Infectious-disease prevention and treatment

A large randomized shingles-vaccine trial will directly measure cardiovascular events. Travelan’s new full challenge-study report retains a nonsignificant diarrhea endpoint and wide uncertainty; newer antibiotics, phages and transplant-vaccine programs remain distinct.

Where things stand

Established uses

New antibiotics and prevention products are approved for specific infections and populations; future antibiotics and resistant infections are part of the continuing watch.

In development

A large randomized shingles-vaccine trial will directly measure cardiovascular events. Travelan’s new full challenge-study report retains a nonsignificant diarrhea endpoint and wide uncertainty; newer antibiotics, phages and transplant-vaccine programs remain distinct.

Key finding

In the Zaynich complicated-UTI trial, combined clinical/microbiological response was 89.0% versus 68.4% with meropenem. The primary population excluded meropenem-resistant pathogens.

Main uncertainty

Laboratory activity does not establish clinical benefit against every resistant infection. Pathogen, infection site, resistance emergence, relapse and toxicity matter.

What comes next

EMA schedules a bacteriophage regulatory workshop

Agenda updated October 9; checked October 16. EMA schedules its bacteriophage-therapy workshop for October 30, 2026, 09:00–18:00 Amsterdam time (CET), with online participation. The agenda covers the clinical evidence and potential European regulatory framework for phage treatment of resistant infections.

This is a policy/evidence discussion, not a drug approval, new randomized result or harmonized authorization to use every phage product. No change to U.S. availability is established. The event page has a conflicting CEST broadcast heading; its dated agenda explicitly says CET.

Sources: EMA · source page; EMA · PDF document.

Remaining gaps: All saved named registry records and every page of bounded September 5–October 16 CT.gov discovery were retrieved; 355 discovery records were screened selectively. Retrieval is not full appraisal. Worldwide registries, all publications/regulatory files, complete harms and original audio/transcripts remain incomplete. Prior full successful-check dates are preserved. New completion labels for VAX-31 and VYD2311 do not add results beyond earlier disclosures; future-antibiotic/phage worldwide coverage remains selective.

Informal-source coverage: Targeted Reddit, interview, podcast and saved unresolved-claim searches were appraised selectively; original recordings and worldwide informal coverage remain incomplete. Routine anecdotes and reposts were not treated as new trial evidence. Original adibelivir Reddit post describes an October 9 non-substantial CTIS modification whose contents were unavailable. That routine, unverified administrative inference was not promoted to an efficacy or approval rumor.

Large shingles-vaccine trial will measure cardiovascular events

First posted October 13, 2026. GSK’s NCT07872046 plans a randomized masked phase 3 study of 16,390 adults aged at least 50, with established coronary/peripheral/cerebrovascular disease or at least two cardiovascular risk factors, on standard care. Two recombinant-zoster-vaccine doses at day 1/month 2 are compared with placebo. Primary time to first three-point MACE comprises nonfatal myocardial infarction, nonfatal stroke or cardiovascular death, with follow-up up to month 74. The population therefore spans secondary prevention and high-risk primary prevention, not everyone with prior MI.

Status/limits/safety/U.S.: Not yet recruiting; no hazard ratio or adverse-event results. A vaccine’s established shingles indication does not establish approval or efficacy for CV prevention. This randomized outcomes trial is distinct from observational vaccine associations.

Timing: November 30, 2026 estimated start; November 25, 2032 estimated primary/final completion. These are registry estimates, not confirmed readout dates.

Travelan’s full challenge-study report retains a nonsignificant primary result

Published October 12, 2026; full report of an older completed trial. NCT05933525 randomized 63 healthy adults (safety set); 60 underwent ETEC H10407 challenge, 30 per arm. Once-daily oral bovine-antibody Travelan 1,200 mg for seven days produced moderate/severe diarrhea in 6/30 (20%) versus 10/30 (33.3%): reported protective efficacy 40%, 95% CI −82.2% to 82.1%, nonsignificant. The lower-than-planned placebo attack rate reduced power but does not establish efficacy. Secondary endpoints had no multiplicity adjustment.

Safety/limits: No deaths or serious adverse events; one possible pre-challenge drug reaction. Post-challenge 55 versus 109 events are event counts largely attributed to infection, not proportions harmed by treatment. Nine withdrew before six-month follow-up. Single-strain challenge is distinct from natural-exposure diarrhea; microbiome/inflammation markers are not clinical protection.

U.S./next: Marketed as a dietary supplement, not FDA-approved travelers’-diarrhea prevention. Larger confirmatory human studies and an investigational enteric formulation are undated. Earlier sponsor topline dates remain historical.

