Recovered ASN abstracts add 76-week pegcetacoplan follow-up
Source date unknown; checked October 27
A summary update checks selected findings. It does not mean every program, safety question or access detail was reviewed again. Source and event dates remain attached to their findings below.
Selected fields updated October 27. Historical finerenone T1D approval preserved and obsolete 404 Bayer citation replaced with FDA label; ravulizumab approval is source-dated October 21. Full-topic coverage remains incomplete.
Topic baseline checked coverage: Selective starting brief; source-access and evidence limits are documented in the topic page. Informal-source coverage was not exhaustive.
Read the dated research notesUltomiris receives accelerated U.S. IgA nephropathy proteinuria approval, with confirmatory kidney-function benefit pending. Recovered pegcetacoplan 76-week abstracts add uncontrolled extension data and baseline-scarring differences; PRESERVE registers post-transplant recurrence prevention without results.
Finerenone has a U.S. indication for adults with T1D-associated CKD to lower UACR with expected kidney-progression benefit. Ultomiris now has accelerated U.S. approval to reduce proteinuria in specified adults with primary IgAN; slowed kidney-function decline is not established. Indications and outcome evidence remain separate.
FDA accepts the finerenone application for CKD without diabetes, without a disclosed decision date; this is not approval. PYC-003 adds early ADPKD target engagement, not demonstrated kidney-function preservation.
ORIGIN 3 sponsor topline reports an annualized eGFR-slope difference of 5.0 mL/min/1.73 m² (95% CI 3.6–6.5) in IgA nephropathy; this is disease-specific evidence.
REMODEL’s primary MRI comparisons were null. Secondary markers had no multiplicity correction; adverse-event discontinuation was 12.7% versus 2.9%. These findings cannot independently establish less kidney failure.
FDA decision under review; no target date disclosed.
Selected findings from the dated research notes; not an exhaustive treatment guide.

AstraZeneca announced U.S. accelerated approval of Ultomiris (ravulizumab-cwvz) to reduce proteinuria in adults with primary IgA nephropathy at risk of progression. The current FDA label confirms this indication and explicitly says that slowing kidney-function decline has not been established. The October 21 announcement date is verified; the separate FDA decision-letter date was not recovered.
In the I CAN interim main cohort, 352 participants (176 per arm) could reach week 34. UPCR fell 46.6% with ravulizumab versus 5.6% with placebo: a 43.4% relative reduction versus placebo. The new NEJM report gives a 99% CI of 30.1–54.2%; the sponsor separately reports a 95% CI of 33.5–51.8%. These intervals are different analyses, not interchangeable.
The label's safety population was 239 versus 244, with serious adverse reactions in 11/239 versus 22/244 over a median 35 weeks; infusion reactions were 20/239 versus 12/244. This does not establish a safety advantage. The boxed warning concerns serious/fatal meningococcal infection, including risk despite vaccination, with REMS requirements. Confirmatory 106-week eGFR evidence remains pending; the registry's July 18, 2030 overall-completion estimate is not an announced results date.
U.S. status: FDA accelerated approval for specified adults with primary IgAN; not established to slow kidney-function decline.
Next milestones: I CAN confirmatory 106-week eGFR analysis: date unknown (Result timing not announced); I CAN overall study completion: 2030-07-18 (Registry estimate).

Source date: unknown; verified October 27, 2026. Recorded in this review: October 27, 2026.
Verified October 27; the abstracts do not disclose their first online publication dates. ASN schedules list October 24 for the overall analysis and October 22 for the adolescent analysis. Those dates do not independently verify delivery of the talks.
The VALIANT/VALE follow-up aligned participants by time on pegcetacoplan and had no concurrent placebo comparison at week 76. In the overall cohort (N=120), UPCR changed −64.1% (95% CI −71.2 to −55.2) and eGFR −1.4 mL/min/1.73m² (95% CI −6.6 to +3.9). The more scarred biopsy subgroup (IFTA 26–50%, n=23) had eGFR change −11.0 (95% CI −20.3 to −1.6), contrasting with +1.7 (−3.9 to +7.3) among IFTA ≤25% (n=94); a headline of universal stable kidney function would be misleading.
The adolescent analysis overlaps this cohort: 55 started VALIANT and 53 entered the aligned follow-up analysis. At week 76, UPCR changed −67.5% (95% CI −77.0 to −54.1) and eGFR +8.4 (SE 4.43). The SE is not a confidence interval. Abstract-level statements of no new safety signals lack detailed event counts; full slides/posters, attrition and longer-term harms remain gaps. Existing infection warnings and indication limits remain relevant.
U.S. status: Pegcetacoplan has a U.S. indication for specified C3G/primary IC-MPGN patients aged 12+; these uncontrolled follow-up abstracts do not expand it.
Next milestones: Detailed ASN posters/slides and harms tables: date unknown (Awaiting accessible original material).
Date precision: Original source date not established; checked October 27, 2026. This is a verification date, not a new publication date.

