Autoimmune disease and immune reset

Immune-reset research includes a newly registered in-vivo CD19 CAR-T study, without confirmed dosing or results. Two distinct Chinese CAR-T protocols are withdrawn with zero participants. A completed rap-cel lupus record does not establish lifting of the wider autoimmune-program hold; remission, durable benefit and safety remain separate questions.

Where things stand

Established uses

The autoimmune CAR-T uses featured here remain investigational in the U.S.; an immune-cell response alone does not establish a cure.

In development

Immune-reset research includes a newly registered in-vivo CD19 CAR-T study, without confirmed dosing or results. Two distinct Chinese CAR-T protocols are withdrawn with zero participants. A completed rap-cel lupus record does not establish lifting of the wider autoimmune-program hold; remission, durable benefit and safety remain separate questions.

Key finding

A 15-patient case series reported lupus remission or disease improvement after CAR-T, with immunosuppression withdrawn and median follow-up of 15 months.

Main uncertainty

Prula-cel’s one-year denominators are small: 7/13 met lupus-remission criteria and 5/10 had a complete renal response. Infections occurred in 13/24 of the safety cohort, including two grade ≥3. Lymphodepletion was required; remission is not cure.

What comes next

BMS registers an in-vivo CD19 CAR-T study across autoimmune diseases

First posted October 14 — NCT07873931. Bristol Myers Squibb registers a 60-person, open-label, single-group Phase 1 study of in-vivo CD19 CAR-T therapy BMS-986553 for refractory lupus/lupus nephritis, inflammatory myopathies, systemic sclerosis, rheumatoid arthritis and Sjögren disease. Primary assessments include dose-limiting toxicity over 28 days and adverse events, serious events and laboratory abnormalities through approximately 90 days.

Status and U.S.: Not yet recruiting; no efficacy or safety results and no approval for these uses. The record does not disclose enough delivery or conditioning detail to call it mRNA-based or chemotherapy-free. This separate protocol does not establish restart of paused ex-vivo CAR-T programs.

Timing: October 30, 2026 start, October 20, 2032 primary completion and January 31, 2033 final completion are registry estimates, not promised readout dates.

Sources: ClinicalTrials.gov · NCT07873931.

Two autoimmune CAR-T protocols are withdrawn with no participants

October 15 registry updates. NCT07448298 (AFN50, early Phase 1, in-vivo CAR generation using T-cell-targeted lipid nanoparticles containing CAR-encoding RNA) and NCT07361094 (autologous CD19/BCMA dual-target CAR-T, Phase 1, manufactured ex vivo) are both withdrawn, each with zero actual participants and the stated reason “No participants.”

These are different constructs. Neither record supplies a human efficacy result, a toxicity failure or a reason to discontinue the broader platform. Date fields still include an 'actual' start despite zero enrollment; that inconsistency is not evidence of dosing. No U.S. product approval follows from these registrations.

Rap-cel lupus record changes to completed without posted results

October 15 registry update — NCT05798117. The uncontrolled Phase 1/2 lupus study of YTB323/rapcabtagene autoleucel now lists completed, 21 actual participants, and actual primary/final completion September 24, 2026. No results are posted.

Completion of this record does not establish resumption of recruitment, clearance of the previously reported autoimmune-program hold, remission, cure or a favorable safety profile. The wider hold remains unresolved in the sources checked. U.S. approval for lupus is not established.

Sources: ClinicalTrials.gov · NCT05798117.

Remaining gaps: All saved named registry records and every page of bounded September 5–October 16 CT.gov discovery were retrieved; 35 discovery records were screened selectively. Retrieval is not full appraisal. Worldwide registries, all publications/regulatory files, complete harms and original audio/transcripts remain incomplete. Prior full successful-check dates are preserved.

Informal-source coverage: Targeted Reddit, interview, podcast and saved unresolved-claim searches were appraised selectively; original recordings and worldwide informal coverage remain incomplete. Routine anecdotes and reposts were not treated as new trial evidence. Saved Mount Sinai IBD CAR-T planning remains unconfirmed; the new BMS protocol does not confirm that institutional claim.