Baseline recovery: TNP-2092 tests local antibiotic treatment for prosthetic-joint infection

Historical baseline, recovered October 13, 2026. NCT06889701 began April 3, 2025 (actual) and now lists recruiting, 33 planned participants: three sentinel patients, twenty randomized open-label participants and ten in an expansion cohort. TNP-2092 50 mg intra-articularly is studied alongside background antibiotics; randomized controls receive local vancomycin plus systemic/background antibiotics. Primary adverse-event safety runs through day 187; cure and prosthesis preservation require separate comparative evidence.

Limits/safety/U.S.: No results posted. This is a formal investigational antibiotic program, not established treatment or proven resistance reversal. Existing co-treatment limits attribution. Next: January 2027 primary/final completion estimates, no promised publication date.

Sources: ClinicalTrials.gov · NCT06889701.

Remaining gaps: All saved named registry records retrieved and all pages of bounded September5/6–October13 CT.gov discovery downloaded; 325 records screened selectively. Retrieval is not full appraisal. Worldwide registries, all publications/regulatory files, comprehensive harms and full audio/transcripts remain incomplete; prior full successful-check dates preserved.

Informal-source coverage: Targeted antibiotics/phage/vaccine Reddit and interview searches returned older studies and routine treatment anecdotes. No specific attributable new development rumor selected; full podcast/talk transcripts not appraised.

RSV study compares vaccine strategies after lung or stem-cell transplantation

First posted October 7, 2026. NCT07863401 plans 400 lung-transplant or allogeneic hematopoietic-stem-cell-transplant recipients in a randomized, open-label comparison of two RSV vaccination strategies. The record compares a single adjuvanted regimen with a two-dose nonadjuvanted recombinant prefusion-F regimen, with antibody-response assessment one month after completing vaccination in each transplant subcohort. The narrative and coded product names disagree, so exact product identity requires protocol clarification.

Limits/safety/U.S.: The narrative also refers to RSVpreF while intervention coding uses RSVpreF3; product identity needs protocol clarification, so no brand is assigned here. An antibody-rise endpoint is not measured protection from RSV hospitalization or death. No human outcomes or quantified harms are posted. This study does not create a new U.S. transplant-specific indication.

Next: Not yet recruiting; October 2026 start, June 2028 primary and May 2029 final completion are registry estimates at month precision. Public-result timing is unknown.

Primary registry.

Remaining gaps: All saved named NCT records refreshed and all pages of official overlap discovery screened (312 returned records). Search starts September 5/6 from the last full successful check, not October 6 alone. Incidental matches excluded; literature, international registries, all regulatory files and complete podcast/audio appraisal remain incomplete.; VAX-31 topline and earlier PLG0206/China RSV/TNP2092 events already saved and deduplicated. No actual STEPS-CF first dosing verified. Broader new-antibiotic human efficacy, regulator files, conference abstracts and long-acting infection-prevention census remain incomplete. Gepotidacin resistance paper has September 1 online date despite October issue; not relabeled new October news.

Informal-source coverage: Located Communicable E64 (October4, Emily McDonald/Marc Bonten with Sylvain Lother/Valentijn Schweitzer) and Children’s Colorado Charting Pediatrics October6 show notes. These discuss existing antibiotic evidence/ stewardship rather than a new-development claim; full audio unreviewed. Read original personalized-phage-guideline Reddit discussion; expert consensus is not randomized efficacy. No separate noteworthy new rumor selected.

VAX-31 meets primary immune-response endpoints, with a comparator-specific caveat

Source date: October 5, 2026. Evidence: Sponsor topline and SEC-filed slides; OPUS-1 NCT07284654 randomized 4,047 adults, including 3,572 aged ≥50 and 475 aged 18–49.

At one month, all 28 shared-serotype primary noninferiority assessments and four unique/cross-reactive superiority assessments passed. Primary analysis allowed PCV20 and/or PCV21 comparators with multiplicity control. Individual PCV21 comparisons passed 17/19 at the >0.667 confidence-bound margin; serotypes 3 and 12F only met the historical >0.5 margin. Younger-to-older immune bridging passed all 32 assessments.

Safety: In adults ≥50, serious adverse events were 14/1,194 VAX-31, 13/1,195 PCV20 and 13/1,183 PCV21. Four deaths occurred (two VAX-31, two PCV21), all assessed unrelated; no vaccine-related serious events or AE discontinuations were reported.

Limits/U.S.: Six-month safety and antibody function do not directly measure pneumonia/mortality prevention or prove superiority in every comparison. Investigational, not approved.

Next: OPUS-2/3 and infant phase 2 data H1 2027; adult BLA H1 2028, sponsor forecasts.