The Phase 3 PRESERVE / NCT07898774 protocol plans 30 adolescent/adult kidney-transplant recipients at high risk of recurrent C3 glomerulopathy or primary immune-complex MPGN. Pegcetacoplan is added to standard post-transplant care in a single-arm, unmasked study; the Phase 3 designation does not make this a randomized comparison.
The primary endpoint is clinically significant disease recurrence within two years. Eligibility includes prior allograft loss or investigator-assessed high recurrence risk and vaccination requirements for encapsulated bacteria. The trial is not yet recruiting; estimated start is January 21, 2027, with primary/final completion January 17, 2031.
This new prevention question is distinct from treatment of established proteinuria. No recurrence-prevention benefit, transplant survival benefit or new prophylaxis indication is established.
U.S. status: Existing pegcetacoplan C3G/IC-MPGN proteinuria approval does not establish a new transplant-recurrence-prevention indication.
Next milestones: PRESERVE estimated start: 2027-01-21 (Registry estimate); PRESERVE primary completion: 2031-01-17 (Registry estimate).

Registry tables first posted October 27 for FINE-ONE / NCT05901831 corroborate the previously recorded March 4, 2026 publication, rather than providing a new trial/readout. In 242 adults with type 1 diabetes and CKD (120/122 randomized), six-month UACR ratios to baseline were 0.66 (95% CI 0.60–0.73) with finerenone and 0.88 (0.79–0.98) with placebo. The between-group ratio was 0.75 (0.65–0.87), p=.0001. This remains an albuminuria surrogate, not measured prevention of kidney failure.
The treated safety populations were 119/122. The registry separately lists nonserious hyperkalemia in 10/119 versus 3/122, and serious hyperkalemia in 2/119 versus 0/122. These categories must not be collapsed without person-level overlap information or substituted for the paper's total hyperkalemia percentages. Overall serious events affected 14/119 versus 14/122. The collection window was first dose through three days after interruption/last dose, with median treatment 6.07 months. No new regulatory decision is established by this posting.
The FDA approval letter is signed September16,2026; the current September2026 label includes T1D-associated CKD, to reduce UACR with expected kidney-progression benefit. Preserve this historical approval and replace obsolete May pending-review wording; October27 is only the registry-results posting date.
U.S. status: FDA September2026 label confirms a T1D-associated CKD indication to reduce UACR, expected to reduce kidney-progression risk. The October27 registry posting does not alter that indication.

October 19 Biogen and Sobi announcements schedule 76-week VALIANT/VALE pegcetacoplan follow-up, including adolescent C3G/primary immune-complex MPGN evidence, at ASN Kidney Week. The adolescent oral slot was October 22 and the overall follow-up oral slot is October 24. The sponsors give conflicting clock times, so retain dates only. Their release titles promise efficacy/safety follow-up but do not provide a new analyzable sample, effect estimate, uncertainty interval or harms table.
October 23 follow-up: the October 22 window has elapsed, but the actual abstract/presentation tables were not retrieved in this bounded check. Do not assume delivery, positive results or updated effect sizes from the schedule alone. October 24 remains the next scheduled opportunity. Previously approved U.S. use for patients aged 12 and older with C3G/primary immune-complex MPGN is indication-specific; the existing serious encapsulated-bacterial-infection warning and REMS are separate from these presentation plans. This is not general chronic-kidney-disease approval.