C-CAR168 plans a dual-target CAR-T trial for progressive multiple sclerosis

First posted September 17; updated October 13, 2026; newly appraised October 13. AbelZeta’s NCT07825389 plans 119 participants with refractory primary or secondary progressive multiple sclerosis, phase 1b/2, single-arm open-label. Autologous C-CAR168 targets CD20 and BCMA after fludarabine/cyclophosphamide lymphodepletion. Primary assessments include serious/toxic events, CRS and ICANS through month 24, plus six-month confirmed composite disability progression through month 12 in phase 2.

Status/limits/U.S.: Not yet recruiting; no clinical or safety results. Without randomized standard care, disability changes require caution. This is not a verified restart of other companies’ paused CAR-T programs or a demonstrated MS cure. Investigational.

Timing: February 2027 estimated start and February 2030 primary/final completion, month precision. The separate C-CAR168 lupus-nephritis record still says not yet recruiting after an elapsed September 30 start estimate; no enrollment inferred.

Sources: ClinicalTrials.gov · NCT07825389.

Remaining gaps: All saved named registry records retrieved and all pages of bounded September5/6–October13 CT.gov discovery downloaded; 29 records screened selectively. Retrieval is not full appraisal. Worldwide registries, all publications/regulatory files, comprehensive harms and full audio/transcripts remain incomplete; prior full successful-check dates preserved. C-CAR168 lupus record remains not yet recruiting after elapsed start; no verified rap-cel/zola-cel restart, full FB102 effect/harms or Mount Sinai IBD initiation.

Informal-source coverage: Revisited original lupus Reddit thread, which repeats prula-cel release and anonymous cross-trial safety predictions. These are not proof of safer platforms. Mehandru IBD planning remains unconfirmed without matched initiation; review literature is not confirmation of the specific Mount Sinai plan.

FB102 meets a celiac gluten-challenge histology endpoint; clinical detail remains limited

Source date: October 8, 2026. Evidence: sponsor-only topline. FB102-301 (NCT06982963) enrolled 126 symptom-free adults on a strict gluten-free diet for at least twelve months. Randomization was 2:2:1 to two intravenous anti-CD122 doses or placebo during an eight-week gluten challenge. Argenx reports a significant day-78 villus-height/crypt-depth response (p=.0176) and supportive inflammatory/symptom measures, without absolute effect estimates, intervals, dose-specific results or numerical safety tables.

Limits/registry discrepancy: The last registry update, November 18, 2025, still lists 100 estimated participants and the Vh:Cd–IEL composite (VCIEL) as primary, while the release calls Vh:Cd primary. The stale record cannot reconcile endpoint designation or replace the reported 126 with 100. Full protocol/statistical plan is needed. A short controlled challenge does not establish unrestricted dietary tolerance, long-term organ protection or cure.

Safety/U.S./next: “No new safety signals” is a qualitative sponsor statement, not quantified risk. FB102 remains investigational; Fast Track is not approval. Phase 3 is planned. Detailed results are promised at an unspecified meeting, with no firm next date.

Litifilimab adds one-year cutaneous-lupus follow-up after placebo crossover

Source date: October 2, 2026; newly appraised October 9. AMETHYST Part A (NCT05531565) initially evaluated 93 adults with active cutaneous lupus despite or intolerant of antimalarials. In the originally active-treated group, clear/almost-clear skin by the CLA-IGA erythema endpoint rose from 19.0% at week 24 to 27.2% at week 52, and CLASI-70 from 21.7% to 28.8%. The placebo-crossover group reached 33.7% clear/almost-clear skin at week 52.

Design/safety limits: After week 24 all participants received active drug, so these are longitudinal/crossover observations rather than a contemporaneous placebo comparison. Arm-specific evaluable denominators and intervals were not supplied. The extension safety denominator is 88; 3/88 (3.4%) had serious adverse events. Common events included nasopharyngitis, influenza and joint pain. This does not demonstrate cure, reversal of established scars or prevention of systemic disease.

U.S./next: Investigational; Breakthrough Therapy designation is not approval. The randomized phase 3 part remains separate, with results forecast H1 2027. The October 9 registry lists active-not-recruiting, estimated primary completion October 20, 2026; completion is not publication.

October 2 sponsor report; primary registry.

