Liver health and noninvasive assessment

A new placebo-controlled Kylo-0603 trial will measure MRI liver-fat reduction. Earlier senolytic biopsy signals and larger FGF21/THRβ programs remain distinct from proven reductions in liver complications; complete patient results and harms remain pending.

Where things stand

Established uses

Resmetirom has FDA accelerated approval for adults with noncirrhotic MASH and moderate-to-advanced fibrosis; confirmation of clinical benefit remains required.

In development

A new placebo-controlled Kylo-0603 trial will measure MRI liver-fat reduction. Earlier senolytic biopsy signals and larger FGF21/THRβ programs remain distinct from proven reductions in liver complications; complete patient results and harms remain pending.

Key finding

In ESSENCE, semaglutide achieved MASH resolution without worsening fibrosis in 62.9% versus 34.1% with placebo at 72 weeks.

Main uncertainty

TRUTH randomized only 31 people. Missing-biopsy sensitivity, more adverse events, diabetes imbalance and null noninvasive comparisons limit its biopsy signal; senolytic target engagement is not proof of longer life.

What comes next

NATiV3 Q4 2026 sponsor forecast; ESSENCE clinical-outcome follow-up ongoing. September CHMP outcome remains unverified.

Kylo-0603 registers a placebo-controlled liver-fat trial

NCT07878091 / Kylo-0603-IIa-01, first posted October 16. Shanghai Chia Tai Tianqing sponsors a masked randomized Phase 2a trial in 120 adults with MASLD and overweight/obesity in China, comparing daily oral Kylo-0603 8, 12 or 16 mg with placebo for 12 weeks.

Kylo-0603 is described as a liver-targeted GalNAc-conjugated thyroid-hormone receptor β agonist. Primary is percentage change in MRI-PDFF liver-fat content at week 12. Secondary measures include liver stiffness, LDL cholesterol, liver enzymes and other biomarkers. Neither liver-fat reduction nor a stiffness/LDL change alone establishes fibrosis reversal, fewer liver complications or cardiovascular benefit.

The record is not yet recruiting. Start November 15, 2026 and primary/final completion August 15, 2027 are estimates; no results or participant adverse-event rates are posted. Earlier healthy-volunteer work is historical context, not a new October efficacy result. No U.S. approval or availability is established.

NEBULA-1 tests clinical liver outcomes in compensated MASH cirrhosis

Baseline recovery, checked October 13 — NEBULA-1, NCT07701993 / GSK 306300. GSK's randomized masked placebo-controlled Phase 3 trial plans 1,740 adults aged 18–75 with compensated MASH cirrhosis. Efimosfermin alfa is an investigational FGF21 analogue given subcutaneously.

Primary is time to an adjudicated composite of all-cause mortality, liver transplantation and significant hepatic decompensation, over up to 356 weeks. Liver stiffness, ELF, metabolic measures, quality of life and harms are secondary; biomarker improvement alone will not prove this primary benefit.

The September 30 registry and original GSK protocol say recruiting, with actual start July 22, 2026. Estimated primary completion is June 23, 2033, final August 24, 2033. No results; U.S. treatment remains investigational. This earlier program fills a saved coverage gap rather than a new October launch.

A secondary CTIS mirror displays an October 12 EU authorization date and a different 1,468 planned total; the primary EMA linked search did not expose the decision details. That regional claim/count is not promoted as verified or substituted for the source-backed 1,740 protocol count.

September 5–October 13 overlap preserves the September 12 full-review dates; October 6/9 entries were deduplicated. All pages of each bounded ClinicalTrials.gov query were downloaded, and named records plus selected relevant new/changed protocols were appraised. This is focused partial review, not a worldwide full literature/regulator/safety census; successful full-topic dates must remain unchanged.; Bounded query retrieved 138 records across 2 pages; incidental studies excluded. Named topic records refreshed: 12. Full older evidence, international registries, comprehensive harms and all original conference/podcast recordings remain gaps.; EMA current Wegovy overview and available minutes still do not resolve the September MASH variation. September minutes expected after the October CHMP; no rejection/delay inferred from omission. Secondary CTIS mirror NEBULA authorization not independently established from primary EMA; record discrepancy retained. Worldwide fibrosis/noninvasive utility and complete FGF21 harms corpus remain incomplete.

