Cancer treatment, vaccines and recurrence prevention

FDA’s October 8 action adds atezolizumab with chemotherapy after resection of stage III dMMR colon cancer, supported by ATOMIC disease-free survival and substantial treatment toxicity. Formulation-specific ages and weight limits differ; overall-survival benefit is not established. New NEONIPIGA follow-up is small, uncontrolled and distinct from randomized recurrence-prevention evidence.

Where things stand

Established uses

FDA approvals include daraxonrasib for specified metastatic pancreatic cancer (August 26) and atezolizumab plus chemotherapy after resection of stage III dMMR colon cancer (October 8). The latter reduces recurrence/death risk after cancer; it is not vaccination of healthy people.

In development

New phase 3 T-bren urothelial-cancer and phase 2a LUTEON ATLAS kidney-cancer registrations expand the ADC/radiopharmaceutical pipeline. They have no reported efficacy or safety outcomes; prior NRG-BR002 negative survival findings remain relevant.

Key finding

In 644 eligible advanced triple-negative breast-cancer patients, Datroway improved median survival to 23.7 months versus 18.7 with chemotherapy in a randomized trial.

Main uncertainty

Cancer and treatment setting matter. SERENA-4 missed its PFS endpoint; a different strategy is tested in SERENA-6. NEONIPIGA reports durable responses in localized dMMR/MSI-high gastric/GEJ cancer, but its uncontrolled follow-up does not show a clear survival improvement over surgery-only studies.

FDA approves adjuvant atezolizumab for resected stage III dMMR colon cancer

U.S. status — October 8 approval: FDA approved IV Tecentriq (atezolizumab) with fluoropyrimidine- and oxaliplatin-based chemotherapy for adjuvant treatment of stage III mismatch repair–deficient colon cancer in adults and children aged 2 or older. Subcutaneous Tecentriq Hybreza (atezolizumab and hyaluronidase-tqjs) received the same indication for patients aged 12 or older weighing at least 40 kg. This is postoperative cancer treatment, not primary prevention in healthy people. Approval alone does not establish payer coverage or local inventory.

Evidence: In ATOMIC (NCT02912559), 712 patients with completely resected stage III dMMR disease were randomized to atezolizumab plus mFOLFOX6 followed by atezolizumab, or mFOLFOX6 alone. FDA reports disease-free-survival HR 0.50 (95% CI 0.35–0.73); median DFS was not reached in either arm. The original March 25, 2026 paper reports 3-year DFS 86.3% (95% CI 81.8–89.8) versus 76.2% (70.9–80.6) and grade 3/4 adverse events in 84.1% versus 71.9%. These are previously published trial results supporting the new approval, not a new October efficacy analysis.

Limitations and harms: The FDA summary does not establish an overall-survival advantage. Immune-mediated adverse reactions and infusion reactions require monitoring; toxicity is material despite the recurrence benefit.

Next milestone: Longer-term survival and real-world access/safety follow-up; no dependable next release date identified.

NEONIPIGA publishes four-year follow-up, with an important survival limitation

Evidence — October 15 publication: NEONIPIGA (NCT04006262) enrolled 32 patients with localized dMMR/MSI-high gastric or gastroesophageal-junction adenocarcinoma. Treatment combined neoadjuvant nivolumab/ipilimumab, planned surgery and adjuvant nivolumab. At median follow-up of 48.3 months, 4-year overall survival was 84.1% (95% CI 65.8–93.0; all 32 patients); 4-year event-free survival was 83.5% (64.8–92.8; 31 surgery-eligible patients). Pathological complete response occurred in 17 of 29 surgical specimens, as previously reported.

Limitations and harms: This was not randomized. The authors explicitly found no clear survival improvement over other studies of surgery alone. Only 14 of 32 completed all planned treatment. Grade 3/4 treatment-related adverse events affected 6 patients during neoadjuvant therapy and 4 during adjuvant therapy; one death involved postoperative complications. No additional late toxic effects were reported. This cannot establish that surgery can safely be omitted.

Status and timing: This is a new journal report of long-term follow-up previously presented at ASCO 2026, not a fresh randomized readout or new U.S. approval. A randomized organ-preservation strategy needs further testing; no next result date was identified.

Sources: JAMA Network · article; ascopubs.org · article.