October 8 Bayer announcement — filing acceptance. FDA accepted the supplemental application for finerenone/Kerendia in adults with CKD without diabetes receiving standard care. This is a regulatory milestone based on the existing randomized phase 3 FIND-CKD (NCT05047263) results, not a newly run trial or a newly approved indication.
For context, FIND-CKD included 1,584 adults, comparing finerenone with placebo on maximally tolerated ACE inhibitor/ARB treatment. Prior results showed a 0.7 mL/min/1.73 m²/year slower eGFR decline and a composite kidney/cardiovascular benefit; these earlier findings retain their original dates. Hyperkalemia remains a relevant harm.
U.S. status: Finerenone is not yet approved for CKD without diabetes. Existing diabetes-associated CKD and heart-failure indications remain separate. Neither the global nor U.S. release discloses an FDA decision target date. The U.S. webpage is headed October 9 but its release dateline is October 8; use October 8 for this event.

At day 14 after a single dose, seven patients with usable paired samples had a pooled urinary PC1/CD133 geometric mean fold-change of 1.46 (95% CI 1.11–1.92; paired log-transformed t-test p<0.05): three received 1.2 mg/kg and four 2.4 mg/kg. Small cohorts cannot establish dose response. This is target engagement, not demonstrated preservation of eGFR or avoidance of dialysis.
Exploratory three-month kidney-volume data involve 18 single-dose patients, graphically compared with illustrative historical untreated progression, not a randomized concurrent comparator. Source/event date: October 1, 2026. Evidence: peer-reviewed randomized mechanistic trial. REMODEL randomized 106 adults with type 2 diabetes and CKD (71 semaglutide, 35 placebo; 25 women) across eight countries for 52 weeks. The three coprimary MRI measures—oxygenation, global perfusion and T1 mapping—did not significantly differ. Secondary renal artery resistive index favored semaglutide (ratio 0.96, 95% CI 0.93–0.99), as did cortical diffusion (1.05, 1.01–1.09). Exploratory UACR was 40% lower (ratio 0.60, 0.41–0.88).
The analysis assumes treatment adherence; multiple comparisons had no multiplicity correction. Biopsy analyses involved 33 participants. These mechanistic/biomarker findings do not independently establish less dialysis or additive combination benefit. Adverse-event discontinuation was 9/71 (12.7%) versus 1/35 (2.9%); serious-event rates were similar. This paper creates no new U.S. indication; existing semaglutide indications are separate. Next mechanistic replication/publication date is unannounced.

Source date: October 5, 2026; actual study start: August 20. Evidence: newly posted older-start registration. A planned 1,500-person U.S. prevention study randomizes primary-care providers to patient letters, letters plus calls, or augmented usual care following a CKD risk-model alert. Its primary outcome is days to completion of either eGFR or UACR testing over 12 months; time to CKD diagnosis is secondary. RECRUITING; primary completion August 20, 2027 and final completion December 31, 2027 are estimates.
This is useful screening/implementation research, not a newly started October study or evidence that the model prevents kidney failure. Completing one test is not the same as evaluating both filtration and albuminuria. No benefit or safety results are posted; testing burden, false positives and downstream care remain prospective.

The September 23 Explore-CKD paper reports a randomized masked crossover trial in 59 adults with uncontrolled hypertension, CKD and albuminuria. Lorundrostat 25 mg daily was added to SGLT2 inhibition and ACE-inhibitor/ARB background treatment. Four weeks of treatment lowered office systolic blood pressure by 7.5 mmHg versus placebo; albuminuria also fell. This publishes a trial previously announced at topline in 2025, not a new trial launch. Four-week biomarker changes do not establish sustained kidney preservation or fewer dialysis events. Full adverse-event and interval tables were not appraised here, so comparative safety is unresolved in these notes. No new U.S. approval is established. Limitations: No hazard ratio, event counts or detailed safety analysis disclosed; do not invent a quantified harm signal. This older missing event is not a September 22 result.
Safety: Benefit-risk review supported stopping; detailed analyses ongoing. Other EASi trials were separately reviewed and continue unchanged.
U.S. status: Vicadrostat remains investigational. Empagliflozin's approved uses and established benefit-risk profile are unchanged.
Next milestone: Detailed analyses and final publication undated; registry administrative closeout estimate is not a result-release promise.