Informal-source coverage: Revisited saved September 18 Reddit inference about Wegovy CHMP omission against current EMA page and available agendas/minutes. Small randomized senolytic MASH trial reports a biopsy signal and more adverse events

TRUTH / NCT05506488, published October 1; recovered in this October 9 overlap review. Amsterdam investigators randomized 31 people with fibrotic MASH (F2–F3) to intermittent dasatinib plus quercetin or placebo for 21 weeks.

Fibrosis improvement without MASH worsening occurred in 8/17 (47%) versus 1/14 (7%): risk difference 40 percentage points (95% CI 8.2–64.5), reported p=0.021. The reported risk ratio was 6.59 (95% CI 0.93–46.53); preserve this wide interval despite the different p-value method. MASH resolution was 9/17 (53%) versus 1/14 (7%), risk difference 46 points (95% CI 13.6–69.3).

Adverse events occurred in 82% versus 43%; one active participant stopped because of GI symptoms/fatigue. One serious event, planned knee surgery, was considered unrelated; severe dysphagia was attributed to previously undisclosed malignancy and that participant withdrew. Diabetes prevalence was imbalanced (76.5% versus 35.7%). Small size, short follow-up and biopsy surrogates limit inference: this does not establish fewer liver complications, a durable cure or lifespan extension. The combination has no U.S. MASH/longevity approval. Larger replication timing is undated.

Additional limitations: Four participants lacked follow-up biopsy and were counted as nonresponders in the primary analysis. A worst-case missing-data sensitivity gave RR 3.29 (95% CI 0.83–13.08), p=0.068. Noninvasive FIB-4, ELF and liver-stiffness comparisons were not significant. Histologic response is hypothesis-generating pending larger replication.

Sources: Primary source 1; Primary source 2.

Remaining gaps: Window: September 5–October 9, 2026, retaining the seven-day overlap from the last full successful check on September 12. October 6 findings were deduplicated.; 93 named NCT records across the ten topics and ten discovery queries downloaded successfully through the ClinicalTrials.gov public API. Download is not a claim that all fields/safety/results were exhaustively appraised.; Focused review only. Do not advance full successful-check timestamps or label incomplete remaining searches as no news.; Liver discovery returned 171 records; first 100 downloaded; selected relevant records screened, remaining page not reviewed. Updated saved RNA-interference protocols rechecked. September 29 TRIUMPH-1 full methods/CI access gap and worldwide noninvasive-screening/approval coverage remain.

Informal-source coverage: October 9 original r/Futurology post located: https://www.reddit.com/r/Futurology/comments/1x1lwvz/in_a_world_first_in_a_phase_2_trial_with_31/ . It frames histologic MASH resolution as a 50+% cure rate; original trial supports a short-term biopsy outcome, not proven durable cure. This is interpretation of a published paper, not a new clinical leak. Wider podcast audio and existing European Wegovy-MASH rumor not comprehensively refreshed.

ARO-INHBE adds a randomized MASH phase 2 plan

Source/event date: October 6, 2026. Evidence: new phase 2 registration. AROINHBE-2001 plans 252 adults with suspected MASH, BMI 30–50, with or without diabetes and with stable background GLP-1/GIP therapy allowed. Randomized double-masked ARO-INHBE dose/schedule groups are compared with matched placebo. The primary endpoint is MRI-PDFF liver-fat change at week 48; histologic MASH resolution and fibrosis improvement are secondary. NOT_YET_RECRUITING; September 2026 start is still estimated and elapsed. Primary/final completion is estimated October 2029. No human results or safety estimates are posted. Registration establishes a liver development plan, not proven fibrosis benefit, fewer complications or U.S. approval.