Remaining gaps: All saved named NCT records refreshed and all pages of official overlap discovery screened (28 returned records). Search starts September 5/6 from the last full successful check, not October 6 alone. Incidental matches excluded; literature, international registries, all regulatory files and complete podcast/audio appraisal remain incomplete.; No matched rap-cel/zola-cel restart announcement verified; pre-pause or unchanged recruiting labels do not prove restart. Full CAR-T longer-term safety, ACR abstract census, FB102 protocol and litifilimab arm-level tables remain gaps.

Informal-source coverage: Read original OncLive September22 discussion of Mount Sinai IBD CAR-T planning; no matched newly initiated study verified, existing stable claim remains unconfirmed. Read lupus/scleroderma Reddit anecdotes and discussions; unverified identity, selected responders and unsupported safety rankings mean no comparative or cure result. Full podcast recordings unreviewed.

South Korean childhood-onset lupus CAR-T study enters the registry

Source date: First posted October 2, 2026; registry-reported actual start September 14. Evidence: NCT07855302, recruiting phase 1 at Seoul National University Hospital.

Eight planned participants up to age 25 with refractory childhood-onset systemic lupus receive a CD19-targeted autologous CAR-T infusion, preceded by protocol-defined lymphodepletion. The primary outcome is lupus low disease activity at one year; renal response is secondary.

Limits/safety: Single-arm early research, no posted outcomes or safety rates. Low disease activity is not cure; lymphodepletion and CAR-T risks require direct follow-up. This does not establish benefit for other autoimmune diseases.

U.S./next milestone: No U.S. authorization established for this construct. Estimated primary/final completion December 2028; public readout date unknown.

Primary registry.

Review coverage and remaining gaps

Coverage: Official registry overlap discovery

Remaining gaps: Named registry refresh and overlap discovery completed; comprehensive literature, all regulatory files and informal transcripts remain incomplete. Full-topic successful-check date is preserved. Official registry discovery returned 28 records, from all returned pages; appraisal focused on named programs and new first postings. Incidental matches were excluded. No new restart announcement verified for paused rapcabtagene/zolecabtagene; unchanged/pre-pause registry records cannot prove restart. C-CAR168 remains not yet recruiting. Complete long-term CAR-T safety and transcript review incomplete.

Informal-source coverage: Focused Reddit and named-program searches; anecdotes/reposts were not treated as trial evidence. Broader podcast/talk coverage remains incomplete.

Miv-cel sponsor reports one-year durability in small SPS and myasthenia cohorts

Source/event date: 2026-09-24. Evidence: Sponsor-only uncontrolled clinical follow-up; recovered September event.

Kyverna reported single-arm CD19 CAR-T follow-up in refractory stiff-person syndrome (KYSA-8, NCT06588491; 26 adults) and generalized myasthenia gravis (KYSA-6, NCT06193889; seven patients). In SPS, median timed-25-foot-walk improvement was 49% at 12 months; 92% were off chronic SPS immunotherapies. Only five myasthenia patients had at least one-year follow-up; all seven had week-24 mean MG-ADL and QMG improvements of 8.3 and 11.7 points, and six were off immunosuppression at last follow-up. No high-grade CRS, ICANS or IEC-HS was reported. Selected uncontrolled cohorts do not prove comparative benefit, cure, or rare/long-term safety; lymphodepletion remains part of treatment. Miv-cel is investigational. Sponsor forecasts full SPS results at MS Toronto October 21–23 and rolling SPS BLA completion in Q4 2026. Sponsor.

Next milestone: October 21–23, 2026 — sponsor-announced presentation window for full SPS results. Rolling BLA completion is forecast for Q4 2026.

FDA accepts fenebrutinib application with Priority Review for relapsing and primary progressive MS

Source/event date: 2026-09-29. Evidence: Sponsor-reported regulatory filing acceptance.

Genentech reports FDA acceptance and granted Priority Review for oral fenebrutinib in relapsing and primary progressive MS. This is review, not approval. FENhance 1 and 2 reduced annualized relapses versus teriflunomide by 51.1% and 58.5% over 96 weeks. FENtrepid met noninferiority to ocrelizumab for 12-week confirmed composite disability progression (HR 0.88, 95% CI 0.75–1.03); numerical benefit does not establish superiority. Serious adverse events were 9% versus 9% and 11% versus 6% in relapsing trials, and 19% in both progressive-disease arms. Liver-enzyme elevations were more frequent than with ocrelizumab. The sponsor acknowledges a fatality imbalance; detailed attribution needs full evidence review. No FDA action date appears in the announcement. Fenebrutinib remains investigational. Sponsor.