Remaining gaps: Focused cancer-treatment, recurrence and safety update. The missed October 8 FDA ATOMIC approval was checked against the regulator and the original March 25 paper; October 15 NEONIPIGA publication was appraised. Saved registries and 72 discovery records were retrieved; each record and the full worldwide oncology pipeline were not individually appraised. ESMO late-breaking abstracts and all ADC/cell/radiopharmaceutical entrants remain incomplete. Previously saved NRG-BR002, T-bren and LUTEON/ATLAS entries were deduplicated. No full successful-check advancement.

Informal-source coverage: Indexed original SELLAS October 5, 6, 11–15 discussions continue event-count modeling and imminent-readout speculation. The October 12 modeling thread predicts a December 24 announcement, while others predict earlier; these are incompatible anonymous estimates, not independent evidence. No official new REGAL final event/OS result was established in the checked sources; the saved imminent-event rumor remains unresolved. No repeated alert.

T-bren registers a phase 3 study in HER2-expressing urothelial cancer

First posted: October 13, 2026. NCT07869836, SystImmune, registers a 520-person phase 3 comparison of trastuzumab brengitecan/T-bren/BL-M07D1, a HER2 ADC, against gemcitabine plus cisplatin or carboplatin in HER2-expressing metastatic urothelial cancer progressing after anti-PD-(L)1-based treatment.

The trial is randomized and open-label with masked outcome assessment. BICR progression-free survival in the primary population is primary; overall survival, intent-to-treat analyses, response and toxicity are secondary. Two investigational dose levels are listed.

Status/limits/U.S.: Recruiting, but September 2026 start remains estimated. No new response, PFS, OS or safety result is posted. HER2 expression and this treatment line must not be replaced with a tumor-agnostic claim. No U.S. marketing approval established by this record.

Next: December 2028 estimated primary and December 2029 final completion; neither is an announced public readout date.

LUTEON ATLAS registers radiopharmaceutical dose optimization in kidney cancer

First posted: October 13, 2026. LUTEON ATLAS/NCT07870746, Telix, plans 40 adults with CAIX-expressing advanced relapsed/recurrent clear-cell renal-cell carcinoma, following prior systemic therapy. Two regimens of 177Lu-TLX250/lutetium 177 girentuximab tetraxetan will be assessed to choose a later phase 3 dose.

The phase 2a design is open-label, nonrandomized and sequential, rather than a randomized standard-care comparison. Safety/tolerability dose optimization is primary; tumor/organ dosimetry, pharmacokinetics, response and survival monitoring are separate secondary measures. One secondary OS description refers to randomization despite the nonrandomized design; retain the inconsistency pending protocol clarification.

Status/limits/U.S.: Not yet recruiting, no efficacy/safety outcomes or new approval. This treats diagnosed kidney cancer; it is separate from kidney preservation and not evidence of longer survival.

Next: October 15, 2026 estimated start; December 31, 2028 primary and October 13, 2032 final completion estimates. Verify actual first treatment.

Remaining gaps: Read expanded oncology topic/state and September 5 overlap; retrieved 26 saved registries and 65 new-study matches. Appraised selected new phase 3 HER2 ADC and phase 2a CAIX radiopharmaceutical registrations; no duplicate NRG-BR002 or prior vaccine/regulatory events.; FDA oncology index retrieved but its latest visible update remains September; not a comprehensive Oct 13 approval/label audit. BNT122 registry active/not-recruiting versus August 28 sponsor termination/futility/OS imbalance remains a source conflict; no restart inferred. NRG-BR002 full paper/toxicity and global cellular/radiopharmaceutical census remain incomplete.

Informal-source coverage: REGAL event 80 imminent inference rechecked against sponsor news index and new Reddit discussions; no official trigger or topline confirmation retrieved, index lacks rendered dynamic entries. Leave unconfirmed without repeat alert. Leronlimab/AlphaDaRT investor forecasts not efficacy/approval evidence. All original investor calls/transcripts remain incomplete.

NRG-BR002 publication finds no added survival benefit from ablating breast-cancer metastases

Source/event date: October 7, 2026 publication; this reports an older trial, not new October treatment. Evidence: Randomized NRG-BR002/NCT02364557, described in the investigator's release; full paper and toxicity tables remain unappraised.

The U.S./Canadian trial enrolled 129 patients, with 125 eligible for analysis, with at most four breast-cancer metastases. First-line systemic therapy alone was compared with systemic treatment plus ablation of all visible metastatic sites, usually SBRT. Median PFS was 23.0 versus 19.5 months; neither PFS nor overall survival improved significantly. The trial did not progress to phase 3.