ALN-CIDEB registers a large MASLD study with primary safety endpoints

Source/event date: October 2, 2026. Evidence: new phase 3 registration. Regeneron’s ALN-CIDEB-MASLD-2604 plans 1,500 adults with MASLD and FibroScan liver stiffness 6–20 kPa. The randomized quadruple-masked study compares two ALN-CIDEB dose levels with placebo. Registered primary endpoints are incidence and severity of treatment-emergent adverse events through week 64; week-52 FibroScan fat/stiffness and liver enzymes are secondary. NOT_YET_RECRUITING, with November 6, 2026 estimated start and March 3, 2029 completion.

This registration does not show new human efficacy, clinical-event prevention, marketing approval or first dosing. MASLD inclusion by noninvasive stiffness is distinct from a biopsy-confirmed F2–F3 MASH pivotal population. The large phase designation does not convert biomarkers into clinical benefit; no results are posted.

New joint MASH guidance addresses resmetirom and semaglutide selection

Source/event date: 2026-09-28. Evidence: Professional-society clinical guidance; not a new trial.

The September 28 EASL–EASD–EASO guidance addresses selection, initiation, monitoring and stopping of resmetirom and weekly semaglutide 2.4 mg as MASH-targeted treatments. It recommends individualizing choice by metabolic disease, body composition, other conditions and preferences; the strength of the recommendation should not be mistaken for high-certainty comparative evidence. This is an interpretation of existing evidence, not a new randomized head-to-head trial or proof of reduced liver-related deaths. Existing U.S. indications remain separate from European review and access. The guidance does not resolve the previously tracked September CHMP outcome. Further clinical-outcome follow-up remains necessary.

Singapore adds a Wegovy indication for noncirrhotic MASH

Source date: 2026-09-23. Event date: exact date not disclosed. Date basis: Announcement date; exact HSA action date not independently verified. Evidence: Sponsor announcement of regulatory approval.

Novo announced HSA approval of weekly injectable semaglutide 2.4 mg for adults with noncirrhotic MASH and F2–F3 fibrosis, with diet and activity. The decision draws on the existing ESSENCE week-72 biopsy evidence, not a newly completed outcomes trial.

Limitations: Not an indication for cirrhosis; geographic approval does not imply local stock or reimbursement. Liver-fat or biopsy improvement is separate from preventing decompensation, transplant or death. HSA decision letter was not independently retrieved.

Safety: No new safety analysis; known gastrointestinal tolerability and indication-specific prescribing information remain relevant.

U.S. status: Existing U.S. MASH approval is separate and unchanged.

Next milestone: ESSENCE part 2 continues to 240 weeks for liver-related clinical events; this release does not give a new readout date.

Registry clarification: RESTORE-MASH is NCT07427680, a randomized, double-masked phase 1/2 study with 99 planned participants. The May 19 record lists recruiting, actual start March 3, 2026, estimated primary completion March 2028 and final completion June 2028; no results. This closes the identifier gap for the already recorded TGM-312 program.

Source date: 2026-09-21. Event date: 2026-09-21. Evidence: Sponsor development forecast.

Novo plans a Phase 3 MASH program for UBT251 and a dedicated efruxifermin alcohol-related liver disease trial during 2027. The CMD presentation places UBT251 in F2–F3 disease and efruxifermin in advanced fibrosis/compensated cirrhosis. Existing SYNCHRONY OUTCOME results are forecast around 2030.

Limitations: Plans do not establish initiation or clinical benefit in the new indications. Historical efruxifermin biopsy data displayed in the presentation are not new September results.

Safety: No new safety dataset for the planned programs.

U.S. status: No U.S. approval established for UBT251 or efruxifermin.

2026-09-02 — Older context added: NATiV3 reaches final patient visit. Inventiva’s September 2 announcement says the last NATiV3 participant completed the 72-week visit. The main cohort included 1,009 adults with noncirrhotic MASH and F2–F3 fibrosis, plus 410 exploratory participants. No efficacy or safety results were reported. Topline is sponsor-targeted for Q4 2026; a possible H1 2027 filing depends on results. Sponsor announcement. The July 2 NCT04849728 registry retains a September 2027 completion estimate encompassing an additional 48-week extension safety follow-up; this is distinct from the 72-week topline milestone. Its estimated enrollment of 1,000 differs from the later sponsor counts. Neither registry completion nor the final visit guarantees a publication date. Added September 14 as missing earlier context, not same-day news.