In 428 adults with primary IgA nephropathy, atacicept's final analysis reported slower filtration decline and fewer composite kidney-progression events. At 52 weeks eGFR change was −0.1 versus −5.7 mL/min/1.73 m²; difference 5.6 (95% CI 3.7–7.5). Annualized slope through 104 weeks: −0.6 versus −5.6; difference 5.0 (3.6–6.5). Composite progression: 11 versus 38 events; HR 0.24 (0.12–0.48). NCT04716231.
Limitations: Sponsor-only final analysis; detailed congress report pending. Historical registry still lists 376 estimated participants, superseded by 428 analyzed. These are IgAN findings, not all-cause CKD prevention.
Safety: Sponsor reports similar overall AE/infection rates and no opportunistic infections or clinically relevant hypogammaglobulinemia; detailed final-arm table absent. Existing label warns of immunosuppression/infection and injection reactions.
U.S. status: TRUTAKNA has accelerated approval for reducing proteinuria in adults with primary IgAN at progression risk; full approval is not yet established.
Next milestone: Supplemental BLA planned Q4 2026; potential full approval in 2027 is a sponsor expectation, not FDA commitment.

Akebia announced the first patient dosed in its Phase 2 trial across IgA nephropathy, lupus nephritis and C3 glomerulopathy. Ebribafusp, formerly AKB-097/ADX-097, is a tissue-targeted anti-C3d/factor-H fusion protein. NCT06419205: open-label, approximately 30 participants, weekly subcutaneous treatment for 26 weeks; primary safety endpoint, secondary UPCR/eGFR/PK. Registry lists actual study start July 30; September 14 is the announcement date.
Limitations: No patient efficacy result. The proposed infection-risk advantage over systemic complement blockade remains unproven.
Safety: Safety/tolerability is the primary question; earlier healthy-volunteer tolerability cannot establish safety in kidney disease.
U.S. status: Investigational.
Next milestone: Initial data in 2027, sponsor forecast; February 2028 registry completion estimate.
Akebia September 14 first-dose announcement; NCT06419205 registry.
Coverage: Recovered September 15 EASi-KIDNEY negative development and ORIGIN 3 final readout, September 14 ebribafusp first-dose announcement; finerenone/new kidney trial registries and BEAT-PKD gap.
Remaining gaps: Detailed final ORIGIN 3 arm-level safety/estimand/composite definitions need congress/publication; FDA full-approval action not yet occurred. EASi-KIDNEY registry remains active/not recruiting while sponsor announced stopping; status is administrative, not proof that decision reversed. BEAT-PKD estimated September 21 start elapsed while registry says not yet recruiting; actual dosing not verified. Kidney early-detection and global disease-specific census incomplete; full audio sources not reviewed.
Informal-source coverage: Read Reddit kidney/ADPKD/lupus trial discussions: self-identified VX-407 participant's mixed cyst-diameter anecdotes do not establish trial efficacy; trial-intake and transplant posts are leads, not outcome announcements. Primary sponsor/registry evidence used for included findings.
Next update: EASi-KIDNEY is stopped; other EASi programs must be tracked separately. Atacicept regulatory steps and broader CKD outcomes need independent confirmation.
Source date: 2026-09-17. Evidence: Regulatory action; September 2026 U.S. prescribing information verified.
The formerly pending type 1 diabetes indication is now approved. The label specifically describes UACR reduction expected to reduce sustained eGFR decline and end-stage kidney disease. FINE-ONE randomized 242 adults and measured albuminuria over six months. The new label infers kidney-event benefit from type 2 diabetes outcome trials together with pharmacodynamic and safety evidence in type 1 diabetes. The approval expands the indication; it does not represent a new trial directly demonstrating dialysis prevention.
Limitations: Bayer announced the approval on September 17; the dated FDA action letter was not retrieved. The indication should not be generalized to all non-diabetic CKD.
Safety: Hyperkalemia remains the principal monitoring issue; strong CYP3A4 inhibitors and adrenal insufficiency are contraindications. Pooled 14% versus 6.9% hyperkalemia figures in the label belong to type 2 diabetes trials, not FINE-ONE.
U.S. status: Approved in the U.S. for adults with CKD associated with type 1 diabetes.
Next milestone: Implementation and longer-term evidence; no firm date for the next readout was verified.

Source date: 2026-09-14. Evidence: New retrospective trial registration.
A newly posted human trial will be followed for results; it is not a new trial launch. NCT07819227 was first posted on September 14. The open-label, randomized phase 2 crossover study enrolled 41 participants, started on February 11, 2025 and completed on August 31, 2026. Its primary endpoint is the proportion achieving at least a 30% reduction in UACR during single-drug or combination treatment through week 37.
Limitations: Registered after completion, without posted results. Albuminuria endpoint is not kidney-failure prevention.
Safety: No trial results available.
U.S. status: No new combination indication established.
Next milestone: Publication or posted results undated.
NCT07819227 current primary registry.