Limits/safety: This does not establish equivalence, generalize to other cancers, or negate local treatment for pain or other symptoms. Low circulating-tumor-cell subgroup findings are exploratory, not a validated selection test. Detailed serious-toxicity counts were unavailable in the appraised primary release.

U.S. status / next milestone: No drug approval change. Biomarker-guided follow-up studies are described without a verified next readout date.

ImmProCure registers a vaccine and bispecific trial for prostate-cancer recurrence

Source/event date: First posted October 6, 2026. Evidence: ImmProCure/NCT07860047, University Hospital Tübingen phase 2 registration.

The open-label randomized German study plans 84 men with biochemical prostate-cancer recurrence after treatment intended to cure, without distant metastases on PSMA-PET, selected for relatively slow progression. It compares VAC-XS15-PC01 multipeptide vaccine, CC-1 PSMA×CD3 bispecific antibody, and their combination. The primary endpoint is a PSA response of at least 50%; safety is monitored.

Limits: A PSA decrease or immune response is not proven metastasis prevention, survival benefit, or cure. No efficacy/safety results are posted. This treats recurrence after cancer, not prevention in healthy people.

Development / U.S. status: Registry lists recruiting, but the September 24 start remains an estimate. Tübingen is recruiting; the other listed German sites are not yet recruiting. Both experimental approaches remain investigational, without a new U.S. approval.

Next milestone: March 31, 2029 primary and March 31, 2030 final completion, registry estimates rather than readout promises.

VA registers a LabVax combination study in advanced lung adenocarcinoma

Source/event date: First posted October 6, 2026. Evidence: VA-sponsored phase 2 NCT07859592.

The study plans 30 participants with advanced/metastatic or recurrent lung adenocarcinoma after at least one checkpoint-inhibitor line. LabVax 3(22)-23, targeting the labyrinthin antigen, is combined with GM-CSF and pembrolizumab. It uses an open-label sequential design without a standard-treatment control arm. Objective response is primary; PFS, OS and toxicity are also tracked.

Limits/safety: No results establish response, survival or safety. An antigen-directed immune response would not itself establish clinical benefit. The protocol can stop for insufficient safety or response. This is treatment of existing advanced cancer, distinct from recurrence prevention after surgery or primary prevention.

Development / U.S. status: Not yet recruiting. December 1, 2026 start is estimated; experimental vaccine access is through research, not an approval.

Next milestone: November 30, 2030 primary and December 1, 2035 final completion estimates; neither is a guaranteed publication date.

Final IMpassion050 follow-up finds no significant event-free or disease-free benefit

Source/event date: September 24, 2026 publication; data cutoff August 24, 2023. Evidence: Peer-reviewed final follow-up, IMpassion050/NCT03726879; primary abstract appraised.

The randomized phase 3 trial enrolled 454 patients with high-risk HER2-positive early breast cancer. Atezolizumab or placebo was added to neoadjuvant pertuzumab/trastuzumab/chemotherapy and continued after surgery with HER2 therapy. Median follow-up was 44.2 / 43.4 months. Three-year EFS 91.4% versus 89.0%; HR 0.90 (95% CI 0.50–1.59). Among 434 surgical/postneoadjuvant participants, DFS 92.9% versus 88.5%; HR 0.71 (0.38–1.32). Neither difference was statistically significant; numerical subgroup findings do not reverse the earlier null pathologic-complete-response result or establish survival benefit.

Safety: Adjuvant special-interest AEs were 59.4% versus 47.0%, with none grade 5 in that subset. This is not a statement that the whole trial had no fatal events; the full safety appendix remains unappraised.

U.S. status / next milestone: No new approval from publication; this regimen should not be generalized to other breast-cancer subtypes or cancer prevention. No announced next confirmatory/readout date.

Shorter pancreatic radiotherapy better preserves reported quality of life; survival advantage unproven

Source date: 2026-09-28. Evidence: Randomized phase 2 conference abstract and investigator-society report.

SOFT Preop enrolled 102 people with operable or selected locally advanced pancreatic cancer after chemotherapy; four withdrew. Five SBRT sessions were compared with 28 radiation sessions plus concurrent chemotherapy. Among 83 surgical completers, node positivity was 41% versus 38% (p=.754), missing the hypothesized advantage of longer chemoradiation. Physical function and fatigue were better preserved during radiotherapy, with no significant quality-of-life difference after surgery. Median survival from enrollment after initial chemotherapy was 26.04 months (95% CI 17.9–59.28) versus 42.96 (18.72–54.72), p=.639; median follow-up was 3.4 years.