TNP-2092 adds an intra-articular infected-prosthesis trial

Source date: October 5, 2026 first posting. Evidence: NCT07858916, phase 2b design (registry phase 2), not yet recruiting in China.

Forty planned adults with prosthetic-joint infection receive intra-articular TNP-2092 with background infection care. Two acute-infection groups randomize daily 14-day versus weekly four-week dosing, ten per arm; up to 20 chronic-infection patients form a separate cohort. Primary outcome is six-month treatment failure, including infection-related death or additional treatment.

Limits/safety/U.S.: Open-label, no placebo/usual-care-only comparator, and no posted results. This cannot establish superiority, safety or eradication of biofilm. U.S. approval/access for this investigational regimen is not established.

Next: October 2026 start, September 2027 primary and March 2028 final completion are estimated.

China registers a large placebo-controlled RSV-vaccine trial

Source date: October 6, 2026 first posting; registry-reported actual start August 21. Evidence: NCT07860216, recruiting phase 3 randomized masked trial in China.

The study plans 25,000 adults aged ≥60, comparing a recombinant RSV vaccine produced in CHO cells with saline placebo. Primary outcome is first RT-PCR-confirmed RSV lower-respiratory disease over the first surveillance season.

Limits/safety/U.S.: No clinical results, adverse-event rates or demonstrated efficacy. A protocol’s assumed vaccine efficacy is a sample-size assumption, not a trial result. Approval of other RSV products does not authorize this formulation; U.S. approval is not established here.

Next: Primary/final completion December 31, 2028 is estimated; public readout timing unknown.

SOLARIO supports shorter systemic antibiotics when surgery includes local antibiotic implants

Source date: 2026-09-16. Event date: 2026-09-16. Evidence: Peer-reviewed randomized noninferiority trial.

For selected surgically managed orthopedic infections with local antibiotic carriers, a short systemic course was noninferior to a longer course. 500 adults were randomized; 475 entered primary analysis. At 12 months, failure occurred in 26/234 (11.1%) assigned ≤7 days versus 34/241 (14.1%) assigned ≥4 weeks. Difference −3.0 percentage points (95% CI −9.0 to 3.0), against a 10-point noninferiority margin. Trial: NCT03806166.

Limitations: Open-label treatment with blinded endpoint adjudication. Do not extrapolate to infections without implanted local antibiotics or to all orthopedic/hardware infections. September 16 publication recovered in this overlap search, not a September 27 result.

Safety: Potential treatment-related symptoms at six weeks were 17.2% versus 45.2% (difference −28.0 points, 95% CI −36.4 to −19.6). Full serious-event tables not appraised.

U.S. status: Treatment-duration evidence, not a new antibiotic approval.

ARIKAYCE broader-use application gets priority review

Source date: 2026-09-21. Event date: 2026-09-21. Evidence: Sponsor-reported FDA filing action.

FDA accepted Insmed’s supplemental application for full approval and broader MAC lung-disease use, with a January 28, 2027 action target. The application relies on phase 3b ENCORE: 425 participants with a new occurrence of MAC infection received ARIKAYCE plus azithromycin/ethambutol or placebo plus the same antibiotics. Respiratory symptoms and culture conversion are separate outcomes. Existing results are not newly generated by filing acceptance.

Limitations: Priority review is not approval; the release does not supply full numerical endpoint or safety tables.

Safety: The established product carries respiratory-risk warnings, including hypersensitivity pneumonitis, hemoptysis, bronchospasm and exacerbation of underlying lung disease; the sponsor reports no new ENCORE signal.

U.S. status: Current accelerated approval remains limited to adults with refractory MAC, limited alternatives and persistent positive cultures after at least six months of multidrug treatment. Earlier-line use remains under review.

Next milestone: January 28, 2027: sponsor-reported FDA action target, not guaranteed approval.

An antimicrobial peptide is being tested against prosthetic-joint infection

Source date: 2026-09-11. Evidence: Investigator enrollment report and trial registry.

WVU reported its first participant in RETAIN, a controlled trial of PLG0206 irrigation alongside surgery and antibiotics for infected knee replacements. NCT07214311 is a recruiting randomized placebo-controlled phase 2/3 study targeting 240 participants. It measures treatment failure through 12 months after debridement, antibiotics and implant retention, with surgery, survival and function also tracked. The registry lists a February 3, 2026 global start; September is a site-enrollment report, not global trial initiation.

Limitations: This newly added formal antimicrobial program addresses biofilm-associated infection, not cartilage regeneration. Laboratory activity and earlier small studies do not establish superiority, broad resistant-infection efficacy or implant retention benefit.