Current position. Kidney preservation has established clinical-outcome evidence, with newer trials extending therapies beyond their original diabetic populations and studying combinations. The strength of evidence depends on the endpoint: lower albuminuria, slower filtration decline and fewer kidney-failure events are related but different findings. Earlier recognition is valuable to investigate, but a risk score needs evidence that acting on it improves care.
SGLT2 inhibitors — the established outcomes benchmark. EMPA-KIDNEY, NCT03594110, randomized 6,609 adults with CKD, including many without diabetes, to empagliflozin or placebo. Over a median two years, kidney-disease progression or cardiovascular death occurred in 13.1% versus 16.9%, hazard ratio 0.72 (95% CI 0.64–0.82). Serious adverse-event rates were similar. This is stronger clinical-outcome evidence than a short biomarker trial, but concerns participants with established CKD at risk of progression. Empagliflozin received its U.S. CKD indication in September 2023. Watch longer-term outcomes, underrepresented populations and additive benefit from combinations. Original trial, FDA CKD approval letter.
Finerenone / Kerendia — non-diabetic CKD expansion. The full FIND-CKD Phase 3 trial, NCT05047263, was published June 4, 2026. Among 1,584 participants, annual eGFR decline over 32 months was −3.3 versus −4.0 mL/min/1.73 m², a difference of 0.7 (95% CI 0.3–1.1). The secondary combined kidney/cardiovascular endpoint favored treatment, HR 0.77 (0.60–0.99), but the kidney-only event composite was 0.78 (0.60–1.01). Hyperkalemia occurred in 17.0% versus 13.3% and caused more discontinuations. This supports broader renal protection without establishing a statistically conclusive kidney-only event benefit in this trial. Peer-reviewed FIND-CKD report.
Bayer filed for a non-diabetic CKD indication in China on July 30, 2026. That filing is not approval, and no current U.S. non-diabetic-CKD decision date was verified. Finerenone's established U.S. indications include CKD associated with type 2 diabetes and heart failure with ejection fraction at least 40%; those must not be generalized to every kidney disease. July filing and indication status.
Finerenone in type 1 diabetes — FINE-ONE / NCT05901831. The March 4, 2026 peer-reviewed Phase 3 report randomized 242 adults with type 1 diabetes and CKD. Six-month urine albumin–creatinine ratio (UACR) fell 34% versus 12%, corresponding to a 25% greater relative reduction with finerenone. Hyperkalemia was 10.1% versus 3.3%; the initial eGFR reduction approached baseline after washout. UACR was the primary endpoint, so this is not a direct dialysis-prevention result. FDA accepted the supplemental application and granted priority review on May 21, 2026; the announcement provides no exact decision date. FINE-ONE paper, U.S. regulatory update.
Combination therapy — CONFIDENCE / NCT05254002. In the randomized trial published June 5, 2025, adults with type 2 diabetes and CKD already receiving renin–angiotensin-system inhibition received finerenone, empagliflozin or both. At 180 days, combination treatment reduced UACR 29% more than finerenone alone and 32% more than empagliflozin alone. Unexpected adverse events were not identified, but the short biomarker endpoint does not prove additive long-term kidney-failure prevention. Track clinical outcomes and potassium, blood-pressure and acute-kidney-injury safety. CONFIDENCE paper.
Earlier detection and prediction. KDIGO's evaluation framework distinguishes chronicity and cause, urine albumin and filtration, and recognizes that combined creatinine/cystatin-C eGFR improves accuracy. The research watch should assess who benefits from additional testing, false-positive referrals, external validation and whether test-directed care improves outcomes—not simply whether more abnormalities are detected. KDIGO evaluation summary.
KidneyIntelX.dkd provides a concrete prognostic-test example: FDA's De Novo decision DEN200052 evaluates a combination of TNFR-1, TNFR-2, KIM-1 and clinical variables for progression risk in adults with type 2 diabetes and existing CKD. It is not a universal healthy-population screening test. Its validation separates subsequent risk groups; that alone does not prove the test prevents kidney failure. Watch independent prospective validation and trials comparing care with versus without the test. FDA decision summary.