Limitations: This small superiority trial does not establish survival equivalence or noninferiority. Quality-of-life effect sizes and confidence intervals were absent from the available abstract. SBRT included regional nodes; results cannot be generalized to every SBRT regimen. Exact registry ID not verified.

Safety: No significant surgical-complication difference reported; full toxicity counts were not provided.

U.S. status: A technique-comparison study, not a new drug authorization or recommendation for every pancreatic cancer patient.

Next milestone: The abstract is public. The program scheduled the scientific talk September 29 at 14:55 Eastern, after a scheduled September 28 press briefing; actual delivery was not verified. Larger studies remain undated.

BHV-1530 opens a checkpoint-inhibitor combination cohort

Source date: 2026-09-28. Evidence: Sponsor trial-development announcement.

Biohaven reports enrollment has begun for its FGFR3 antibody–drug conjugate BHV-1530 plus cemiplimab in advanced solid tumors. NCT06874335 is an open-label phase 1 dose study. Its July 9 registry update lists 140 planned participants across the program, not 140 in this new cohort; no results are posted. The study began March 20, 2025.

Limitations: The release does not provide new cohort-specific response rates, denominators or comparative survival. Previously disclosed partial responses cannot establish superiority.

Safety: Sponsor reports no dose-limiting or typical FGFR-inhibitor toxicities in previously disclosed data; small early exposure and missing detailed tables preclude broad safety conclusions.

U.S. status: BHV-1530 remains investigational; cemiplimab approval in other settings does not approve this combination.

Next milestone: Updated phase 1 presentation scheduled November 19, 2026 at 10:00 in Barcelona (release does not explicitly label the time zone). Registry completion March 2029 is an estimate.

FDA approves belzutifan plus lenvatinib after immunotherapy for advanced kidney cancer

Source date: 2026-09-24. Event date: 2026-09-24. Evidence: FDA approval and randomized phase 3 evidence.

Welireg plus Lenvima is approved for advanced renal-cell carcinoma with a clear-cell component after PD-1/PD-L1 therapy. LITESPARK-011 (NCT04586231; n=747) compared the combination with cabozantinib. Median PFS: 14.6 versus 10.6 months; HR 0.74 (95% CI 0.61–0.89). Response: 53% versus 40%.

Limitations: Final overall survival was not statistically significant: 33.7 versus 28.6 months, HR 0.85 (0.70–1.03). Better PFS does not establish longer survival; this is cancer treatment, not kidney preservation.

Safety: Belzutifan carries embryo-fetal, anemia and hypoxia risks. Lenvatinib adds hypertension, cardiac, renal/hepatic and hemorrhage risks.

U.S. status: FDA-approved September 24, 2026 for this combination and population; local stock and coverage not checked.

Next milestone: Longer clinical and real-world follow-up; no verified next public date.

Anito-cel phase 1 publication pairs durable responses with substantial toxicity

Source date: 2026-09-23. Event date: 2026-09-23. Evidence: Peer-reviewed uncontrolled phase 1 study.

Among 38 infused patients with heavily pretreated relapsed/refractory myeloma, all responded; 30/38 had complete responses. NCT04155749 enrolled 40. Median follow-up was 38.1 months; median PFS 30.2 months (95% CI 16.6–36.2), 24-month PFS 57% (39–71), and 36-month survival 65% (46–79).

Limitations: Small uncontrolled dose study; neither complete response nor survival estimates establish cure or superiority to another CAR-T. Laboratory binder comparisons are not a clinical head-to-head trial.

Safety: All 38 had adverse events and 37/38 grade ≥3 events. At the recommended dose, 94% had grade 1/2 cytokine-release syndrome; none grade ≥3. ICANS included one grade 3 event. No delayed/non-ICANS neurotoxicity observed does not exclude rare risks.

U.S. status: Investigational; this paper is not an approval.

Next milestone: Larger confirmatory data and regulatory follow-up; no new date established by this publication.

Human tissue study investigates persistent CAR-T-associated enterocolitis

Source date: 2026-09-23. Event date: 2026-09-23. Evidence: Peer-reviewed observational human study.