Next milestone: FDA decision after granted Priority Review; action date undisclosed.

Prula-cel reports one-year lupus remission signals, with infections in half the safety cohort

Source date: 2026-09-28. Evidence: Sponsor phase 1 uncontrolled human results.

Adicet reports one-year remissions after donor-derived anti-CD20 gamma-delta CAR-T; the small uncontrolled dataset does not establish cure or comparative safety. August 28 cutoff: 22 efficacy-evaluable participants (16 lupus nephritis, 6 extra-renal SLE), followed 6–21 months. At 12 months, DORIS remission was 7/13 (54%) and complete renal response 5/10 (50%). Participants received cyclophosphamide/fludarabine lymphodepletion. All discontinued immunosuppressants; all but one tapered steroids to ≤5 mg prednisone equivalent. NCT06375993.

Limitations: Changing follow-up denominators; no randomized comparator or confidence intervals. Biomarker reconstitution is not proof of permanent immune reset. Continued low-dose steroids mean this is not uniformly medication-free. Registry remains a March snapshot; estimated enrollment 180 is not the analyzed cohort.

Safety: Among 24 safety-evaluable participants, infections occurred in 13 (54%), grade ≥3 in 2 (8.3%); grade 1/2 CRS in 25%. No observed higher-grade CRS, ICANS, IEC-HS or graft-versus-host disease; rare/late risks remain uncertain.

U.S. status: Investigational; FDA alignment on outpatient study dosing is not marketing approval.

Next milestone: Sponsor forecasts pivotal lupus-nephritis study start-up in Q4 2026; systemic-sclerosis data H1 2027 and LN/SLE update mid-2027. Slides forecast pivotal interim data in 2028 and readout in 2029. Start-up is not enrollment; potential nonrenal expansion needs regulatory alignment.

OCTOPUS stops metformin arm in progressive MS for insufficient promise

Source date: 2026-09-17. Event date: 2026-09-15. Evidence: Investigator interim adaptive-trial decision.

The metformin arm stopped September 15; alpha-lipoic acid passed the interim checkpoint and continues to the next stage. The planned interim assessment combined MRI and clinical measures. The investigator team announced the decision September 17; a definitive phase 3 answer for the continuing treatment still requires follow-up.

Limitations: This is progressive-MS evidence, not a healthy-aging or lifespan trial. Numerical comparative effects were not reported in the reviewed summary. Passing the checkpoint does not establish alpha-lipoic acid clinical benefit.

Safety: The stated stopping reason was insufficient benefit; no new quantified safety result was provided.

U.S. status: No new MS or longevity approval.

Next milestone: Ongoing alpha-lipoic acid phase 3 follow-up; no exact result date verified.

IMVT-1402 cutaneous-lupus study misses primary endpoint; indication discontinued

Source date: 2026-09-23. Event date: 2026-09-23. Evidence: Sponsor topline randomized human trial.

Immunovant stopped IMVT-1402 development for cutaneous lupus after its phase 2 proof-of-concept trial missed the primary endpoint. The global randomized, double-blind, placebo-controlled study enrolled 57 patients. Week-12 percent change in CLASI-A did not achieve statistical significance. Other autoimmune programs continue. Trial: NCT06980805.

Limitations: No effect size, confidence interval or p value disclosed. Numerical trends and IgG associations do not overturn the primary miss. The August 28 registry snapshot remains active-not-recruiting and is stale relative to the discontinuation announcement.

Safety: Sponsor described favorable safety without a quantitative adverse-event table.

U.S. status: Investigational; no U.S. approval for this indication.

Next milestone: CLE development discontinued; full result publication date unknown. Other indications retain separate milestones.

ORKA-001 psoriasis responses persist to week 28 after dosing ended at week 4

Source date: 2026-09-23. Event date: 2026-09-23. Evidence: Sponsor phase 2a follow-up.