Safety: No new comparative efficacy or safety results were reported.

U.S. status: Investigational; Orphan/Fast Track/QIDP designations are not approval.

Next milestone: September 2028 primary completion is the September 18 registry estimate; public results timing is unknown.

WVU investigator update, September 11; RETAIN registry.

Review coverage and remaining gaps

Coverage: RETAIN investigator report and fresh registry; AP-SA02/PURPOSE365 registry refresh; antibiotic/antiviral/vaccine discovery. This is a focused review, not a complete re-review of every program.

Remaining gaps: June fosfomycin trial resurfaced in Sept14 commentary, not new September evidence; ENCORE original conference tables and all existing antivirals/vaccines/antibiotic launches not fully appraised.

Informal-source coverage: Informal Reddit/podcast searches were broad with poor original-source yield; no selected new attributable rumor, not an exhaustive no-rumor conclusion.

Full coverage record.

Next expected updates: AP-SA02 Phase 3 initiation H2 2026 and Utebzi availability by end-2026 are sponsor forecasts. Once-weekly oral lenacapavir has an announced FDA target of February 2, 2027. Vaccine postmarketing deadlines below are regulatory submissions, not promised public readouts.

Program / indication and population Development stage Evidence and limitations U.S. status Next milestone and timing basis
Zoliflodacin/Nuzolvence and gepotidacin/Blujepa; uncomplicated urogenital gonorrhea New oral antibacterial options; completed pivotal trials Zoliflodacin's 930-person trial, NCT03959527, had microbiological cure in 90.9% versus 96.2% with ceftriaxone/azithromycin in the FDA efficacy population. Nonassessable outcomes counted against cure; this is not superiority. Zoliflodacin approved December 12, 2025, age ≥12/weight ≥35 kg. Gepotidacin approved for this indication December 11, age ≥12/≥45 kg with limited/no alternatives; separately approved for specified uncomplicated UTIs. Resistance, treatment failures, site-specific evidence and actual access; no firm next date. FDA approvals, FDA trial.
Complicated UTI options: cefepime/zidebactam (Zaynich); oral tebipenem pivoxil (Utebzi) Approved combinations/formulations; continued resistant-infection research Zaynich's 530-person trial, NCT04979806, showed composite clinical/microbiological response of 89.0% versus 68.4% with meropenem; the primary population excluded meropenem-resistant pathogens. Oral carbapenem delivery is a formulation advance, not a new antibiotic class. Zaynich approved May 29, 2026 for adult cUTI/pyelonephritis caused by specified susceptible organisms; Utebzi approved June 17 for adult cUTI with limited/no alternative oral option. Utebzi U.S. availability by end-2026, sponsor forecast; verify launch. Zaynich resistant-pathogen clinical outcomes: no firm date. FDA label, FDA approval list, GSK June announcement.
AP-SA02 intravenous phage adjunct; complicated S. aureus bacteremia, diSArm, NCT05184764 Completed Phase 1b/2a; Phase 3 planned Sponsor/conference evidence: 42 Phase 2a patients, 29 phage plus best antibiotics versus 13 placebo plus antibiotics; early clinical-response signal, small and unequal groups. Investigational; Breakthrough Therapy designation announced September 14 adds to Fast Track/QIDP, none of which is approval. Phase 3 superiority-study start H2 2026, sponsor forecast; actual initiation and registry details need confirmation. Armata program/results; September 14 designation.
Lenacapavir/Yeztugo; HIV pre-exposure prevention Twice-yearly injection approved; weekly oral formulation under FDA review PURPOSE trials established strong prevention efficacy; do not transfer injection efficacy automatically to a new dosing schedule. Annual intramuscular formulation remains Phase 3 research, PURPOSE 365/NCT07047716. Twice-yearly regimen approved for sexually acquired HIV prevention in adults/adolescents ≥35 kg; weekly/annual formulations investigational. Weekly oral FDA target February 2, 2027, announced by sponsor June 15; annual readout unknown. Current label, filing/target.
mFlusiva/mRNA-1010; seasonal influenza vaccine, age ≥50 Approved August 5, 2026; confirmatory/postmarketing studies P304, NCT06602024, randomized 40,805 adults: relative efficacy 26.6% (95% CI 16.7–35.4) against symptomatic laboratory-confirmed flu versus standard-dose vaccine. Traditional approval ages 50–64; accelerated approval age ≥65 based on immune responses against high-dose comparator. Postmarketing safety protocol December 31, 2026; first confirmatory efficacy interim report December 31, 2027: FDA submission deadlines, not public-result promises. FDA regulatory review, product record.
Ensitrelvir/Xocova; COVID-19 post-exposure prophylaxis, SCORPIO-PEP Approved antiviral prevention indication FDA label: symptomatic infection through day 10 in 2.9% versus 9.0% with placebo among 2,041 baseline-negative evaluable participants; risk ratio 0.33 (95% CI 0.22–0.49). Approved in 2026 for age ≥12 after exposure; this U.S. indication does not establish approval for treating established COVID or long COVID. Resistance, effectiveness in current variants and additional treatment indications; no firm next date. FDA label.