Nature Medicine described persistent gut inflammation after BCMA CAR-T therapy, informing safety monitoring as cellular therapies expand. Researchers studied ten patients with enterocolitis after ciltacabtagene autoleucel, seven CAR-T-treated controls without enterocolitis and 26 healthy volunteers. Tissue analyses linked mucosal CAR-T persistence and inflammatory remodeling. Two patients improved clinically, endoscopically and histologically after upadacitinib.

Limitations: These were multiple-myeloma patients receiving BCMA CAR-T, not autoimmune patients receiving CD19 CAR-T. Selected observational cases cannot estimate incidence or establish a class-wide causal mechanism. Two uncontrolled rescue treatments do not establish an effective standard treatment.

Safety: The serious gut complication is the study focus; this is mechanistic evidence about an already recognized risk.

U.S. status: No autoimmune CAR-T approval or approval of upadacitinib for this complication is established by this study.

Next milestone: Controlled management studies and longer follow-up; timing unknown.

Persistence of mucosal CAR-T cells and inflammatory remodeling.

INTerpath-001 detailed phase 3 results scheduled for October 24

Source date: 2026-09-21. Event date: 2026-09-21. Evidence: Sponsor conference announcement.

Moderna identifies ESMO LBA1 as the detailed phase 3 presentation for intismeran plus pembrolizumab in resected stage IIB–IV melanoma. The Presidential Symposium is scheduled for October 24 at 16:30 CEST (10:30 Eastern). The August positive RFS/DMFS topline is already recorded; this announcement adds timing, not new numerical efficacy.

Limitations: Adjuvant recurrence prevention after melanoma treatment, not cancer prevention in healthy people. Numerical benefit, adverse events and survival maturity must be appraised when disclosed.

Safety: No new safety table in the scheduling release.

U.S. status: Investigational combination.

Next milestone: October 24, 2026: scheduled ESMO LBA1; sponsor investor webcast follows at 19:00 CEST.

VNX-101 reports responses in three selected leukemia patients

Source date: 2026-09-21. Event date: 2026-09-21. Evidence: Sponsor preliminary phase 1/2 report.

Three anti-AAV-antibody-negative patients with relapsed/refractory ALL achieved MRD-negative complete responses after VNX-101; the broader SENTRY-CD19 study has dosed nine patients with several CD19-positive malignancies. NCT06533579 is an uncontrolled phase 1/2 dose-escalation study. VNX-101 uses in-vivo gene delivery to produce a CD19/CD3 T-cell engager. The first responding patient remained MRD-negative through day 260 and then received a stem-cell transplant.

Limitations: The three-patient selected ALL subgroup is not the entire nine-patient response denominator. No comparative survival or cure evidence; later transplant complicates durability attribution.

Safety: Cytokine-release syndrome and ICANS occurred during dose escalation and reportedly resolved with standard care. The release omits grades and event denominators.

U.S. status: Investigational; FDA designations are not approval.

Next milestone: Further dose optimization and follow-up are undated; registry estimates primary completion June 2027, not a promised publication date.

Vironexis September 21 release; SENTRY-CD19 registry.

Mosunetuzumab combination meets phase 3 lymphoma endpoint

Source date: 2026-09-16. Evidence: Sponsor topline; randomized phase 3.

CELESTIMO reports improved progression-free survival with mosunetuzumab plus lenalidomide in previously treated follicular lymphoma; overall survival is immature. NCT04712097 compares mosunetuzumab plus lenalidomide with rituximab plus lenalidomide after at least one prior treatment. The registry lists 478 actual participants. Genentech reports that the interim primary endpoint was met, without releasing a hazard ratio or absolute PFS estimates.

Limitations: PFS benefit must not be called a demonstrated survival benefit. The study supports a potential earlier-line indication and confirmatory regulatory review; neither action is completed by the topline announcement.

Safety: Sponsor reports no new safety signal, but comparative adverse-event tables are pending. Known mosunetuzumab risks include serious cytokine-release syndrome and infections.

U.S. status: Existing third-line-or-later monotherapy approval is separate from the investigational second-line combination.

Next milestone: Full results at an unnamed upcoming medical meeting and regulatory submissions; no firm date.

Genentech, September 16; CELESTIMO registry.

Patidegib Gorlin-syndrome study completes follow-up; results forecast for November

Source date: 2026-09-17. Evidence: Sponsor development milestone.

The phase 3 SGT-610 study has completed its final patient visit; the sponsor forecasts topline results in late November 2026. SGT-610-01 tests topical patidegib 2% versus vehicle for 12 months to prevent new facial basal-cell carcinomas in adults with Gorlin syndrome. It enrolled 113 people; 102 completed. Database lock and analysis are next, with assignments still blinded.