EVERLAST-A reported sustained skin clearance 24 weeks after the second ORKA-001 dose. Among 63 initially active-treated participants, week-28 PASI100 was 45/63 (71.4%), PASI90 55/63 (87.3%), and IGA 0/1 53/63 (84.1%). Doses were 600 mg at weeks 0 and 4. The trial randomized 84 participants 3:1; placebo crossed over at weeks 16 and 20.

Limitations: There is no contemporaneous untreated comparator after crossover. These sponsor data do not establish annual dosing, cure, or long-term safety.

Safety: Weeks 16–28: upper respiratory infections 7/83 (8%); two serious events, tibial fracture and prostate adenocarcinoma, both judged unrelated. No injection-site reactions reported. Safety and efficacy denominators differ.

U.S. status: Investigational; not U.S.-approved.

Klygefa receives a positive European committee opinion for myasthenia gravis

Source date: 2026-09-18. Event date: 2026-09-17. Evidence: Regulator recommendation; opinion September 17, published September 18.

EMA’s CHMP recommends gefurulimab as add-on therapy for adults with AChR-antibody-positive generalized myasthenia gravis. Weekly subcutaneous C5 blockade improved disease severity and daily function in the 26-week placebo-controlled PREVAIL trial. The regulatory opinion adds a development milestone rather than a new efficacy readout.

Limitations: A CHMP opinion precedes a separate European Commission authorization; it does not establish U.S. approval or immune-reset remission.

Safety: EMA lists injection-site reactions, back/joint/muscle symptoms, nausea and vomiting among common effects. The complete approved EU label was not yet available at this check.

U.S. status: No new U.S. authorization verified in this European action.

Next milestone: European Commission decision pending; no dependable date retrieved.

IVIG add-on improves early inflammatory-myopathy disease activity

Source date: 2026-09-14. Evidence: Peer-reviewed randomized human trial.

TIME IS MUSCLE supports earlier IVIG add-on in a small newly diagnosed inflammatory-myopathy population. The double-blind trial randomized 44 adults, all treated with high-dose prednisone. The reported primary-outcome sample included 42 participants: 23 receiving IVIG and 19 receiving placebo. At week 12, Total Improvement Score was 60.0 (95% CI 52.6–67.4) versus 42.5 (95% CI 30.6–54.4), p = .01. Moderate improvement occurred in 91% versus 53%. Trial identifier: EudraCT2020-001710-37-NL.

Limitations: This was a single-center study with short follow-up and a disease-specific composite endpoint. It does not establish healthy muscle gain or muscle preservation during GLP-1 treatment. The major-response analysis was post hoc. Full supplemental analyses were not reviewed.

Safety: One asymptomatic deep-vein thrombosis occurred in the IVIG group.

U.S. status: No new U.S. label action established by this publication.

Next milestone: Further comparative evidence and longer follow-up are undated.

JAMA Neurology September14 publication.

Review coverage and remaining gaps

Coverage: Rap-cel/zola-cel/RESET original registries plus new human autoimmune and restart searches; old registry status cannot confirm resumption. This is a focused review, not a complete re-review of every program.

Remaining gaps: No authoritative rap-cel/zola-cel restart notice found; other allogeneic/in-vivo programs and full conference transcripts not comprehensively reviewed.

Informal-source coverage: Reddit repeated late-August pauses. Indexed Sept9 Cabaletta conference claims about safer manufacturing remain sponsor interpretation; no verified comparative safety conclusion or new restart rumor selected.

Next expected updates: Confirm any rap-cel/zola-cel resumption only after a new source; no restart date was verified. Afimetoran full phase 2 results and pivotal development are undated. Rese-cel data mid-2027 and possible filing H2 2027 remain separate, conditional sponsor targets; the other programs’ pauses should not be assigned to rese-cel.

Tracked programs and current evidence

2026-09-12 — Initial evidence baseline

Current position: CAR-T has produced striking remission signals in small studies of severe, refractory autoimmune disease, and registrational programs are now underway. The central unanswered question is whether these responses translate into durable clinical benefit with acceptable treatment burden and safety in larger, more representative populations. An immune system becoming repopulated with B cells after treatment is a biological observation; it does not by itself prove that autoimmunity has been permanently eliminated. This watch records disease-specific responses, how long patients remain off other drugs, and the denominator with adequate follow-up.