Coverage: Lanifibranor NATiV3 final patient visit, September 2, added as older missing development context. The registry includes extension safety follow-up beyond the 72-week milestone; Q4 topline remains a forecast. Estimated registry enrollment differs from the later sponsor count. Paris MASH announcements are not pivotal results. No systematic new biomarker-publication review. This was a focused refresh, not a complete re-review of every program.

Checked sources: Sponsor announcement; NCT04849728 registry; inventivapharma.com · source page; ir.madrigalpharma.com · news release.

Initial evidence brief — September 12, 2026

Current position. MASH now has approved therapies for selected noncirrhotic patients, while preventing decompensation and treating established cirrhosis remain major trial questions. Histologic improvement is encouraging but differs from fewer transplants, liver cancers or deaths. The diagnostic frontier is whether less invasive tests can reliably identify disease and treatment response, and ultimately help improve outcomes.

Pemvidutide PERFORMA phase 3 enrollment began in August

Source/event date: 2026-08-03. Date basis: Sponsor enrollment-initiation announcement. Evidence: Historical development baseline; not October news or a new efficacy readout.

Altimmune announced that enrollment had begun in PERFORMA, a global randomized, double-blind, placebo-controlled phase 3 trial of pemvidutide in MASH with moderate to advanced fibrosis. The program will assess fibrosis improvement, MASH resolution and clinical outcomes. Its current sponsor pipeline also describes active enrollment. This establishes a later MASH development stage than the older IMPACT phase 2b evidence; the obesity MOMENTUM results remain a separate indication and study.

Limitations and safety: Initiation does not establish efficacy, fewer liver complications or long-term safety. Histology and biomarkers remain distinct from clinical outcomes; the announcement is sponsor evidence, with no new outcome effect estimate. Breakthrough Therapy designation is not marketing approval.

U.S. status: Investigational; no pemvidutide approval established.

Next milestone: PERFORMA 52-week data forecast for 2029 by the sponsor, not a guaranteed publication or approval date.

Altimmune August 3 primary announcement; Current sponsor pipeline.

Rezdiffra / resmetirom — Madrigal, liver-directed thyroid-hormone-receptor beta agonist. FDA approved it in March 2024 for adults with noncirrhotic MASH and moderate-to-advanced fibrosis. MAESTRO-NASH, NCT03900429, is randomized and placebo-controlled; FDA's pivotal efficacy population contained 888 patients with F2–F3 fibrosis. The month-12 biopsy endpoints assessed MASH resolution without worse fibrosis and fibrosis improvement without worse MASH. Its accelerated approval requires confirmation of clinical benefit. Common adverse effects include gastrointestinal symptoms; liver toxicity, gallbladder-related events and interactions with statins also matter. FDA evidence and safety, July 2026 development/approval status.

Resmetirom in compensated cirrhosis — a major outcomes watch. MAESTRO-NASH-OUTCOMES, NCT05500222, completed enrollment in October 2024 with 845 participants. It is a blinded randomized comparison of resmetirom versus placebo, with liver-related clinical events as the primary endpoint. This is distinct from the original noncirrhotic indication. Madrigal's July 30, 2026 presentation targets results in 2027. Open-label cirrhosis findings and improvements in risk scores cannot substitute for that randomized result. Enrollment/design announcement, current 2027 target, slide 12.

Wegovy / semaglutide — approved liver indication, outcomes still pending. FDA granted accelerated approval on August 15, 2025 for noncirrhotic MASH with F2–F3 fibrosis. ESSENCE, NCT04822181, provides the Phase 3 evidence: at 72 weeks, MASH resolution without worsening fibrosis occurred in 62.9% versus 34.1%, and fibrosis improvement without worsening MASH in 37.0% versus 22.5%. These biopsy outcomes do not yet establish prevention of decompensation. Novo expects the longer outcomes portion to complete in 2029, a study-completion forecast rather than a publication deadline. Keep liver-specific follow-up here and cross-link obesity news. FDA approval letter, original ESSENCE paper, Q2 2026 timing, slide 42.