Limitations: No efficacy or comparative safety result is released. This inherited high-risk cancer-prevention setting does not establish cosmetic benefit or prevention in the general population.

Safety: The completed-visit announcement contains no new outcome-level safety analysis.

U.S. status: Investigational; trial completion and designations are not approval.

Next milestone: Late November 2026 topline: sponsor forecast, not a scheduled day.

Sol-Gel, September 17.

Broader cancer treatment — selective baseline, September 13, 2026

Program / indication and population Development stage Evidence and limitations U.S. status Next milestone and timing basis
Sevabertinib / Hyrnuo — HER2-mutant advanced non-squamous NSCLC; SOHO-01, NCT05099172 Targeted kinase inhibitor; indication expanded September 9, 2026 FDA review: 69 previously untreated patients in a single-arm cohort; response rate 75% (95% CI 64–85). This is response/duration evidence, without a randomized survival comparison. Diarrhea, liver injury, pneumonitis and cardiac/ocular toxicities matter. Accelerated approval for HER2 TKD activating mutations detected by an FDA-authorized test; prior-systemic-treatment restriction removed. Confirmatory clinical-benefit evidence and updated regulatory requirements; no reliable next public readout date verified. FDA, September 9.
Datopotamab deruxtecan / Datroway — advanced triple-negative breast cancer (TNBC); TROPION-Breast02, NCT05374512 TROP2-directed ADC; completed randomized Phase 3 In 644 patients without prior systemic therapy for advanced disease and ineligible for PD-1/PD-L1 therapy, median overall survival was 23.7 versus 18.7 months with chemotherapy (HR 0.79, 95% CI 0.64–0.98); PFS also improved. Open-label design; selected population. Pneumonitis, ocular injury and stomatitis remain material harms. Approved May 22, 2026 for unresectable/metastatic TNBC in adults who are not PD-1/PD-L1 inhibitor candidates. Longer follow-up, treatment sequencing and new combination/indication results; no firm next release date verified. FDA approval and trial results.
Lifileucel / Amtagvi — previously treated advanced melanoma; TILVANCE-301, NCT05727904, tests earlier combination treatment Autologous tumor-infiltrating lymphocyte therapy; Phase 3 confirmatory trial listed as enrolling Original single-arm approval cohort: response rate 31.5% (95% CI 21.1–43.4; 73 patients in the recommended dose range). The randomized trial compares lifileucel plus pembrolizumab with pembrolizumab in untreated advanced melanoma. Lymphodepletion/IL-2 and treatment-related mortality, severe cytopenia, infection and organ toxicity are substantial burdens. Accelerated approval since February 16, 2024 after PD-1 therapy and, for BRAF V600-positive tumors, appropriate BRAF-targeted therapy. The frontline combination remains investigational. Randomized efficacy and confirmatory outcome reporting; no reliable public readout date verified. FDA evidence/safety; current sponsor trial listing.
Lutetium Lu 177 vipivotide tetraxetan / Pluvicto — PSMA-positive metastatic hormone-sensitive prostate cancer; PSMAddition, NCT04720157 Targeted radiopharmaceutical; completed Phase 3 primary analysis In 1,144 randomized patients, addition to androgen-receptor pathway inhibition with ongoing androgen deprivation improved radiographic PFS (HR 0.72, 95% CI 0.58–0.90). Overall-survival data were immature. Radiation exposure, marrow suppression and renal toxicity remain important. Approved July 31, 2026 with an androgen-receptor pathway inhibitor for PSMA-positive metastatic androgen pathway modulation-naïve or -sensitive disease; approved PSMA PET selection required. Mature overall survival and long-term safety; no firm public date verified. FDA indication and PSMAddition results.

Tracked programs and current evidence

2026-09-12 — Initial evidence baseline

Current position: this is a credible category with a recent phase 3 success, but results depend on the cancer, treatment setting and combination. A vaccine intended to prevent recurrence is still treatment for people who have already had cancer. It is not evidence that healthy people can receive a general cancer-prevention vaccine. Established HPV and hepatitis B vaccination prevent infection-related cancers; the experimental tumor-antigen approaches below are a different research question. FDA-approved sipuleucel-T already provides a treatment-vaccine precedent for selected metastatic prostate cancer patients. NCI treatment-vaccine overview; ACS vaccine distinctions.