Academic CD19 CAR-T establishes the proof of concept. A 2024 peer-reviewed case series included 15 heavily pretreated patients: eight with lupus, three with inflammatory myositis and four with systemic sclerosis. Median follow-up was 15 months, with a range of four to 29 months. The study reported lupus remission, major myositis responses, improvement in systemic-sclerosis disease activity, and withdrawal of immunosuppression. Treatment included fludarabine/cyclophosphamide preconditioning. Most cytokine-release events were mild, but grade 2 cytokine-release syndrome, mild neurotoxicity and a pneumonia requiring hospitalization each occurred in one patient. With no randomized comparator, the study cannot quantify benefit relative to current care or reliably estimate rare adverse events. These were refractory patients selected for an intensive intervention, not people with mild disease. NEJM case series.

Resecabtagene autoleucel, rese-cel/formerly CABA-201 — Cabaletta Bio. The lead watch is RESET-Myositis (NCT06154252), an open-label study listed as recruiting in its June 10, 2026 registry update. The registrational cohort includes dermatomyositis and antisynthetase syndrome; the broader study also includes additional myositis cohorts. Its registration and protocol evolution matter because a small single-arm registrational design provides different evidence from a large randomized trial. RESET-Myositis registry.

Cabaletta's August 13, 2026 update forecasts data from the 17-patient registrational adult cohort in mid-2027, with a possible first BLA in the second half of 2027 if successful. These are sponsor targets, not regulatory commitments. The company is also exploring treatment without preconditioning in RESET-SLE (NCT06121297), pemphigus and myasthenia gravis. After early low-dose findings it is advancing higher-dose cohorts; omission of chemotherapy could reduce treatment burden, but must preserve depth and durability of clinical response. The August update does not establish a chemotherapy-free regimen as effective or approved. Manufacturing improvements are operational milestones, not additional evidence of remission. August business update; RESET-SLE identifier and early clinical program.

In vivo CAR-T is now human research, but much earlier. A September 17, 2025 NEJM letter reports generating CD19 CAR-T cells inside patients with refractory lupus using lipid nanoparticles. It describes B-cell depletion and reduced disease activity without major toxic effects in the reported early experience. This supports feasibility, not an established comparative benefit or a reliable estimate of uncommon harms. It is technically and clinically distinct from collecting and modifying a patient's T cells in a factory. Follow larger cohorts, reproducible delivery, immune recovery, redosing, organ-specific outcomes and duration of remission. No firm next readout date was verified, and this baseline does not invent an NCT identifier for the report. Primary human report.

U.S. status and interpretation: the featured autoimmune uses remain investigational in the verified program descriptions. A designation such as RMAT or an FDA-cleared investigational application is not permission to market the therapy. Positive oncology CAR-T experience does not automatically establish an autoimmune indication. Track all serious infections, prolonged cytopenias, hypogammaglobulinemia, retreatment and hospital burden alongside efficacy; short follow-up cannot settle lifetime safety or whether a finite intervention replaces chronic therapy.

Expected next updates

  • Rap-cel and zola-cel: autoimmune trial activity/enrollment paused as described in the September 14 corrective entry; dates for resumption unverified.

  • Afimetoran: full week-48 SLE data and planned pivotal SLE/CLE development; no firm date announced.

  • Rese-cel registrational myositis: mid-2027 data, then possible second-half-2027 BLA — sponsor forecasts dated August 13, 2026, contingent on results.

  • Preconditioning-free cohorts: ongoing dose exploration; no dependable next public release date in the latest verified update.

  • In vivo lupus approach: expanded human evidence and durability are the next substantive milestones; no reliable date verified. No analyst timing estimate assigned.

Unconfirmed / informal

Informal signals

September 27 status check: Existing claims remain unconfirmed; no material status change or repeat alert.

  • mount_sinai_2026-09-22_ibd_cart_planning — unconfirmed: Saurabh Mehandru’s attributed planning statement remains a planning signal, without verified new trial initiation. The separate September 23 BCMA CAR-T enterocolitis study neither confirms this proposed IBD trial nor establishes IBD treatment efficacy. OncLive CAR-T discussion.

