebo. This supports papulopustular benefit, with limited duration and a predominantly White population. It does not demonstrate permanent remission or treatment of every redness phenotype. Minocycline warnings include serious hypersensitivity, hepatic/autoimmune effects, intracranial hypertension and pigmentation. FDA label and trial table. The sponsor reports commercial promotion beginning April 2025; follow longer-term comparative evidence and material safety changes rather than repeating approval news. 2025 annual filing.

DMT310 / XYNGARI — development-direction correction. Dermata reported positive Phase 3 STAR-1 acne results in 2025, NCT06090721. However, its 2026 annual filing explains a September 2025 strategic shift from prescription-drug development toward cosmetic and OTC skincare. Do not assume an imminent prescription filing or treat a reformulated consumer product as the same tested drug. Sponsor annual report. Separately, the earlier rosacea trial failed its primary endpoints; the sponsor stopped that indication, citing tolerability concerns. This is particularly relevant when databases still list an active rosacea program. Primary sponsor account. No new prescription-development milestone was verified.

September 14, 2026 — Focused update

SP0268 registry timing resolved. The primary registry, last posted August 21, 2026, lists recruiting, 800 planned participants and primary/final completion estimated November 30, 2028. There are no posted results. This supersedes the earlier unresolved registry timing, not the statement that efficacy remains unproven; the long study estimate does not set the date of any interim readout.

Differin Epiduo — older U.S. access update. FDA's Rx-to-OTC switch list records May 22, 2026 approval of Differin Epiduo Acne Gel (adapalene 0.1%/benzoyl peroxide 2.5%, NDA 220736) for nonprescription acne treatment in adults and children 12 and older. This is an access/formulation baseline addition, not a new mechanism or comparative efficacy result. Retail launch was not independently verified.

Newly tracked redness study. NCT07521462, updated September 8, reports an actual September 3, 2026 start and recruiting status. ISISPHARMA plans 50 participants in a blinded split-face dermocosmetic-versus-placebo comparison, alone and then alongside vascular laser treatment. It assesses persistent redness and visible vessels over eight months; it does not establish acne clearance or benefit for every rosacea phenotype. Completion is estimated June 30, 2027, with no results posted. Exact active formulation and U.S. authorization were not established.

When to expect the next useful update

Program or question Timing and evidence status What would matter
SP0268 vaccine Recruiting; primary/final completion November 30, 2028 (registry estimate). No firm public readout date. First efficacy/safety results, dose selection and actual pivotal-trial initiation.
Dermocosmetic plus/minus laser for rosacea redness, NCT07521462 Recruiting; completion June 30, 2027 (registry estimate). No efficacy result yet. Redness/vessel assessments, local tolerability and durability; keep these separate from papulopustular rosacea.
Emrosi No firm next trial or label milestone verified. Already U.S.-approved. Longer follow-up, comparative effectiveness, recurrence and clinically meaningful safety updates.
DMT310/XYNGARI No verified prescription filing/readout forecast. Development strategy changed. A funded new clinical program, full data, or regulatory action; product marketing alone is not a new efficacy result.
New drugs, devices and procedures No category-wide calendar. Human comparative trials, persistent benefit and harms, including studies in darker skin tones.
What we’re watching
  • Follow acne vaccines, new anti-inflammatory and microbiome approaches, improved topical combinations, isotretinoin alternatives, and formal trials of lasers, light and other procedures.
  • Follow papules/pustules, persistent redness, flushing, visible vessels and ocular rosacea separately. Improvement in one feature does not establish efficacy for every form of rosacea.
  • Prioritize visible clearance, sustained remission after stopping, recurrence, prevention of scarring, quality of life and treatment burden. Cross-link scar repair to its own category.
  • Record comparator, lesion counts and investigator/patient assessments; distinguish percentages improving from percentages of lesions eliminated. Include irritation, pigment changes, antimicrobial resistance, systemic harms, downtime and skin-tone representation.

Coverage and informal signals

This was a selective baseline, not an exhaustive pipeline review. The live ClinicalTrials.gov page/API for SP0268 did not yield usable details; the current sponsor page was readable, and registry timing is an explicit follow-up. Searchable older third-party completion dates conflict and were not promoted into the timetable. No specific acne/rosacea rumor was sufficiently appraised for an entry in this baseline; subsequent checks should include informal sources under the cover-page policy.

Dated findings

September 14, 2026 — Focused research refresh

Resolved SP0268's previously inaccessible registry timing and added the September 8 rosacea recruitment update. Added FDA's May 22 Differin Epiduo OTC switch as older baseline context. DMT310's earlier prescription-development pivot remains recorded; no new efficacy or safety conclusion was inferred from marketing or recruitment. Recent formal and informal searches were selective and did not establish a new specific rumor.