Safety and interpretation. Gonorrhea approvals apply to urogenital infection; do not extrapolate to pharyngeal disease or every resistant isolate. Zoliflodacin has reproductive-toxicity precautions based on animal findings, including pregnancy testing and three-month contraception guidance for treated males with partners of reproductive potential. Antibiotic monitoring includes allergy, C. difficile and resistance selection; beta-lactam neurotoxicity and renal dosing matter for Zaynich/Utebzi. AP-SA02 is being tested with antibiotics, not as a routine replacement; the small study cannot settle mortality or uncommon harms. Sponsor reporting describes no treatment-related serious adverse events, but possible liver-enzyme elevation and hypersensitivity warrant follow-up.

Lenacapavir requires HIV testing because undetected infection and its prolonged drug tail can select resistance; injection reactions and delivery continuity remain relevant. Ensitrelvir has major CYP3A interactions, hypersensitivity and fetal-harm precautions. mFlusiva caused more transient local/systemic reactions than comparators; FDA identified unresolved safety questions, including a numerical imbalance in deaths of unspecified cause, for surveillance. The age ≥65 approval does not establish clinical superiority over high-dose vaccine; confirmatory efficacy remains required. These cautions are grounded in the labels/reviews above.

What we’re watching
  • Future antibiotics are explicitly in scope: new antibiotic classes/targets, new agents within existing classes, beta-lactamase-inhibitor combinations, oral alternatives to intravenous treatment, and formal human studies addressing multidrug-resistant bacteria. Discover new trials beyond the named programs; track failures, holds and development discontinuation as carefully as successes.
  • Resistant infections and adjuncts: clinical cure, survival, relapse, source control, resistance emergence, susceptibility testing, microbiome disruption and toxicity. Include phages, lysins and other adjuncts when supported by registered/planned human development or human research; separate investigational use from routine care.
  • Prevention: vaccines, long-acting antibodies and antiviral PrEP/PEP; distinguish prevented infection, symptomatic disease, hospitalization and transmission, with age/risk group, durability and variant/strain coverage explicit.
  • Antiviral treatment: new drugs and combinations for respiratory viruses, HIV, hepatitis and other important infections; separate prevention from treatment, viral-load changes from clinical recovery, and chronic-infection suppression from cure. Cross-link airway-infection overlap with allergy and airway disease.

2026-09-14 — Focused ad-hoc update

AP-SA02 phage therapy receives FDA Breakthrough Therapy designation. Armata announced on September 14 that FDA granted Breakthrough Therapy designation to intravenous AP-SA02, a fixed multi-phage treatment being developed alongside antibiotics for complicated MSSA or MRSA bloodstream infections. The designation builds on the existing phase 1b/2a diSArm study (NCT05184764); it is not a new efficacy readout or an approval. Armata still anticipates initiating a phase 3 superiority study in the second half of 2026. That start remains a forecast. Larger confirmatory results are needed to establish benefit and safety beyond the small earlier trial. Armata, September 14.

Next: Phase 3 superiority-study initiation remains anticipated in H2 2026; the September 14 designation announcement does not confirm that the trial has started.

Coverage: Focused phage/antimicrobial, antiviral and vaccine discovery searches included safety and failures. A conference notice concerning Insmed ENCORE was located, but its new numerical results were not appraised and were not promoted from the notice. Lenacapavir, mFlusiva, ensitrelvir and Utebzi were not individually fully reopened. Informal material, including an AI-generated herpes research summary, did not establish an additional sourced development. This partial refresh does not imply a comprehensive pathogen/pipeline check.

  • September 13, 2026 — selective baseline completed: Checked FDA approvals/labels/review documents, trial identifiers and sponsor updates, including June–September developments. Established six representative development areas and separated approval, actual availability, forecast starts and regulatory reporting deadlines. No exhaustive pathogen/pipeline census or comprehensive rumor search was completed; no rumor was added. Broader antibiotic discovery, vaccine failures and new human antiviral/adjunct entrants remain required future coverage. This baseline does not constitute an all-topic monitoring run.

September 13 scope-expansion record.