2026-09-22 — Informal IBD CAR-T planning signal

Informal statement — unconfirmed. OncLive’s summary of a September 22 discussion attributes to Mount Sinai gastroenterologist Saurabh Mehandru that IBD CAR-T trial planning is underway at his institution. It also describes trials in Europe and his expectation of more specific, shorter-lived approaches.

The indexed article summary was accessible; original video/transcript and timestamps were not. No protocol, trial identifier, start date, construct or efficacy result for this local plan was verified. The anticipated shorter-lived approach is an expert expectation, not a confirmed program design. Source: OncLive discussion summary.

Next check: Seek original recording and matching institutional/registry announcement; do not infer enrollment or benefit.

What we’re watching
  • Follow formal human trials aiming for durable disease control or treatment-free remission in autoimmune disease, with initial emphasis on CAR-T and related approaches targeting disease-driving immune cells.
  • Include lupus/lupus nephritis, myositis, systemic sclerosis and relevant neurologic autoimmune diseases. Keep diagnosis-specific evidence separate.
  • Distinguish autologous cells manufactured outside the body, donor-derived products, and in vivo engineering; record whether lymphodepleting chemotherapy is required.
  • Prioritize durable remission, organ function, relapse, infection and long-term safety. B-cell depletion and changes in autoantibodies alone do not establish an immune reset or cure.
  • Add important new human-stage mechanisms and relevant labeled informal signals under the README rules; exclude gray-market protocols.

2026-09-14 — Focused ad-hoc update

Earlier safety update: rap-cel studies halted; zola-cel enrollment separately paused. In company statements reported by BioPharma Dive on August 31 and updated September 1, Novartis confirmed that it halted rap-cel autoimmune/neurology studies on August 24 after three cases of immune effector cell-associated hemophagocytic syndrome led to fatal complications. BMS separately confirmed a voluntary pause in zola-cel autoimmune enrollment after transient, reversible inflammatory events. These are distinct programs and events; the reports do not establish a class-wide failure. No restart date was confirmed in the sources checked. This corrects missing late-August safety context rather than representing new September 14 news. Original company-confirmed reporting, August 31 / updated September 1. Independent company-confirmed reporting, September 1.

Next: Confirm whether and when the affected programs resume. Rap-cel oncology studies were not included in the reported halt; no restart date was verified for the affected autoimmune/neurology work.

Source limitation: These original reports cite emailed confirmations from both companies; a standalone sponsor/regulator hold notice was not retrieved. The inspected rap-cel NCT06665256 and zola-cel NCT07015983 registry records were last posted before the public pause reports and cannot independently establish the reason for their status. Historical small-study results above are preserved, but they cannot exclude uncommon serious toxicity.

Oral afimetoran meets phase 2 lupus endpoint; full effect sizes pending. Beeline reported positive phase 2 results for afimetoran, an oral TLR7/8 inhibitor, in 248 adults with active moderate-to-severe systemic lupus erythematosus. In NCT04895696, three daily doses were compared with placebo for 48 weeks alongside background therapy and a required corticosteroid taper. All three doses improved the week-48 SRI-4 response endpoint versus placebo (p<0.001 for each). Response percentages, effect sizes and detailed adverse-event tables were not released. The sponsor reported no new safety signal and plans pivotal development. This is a sponsor topline result, not evidence of a cure or an FDA approval; full data are needed to judge clinical magnitude. Beeline, September 9. NCT04895696.

Next: Full congress data and planned pivotal SLE/CLE development; dates not specified.

Coverage: The Cabaletta August 13 update was rechecked without a new clinical readout. A September 9 Reddit discussion of Moderna mRNA-6007 traced to the original June 25 Science Day announcement; this is an older platform-development announcement, not new September human efficacy. Other autologous, allogeneic and in-vivo programs were not exhaustively refreshed. No broad successful-check timestamp is advanced.

Dated findings

  • 2026-09-12: Established baseline focused on clinical remission evidence, rese-cel's registrational program and early in vivo human work. Future monitoring should broaden named programs when new formal human evidence emerges.

2026-09-18 — Scheduled focused review

Rap-cel/zola-cel/RESET original registries plus new human autoimmune and restart searches; old registry status cannot confirm resumption. Original event dates, safety and limitations are retained in the September 18 update above. The complete-topic successful-check date was not advanced because the listed gaps remain. Run record.