Intismeran autogene, formerly V940/mRNA-4157 — Merck/Moderna. On August 19, 2026, the sponsors reported that phase 3 INTerpath-001 (NCT05933577) met recurrence-free survival and distant-metastasis-free survival endpoints in completely resected stage IIB–IV melanoma. The trial enrolled 1,137 patients and randomized them 2:1 to individualized mRNA therapy plus pembrolizumab or the pembrolizumab control regimen. The release reports no new safety signals, but does not provide the phase 3 hazard ratios, absolute event rates or mature overall-survival results. This is a sponsor topline announcement, not yet a fully appraised phase 3 paper. Do not substitute earlier phase 2 effect sizes for these results. Full data are promised at an upcoming international meeting, with no named meeting or date in this release; regulatory engagement is planned, not an approval. August 19 announcement.

The platform also has trials in other cancers, including phase 3 adjuvant non-small-cell lung cancer INTerpath-002 (NCT06077760). An August 2026 investigator/sponsor publication describes the clinical portfolio and production pathway. Manufacturing a unique product from each patient's tumor creates questions about turnaround, unsuccessful manufacture and treatment access; melanoma efficacy cannot be assumed in lung, kidney or bladder cancer. Clinical development and manufacturing paper.

Autogene cevumeran, BNT122/RO7198457 — BioNTech/Genentech. The August 28, 2026 update is a significant negative result: the sponsors terminated phase 2 BNT122-01 (NCT04486378), which tested vaccine monotherapy against watchful waiting after surgery in ctDNA-positive, high-risk stage II/stage III colorectal cancer. A futility boundary had been crossed in October 2025. The later review found an overall-survival numerical imbalance and little prospect that continued follow-up would change efficacy conclusions. The DSMB recommended stopping treatment and the trial; the sponsor separately states that no new safety signal was identified. The public release supplies neither the numerical survival results nor enough detail to resolve the imbalance. Retain both statements and await full reporting. The pancreatic combination study was explicitly unaffected. August 28 sponsor statement.

Pancreatic cancer, IMcode003 (NCT05968326). This randomized phase 2 trial compares autogene cevumeran plus atezolizumab and mFOLFIRINOX with mFOLFIRINOX alone after resection. It is a more informative test than comparing immune responders with nonresponders in a small uncontrolled early study: responder status itself can reflect prognosis. The sponsor's August statement confirms continuation. No reliable next public readout date was verified. Sponsor trial description.

Prevention/interception: Nous-209 in Lynch syndrome (NCT05078866). A January 16, 2026 peer-reviewed phase 1b/2 report describes 45 participants without active cancer. This shared frameshift-antigen vaccine produced responses in all 37 immunologically evaluable participants; 85% retained detectable responses at one year. There were no intervention-related serious adverse events. The initial cohort was single-arm, and safety/immunogenicity were coprimary endpoints: these results do not yet show fewer cancers or improved survival. This is prevention research in a hereditary-risk population, not a general-population indication. Follow revaccination, lesion/cancer incidence and controlled trials. Nature Medicine paper; NCI trial.

Expected next updates

  • Intismeran: full phase 3 conference data and filing details are announced next steps, but there is no verified date in the August 19 release.
  • Autogene cevumeran: await complete colorectal futility/survival reporting and pancreatic trial progress; no reliable release dates verified.
  • Nous-209: follow longer clinical-outcome and revaccination evidence; no verified cancer-incidence readout date. No analyst calendar estimate is assigned.

Speculation — unconfirmed: REGAL’s final-analysis trigger is imminent

Speculation/inference — unconfirmed. On October 2, 2026 (indexed original date), Reddit user Run4theRoses2, r/sellasLifescience, predicted REGAL’s 80th-event announcement was imminent from prior aggregate event counts and elapsed time. The post also promotes large share-price/valuation expectations, indicating an apparent investment interest. No inside access, original investigator report or new event count is supplied.

REGAL studies galinpepimut-S maintenance after second-line salvage remission in AML. The official August 11 sponsor release says reaching event 80 triggers database lock, blinded review, statistical analysis, unblinding and subsequent topline disclosure. The sponsor does not give a calendar date in that source. Slow aggregate event accrual alone cannot identify treatment-arm survival, guarantee a positive result or establish cure. First-line SLS009 is a separate program.

Next check/status: Await official event 80/database-lock/topline announcements. Record unconfirmed on October 6; do not infer marketing approval or alert the same speculative forecast repeatedly.