Efruxifermin / EFX — FGF21 analogue, now within Novo/Akero. The 181-person, placebo-controlled SYMMETRY Phase 2b trial, NCT05039450, studied compensated MASH cirrhosis. It missed the primary 36-week fibrosis endpoint. At 96 weeks, the higher-dose arm showed fibrosis improvement without worsening MASH in 29% versus 11%; that later secondary result is not a successful primary endpoint or proof of fewer clinical complications. Gastrointestinal adverse events were more common. Full SYMMETRY paper.

Phase 3 SYNCHRONY includes Histology (NCT06215716, F2–F3), Real-World (NCT06161571, noninvasively identified disease) and Outcomes (NCT06528314, cirrhosis). The current sponsor presentation confirms ongoing trials and exploration of GLP-1 combinations, but only describes readouts over coming years. No exact next EFX readout date is verified. It remains investigational in the U.S. Current development plan, slides 43–44, trial-program description.

Other human-stage mechanisms. Pegozafermin's Phase 3 ENLIGHTEN MASH program is an additional FGF21 watch. Madrigal is also developing combinations: its current pipeline targets a resmetirom/ervogastat interaction study in Q4 2026, then Phase 2 in 2027 following regulatory discussions. These are development plans, not combination efficacy or approval. Pegozafermin pipeline, Madrigal pipeline.

Noninvasive assessment — a separate research question. In August 2025, FDA accepted a letter of intent to qualify vibration-controlled transient-elastography liver stiffness as a reasonably likely surrogate endpoint in noncirrhotic MASH trials. This is the first qualification step, not completed validation or authorization to replace every biopsy. Watch the qualification plan, external validation, changes during treatment and prediction of actual complications. Better test accuracy alone does not demonstrate benefit from screening an unselected population; changes in stiffness also should not automatically be labeled fibrosis reversal. • Follow human therapies for metabolic dysfunction-associated steatotic liver disease (MASLD), steatohepatitis (MASH), fibrosis and cirrhosis, including new combinations and indications.
  • Track noninvasive diagnosis, fibrosis staging and response assessment using blood markers, elastography and imaging. Separate liver fat, inflammation, fibrosis, portal hypertension and clinical complications.
  • Cross-link GLP-1 and visceral/liver-fat research. Relevant informal claims remain eligible under the rumor policy.

Dated research history

2026-10-02 — Scheduled focused review

1 newly recorded findings or milestones; primary evidence, uncertainty and incomplete coverage are detailed above and in the run record. Full-topic successful-check dates remain unchanged.

  • 2026-09-29: 1 sourced entries, 0 cross-topic links and 1 informal-source checks. Coverage and gaps.

  • 2026-09-27: 0 sourced entries and 0 cross-topic links from a focused scheduled check. Coverage and remaining gaps.

  • 2026-09-22: Focused scheduled research retry; 1 sourced entries and 0 cross-topic links. Coverage and gaps.

Dated findings and coverage

  • Checked September 12, 2026: Selective baseline; sources include FDA decisions, trial publications and current sponsor materials. Registry pages did not consistently render full details, so no unverified registry completion date is used. Pegozafermin and comparative noninvasive-test performance need a deeper follow-up; informal-source coverage was incidental, with no rumor recorded in this baseline.

2026-09-14 — Ad-hoc focused review

Lanifibranor NATiV3 final patient visit, September 2, added as older missing development context. Original event dates and sources are recorded above; unchanged historical trial results remain dated. The registry includes extension safety follow-up beyond the 72-week milestone; Q4 topline remains a forecast. Estimated registry enrollment differs from the later sponsor count. Paris MASH announcements are not pivotal results. No systematic new biomarker-publication review.

2026-09-18 — Scheduled focused review

TGM-312 human development; NATiV3 and Paris MASH updates; discovery searches for new liver programs. The Duke resmetirom/HFpEF announcement was dated August 27, not September 16. Original event dates, safety and limitations are retained in the September 18 update above. The complete-topic successful-check date was not advanced because the listed gaps remain. Run record.