2026-09-14 — Focused ad-hoc update

Enhertu improves first-line HER2-mutant lung-cancer PFS; survival benefit unproven and fatal lung toxicity reported. In DESTINY-Lung04 (NCT05048797), 454 patients with untreated advanced HER2-mutant nonsquamous NSCLC were randomized to Enhertu or platinum/pemetrexed plus pembrolizumab. Sponsor-reported median progression-free survival was 14.3 versus 8.3 months (HR 0.63, 95% CI 0.50–0.79). Overall survival was immature and not formally tested: 29.3 versus 33.1 months (HR 1.15, 95% CI 0.88–1.52), so this readout does not demonstrate longer survival. Interstitial lung disease/pneumonitis occurred in 20.8% with Enhertu, including four grade 5 events (1.8%). This first-line use remains investigational in the US; prior approval for previously treated disease should not be generalized to this setting. Sponsor report, 2026-09-14.

Next: Mature overall survival and any first-line regulatory action; dates unannounced. The September 14 conference report used a June 9, 2026 data cutoff.

Camizestrant misses primary endpoint in upfront first-line SERENA-4. AstraZeneca reported that SERENA-4 did not achieve a statistically significant progression-free survival improvement for camizestrant plus palbociclib versus anastrozole plus palbociclib in untreated advanced ER-positive/HER2-negative breast cancer. The randomized, double-blind phase 3 trial enrolled 1,371 patients. Numerical effect estimates and confidence intervals have not been released; the sponsor reported no new safety concern. This negative result concerns upfront first-line treatment. It should not be conflated with the different SERENA-6 strategy of switching after an emerging ESR1 mutation during endocrine therapy before clinical progression. Full results remain to be presented. Sponsor report, 2026-09-11.

Next: Full numerical SERENA-4 results; date not announced.

ADAURA reports sustained eight-year survival difference after adjuvant osimertinib. An exploratory eight-year ADAURA follow-up reported overall survival of 74% with adjuvant osimertinib versus 58% with placebo in the resected EGFR-mutant stage II–IIIA population (HR 0.53, 95% CI 0.38–0.75). Across stages IB–IIIA, eight-year survival was 79% versus 64% (HR 0.52, 0.39–0.71). The randomized trial enrolled 682 patients. This later analysis has a follow-up limitation: 127 patients supplied no additional survival data beyond the prior planned final analysis and remained censored. It supports durability of the observed benefit but is exploratory follow-up, not a new approval or a new randomized trial. No new safety collection was reported for this analysis. Sponsor report, 2026-09-14.

U.S. status: Osimertinib already has an approved adjuvant use; this follow-up is not a new approval. The data cutoff was May 4, 2026. The linked Journal of Thoracic Oncology article could not be retrieved, so this entry is explicitly sponsor-reported.

DeLLphi-305 maintenance combination meets survival endpoint in extensive-stage SCLC. AstraZeneca reported that adding tarlatamab to durvalumab maintenance improved overall survival, progression-free survival and response rate versus durvalumab alone in phase 3 DeLLphi-305. The open-label trial randomized 563 patients with extensive-stage small-cell lung cancer without progression after durvalumab plus platinum/etoposide induction. Hazard ratios, absolute survival estimates and detailed safety results were not released; the sponsor reported no new safety signal. Full results are required to assess the size and trade-offs of benefit. This report does not establish US approval of the maintenance combination. Sponsor report, 2026-09-08.

Next: Full DeLLphi-305 presentation and any regulatory submission; timing unspecified.

Coverage and deduplication: This was a focused broader-oncology refresh, not a global pipeline census. The September 9 sevabertinib approval was already in the September 13 baseline and was not treated as new. Vaccine informal-source searches located investor discussion about possible INT presentation timing, but no original official confirmation of a named meeting; the existing undated forecast remains. Other vaccine programs, full registry histories and the original investor-conference recording were not comprehensively reviewed.

Dated findings

  • 2026-09-13 scope baseline: added targeted drugs, ADCs, cellular therapies and radiopharmaceuticals, using four FDA regulatory records and an active sponsor trial listing. Preserved all vaccine evidence and its uncertainty. Unknown outcome-release dates remain undated; this is not an exhaustive refresh of the older programs.
  • 2026-09-12: Established baseline; priority events are the August melanoma phase 3 success, August colorectal trial termination, and January Lynch-syndrome prevention study. All are baseline context rather than newly discovered future outcomes.
